Vairumā gadījumu žultspūsli izņem akmeņu dēļ. Taču ir situācijas, kad ultrasonogrāfijā akmeņu nav un pūslis tomēr jāizņem — un ir situācijas, kad cilvēku sūta uz operāciju, kaut vajadzības nav. Šajā rakstā — par abām.
Žultspūšļa polipi — vienīgā tēma ar skaidriem skaitļiem
Polips ultrasonogrāfijas aprakstā izbiedē gandrīz katru. Patiesībā lielākā daļa ir holesterīna polipi, nevis audzēji. Rīcību nosaka izmērs un riska faktori. 2022. gadā četras Eiropas asociācijas (ESGAR, EAES, EFISDS un ESGE) publicēja atjaunotas kopīgas vadlīnijas[2]:

Par riska faktoriem šajās vadlīnijās uzskata vecumu virs 60 gadiem, primāro sklerozējošo holangītu, Āzijas izcelsmi un polipu ar platu pamatni, tostarp fokālu žultspūšļa sienas sabiezējumu virs 4 mm.
Vadlīnijas pieļauj operāciju arī tad, ja polipu pavada sūdzības, ko varētu izraisīt žultspūslis, un citu cēloni neizdodas atrast. Šeit ir svarīgs nosacījums: pacientam iepriekš jāzina, ka sūdzības pēc operācijas var arī saglabāties.
Uz konsultāciju vērts paņemt visas iepriekšējās ultrasonogrāfijas, ne tikai pēdējo. Bez salīdzināšanas nav iespējams pateikt, vai polips aug. Taktiku nosaka izmērs, augšana un riska faktori kopā.
Akūts akalkulozs holecistīts — cita slimība ar tādu pašu nosaukumu
Šo bieži sajauc ar situāciju „pūslis sāp, bet akmeņu nav”. Tā ir pilnīgi cita slimība. Akūts akalkulozs holecistīts visbiežāk attīstās smagi slimam cilvēkam — pēc lielas operācijas, traumas, apdegumiem vai sepses, parasti intensīvajā terapijā. Ārpus reanimācijas tas ir retāk, bet iespējams.
Tur arī taktika ir cita: bieži vispirms veic žultspūšļa punkcijas drenāžu, nevis operāciju, jo pacients operāciju var nepārciest. Cik smagi šie pacienti ir, rāda skaitļi: pētījumā, kurā salīdzināja akalkulozu un akmeņu izraisītu holecistītu pēc drenāžas, 30 dienu mirstība akalkulozajā grupā bija 45%, un lielākajai daļai izdzīvojušo vēlāka žultspūšļa izņemšana vairs nebija nepieciešama[8].
Praktiskais secinājums pacientam: ja šāda diagnoze uzrakstīta ambulatori, ir vērts precizēt, kas tieši ar to domāts. Un, ja stāvoklis strauji pasliktinās, novērtēšanu atlikt nedrīkst.
Ir cilvēki ar tipiskām žults sāpēm, kuriem ultrasonogrāfija ir pilnīgi normāla. Tad izmanto scintigrāfiju ar žultspūšļa iztukšošanās frakcijas mērījumu: pazemināta frakcija ir hipokinēzija, ļoti augsta — hiperkinēzija.
Pierādījumu kvalitāte šajā jomā ir zemāka nekā akmeņu gadījumā, un to es pacientam pasaku. Pētījumi lielākoties ir retrospektīvi un nelieli. Piemēram, darbā par hiperkinētisku žultspūsli 48 pacientiem simptomi izzuda 96% gadījumu, taču tas ir retrospektīvs pētījums bez kontroles grupas[9].
Tāpēc operāciju šeit apspriežu tikai tad, ja vienlaikus pastāv trīs nosacījumi: sāpes patiešām atbilst žults kolikai, citi cēloņi ir izslēgti, un pacients saprot, ka garantiju šeit ir mazāk nekā akmeņu gadījumā.
Žilbēra sindroms — manā praksē viens no biežākajiem nepamatotajiem nosūtījumiem
Žilbēra sindroms nav žultspūšļa slimība. Tas ir iedzimts UGT1A1 gēna variants, kura dēļ aknās lēnāk pārstrādā bilirubīnu. Bilirubīns paaugstinās stresa, badošanās, saslimšanas vai miega trūkuma laikā. Tas nebojā aknas un nesaīsina dzīvi.
Žultspūšļa izņemšana Žilbēra sindromu neārstē. Tieksme uz paaugstinātu bilirubīnu pēc operācijas saglabāsies, un par to pacientam jāzina iepriekš — pretējā gadījumā cilvēks paliek bez žultspūšļa un ar tiem pašiem analīžu rādītājiem.
Vienlaikus saistība ar akmeņiem ir reāla, un to nedrīkst noliegt: UGT1A1*28 variants palielina žultsakmeņu risku[5], bet cilvēkiem ar hronisku hemolīzi akmeņi ir ievērojami biežāki un parādās jaunākā vecumā[6].
Kā es to vērtēju: Žilbēra sindroms bez akmeņiem — operācija nav vajadzīga. Žilbēra sindroms ar simptomātiskiem akmeņiem vai to komplikācijām — operē akmeņu dēļ, ne sindroma dēļ, un pacientam iepriekš pasaku, ka tieksme uz paaugstinātu bilirubīnu saglabāsies.
Hroniska hemolīze
Iedzimtas sferocitozes, sirpjveida šūnu anēmijas un citu hroniskas hemolīzes stāvokļu gadījumā pigmenta akmeņi veidojas agri, bieži jau jaunībā. Šeit žultspūšļa izņemšana nereti tiek plānota kopā ar liesas operāciju, un lēmums vienmēr jāpieņem kopā ar hematologu, nevis atsevišķi.
Kad ir aizdomas par audzēju
Porcelāna žultspūslis (kalcificēta siena) un fokāls sienas sabiezējums prasa atsevišķu izvērtēšanu. Mūsdienu dati par porcelāna pūsli rāda mazāku risku nekā vecās mācību grāmatas: lēmumu pieņem pēc kalcifikācijas veida, atradnes aizdomīguma un operācijas riska, nevis automātiski.
Sistemātiskā apskatā nejauši atklāts žultspūšļa vēzis konstatēts 0,36% holecistektomiju (dažādos pētījumos 0,19–1,6%), un tas ir aptuveni puse no visiem žultspūšļa vēža gadījumiem. Pirms operācijas to biežāk saistīja ar vecumu, sabiezinātu sienu, polipiem, porcelāna pūsli un paaugstinātiem aknu rādītājiem[7].
Kad es saku „neoperēsim”
Tas ir tikpat svarīgi kā saraksts ar indikācijām:
Duļķes (sludžs) bez sūdzībām — tā nav indikācija operācijai.
3–4 mm polips bez riska faktoriem — pēc vadlīnijām pat novērošana nav nepieciešama.
Žilbēra sindroms bez akmeņiem — pūslis nav vainīgs.
Nespecifiskas gremošanas sūdzības ar pilnīgi normālu žultspūsli — vēdera uzpūšanās, grēmas un rūgtums mutē visbiežāk nāk no citurienes. Ja izņemsim veselu žultspūsli, sūdzības paliks, bet žultspūšļa vairs nebūs.
Neatliekami jāvēršas pēc palīdzības, ja sāpes labajā paribē ilgst vairāk par 5–6 stundām, ir drudzis vai drebuļi, dzeltena āda vai acu baltumi, tumšs urīns vai atkārtota vemšana — neatkarīgi no tā, vai ultrasonogrāfijā akmeņi bija atrasti.
Kas jāņem līdzi uz konsultāciju
Lai lēmums nebūtu minējums, man vajag: visas ultrasonogrāfijas ar precīzi norādītiem polipu izmēriem, asins analīzes ar bilirubīnu un aknu rādītājiem un, ja tādi ir, iepriekšējos izmeklējumu aprakstus salīdzināšanai. Ar to parasti jau pirmajā vizītē var izvērtēt taktiku; daļā gadījumu vajadzīgi papildu izmeklējumi.
Biežākie jautājumi
Atrada 4 mm polipu. Kas jādara?Ja nav riska faktoru, pēc Eiropas vadlīnijām šādam polipam novērošana nav nepieciešama. Ja ir kāds riska faktors, ieteicama kontroles ultrasonogrāfija pēc 6 mēnešiem, gada un diviem gadiem.
Vai Žilbēra sindroma dēļ jāizņem žultspūslis?Nē. Žultspūšļa izņemšana Žilbēra sindromu neietekmē, un tieksme uz paaugstinātu bilirubīnu saglabājas. Pats sindroms nav indikācija; operē tikai tad, ja ir atsevišķa indikācija — piemēram, simptomātiski akmeņi vai aizdomīgs polips.
Ultrasonogrāfijā viss kārtībā, bet sāpes tipiskas. Vai vainīgs var būt žultspūslis?Var. Tad izmanto scintigrāfiju, mērot iztukšošanās frakciju. Taču pierādījumu bāze šeit ir vājāka, un par to es brīdinu pirms lēmuma pieņemšanas.
Vai duļķes žultspūslī jāoperē?Ja nav sūdzību — nē. Duļķes pašas par sevi nav indikācija operācijai.

Когда желчный пузырь нужно убирать, если камней нет
В большинстве случаев пузырь убирают из-за камней. Но есть ситуации, когда на УЗИ камней нет, а пузырь всё равно надо удалять, — и есть ситуации, когда человека отправляют на операцию без оснований. Здесь разберу и то и другое.
Полипы желчного пузыря — единственная тема с внятными цифрами
Слово «полип» в описании УЗИ пугает почти каждого. На деле большинство полипов — холестериновые, а не опухоли. Тактику определяют размер и факторы риска. В 2022 году четыре европейские ассоциации (ESGAR, EAES, EFISDS и ESGE) выпустили обновлённые совместные рекомендации[2]:

Факторами риска в этих рекомендациях считают возраст старше 60 лет, первичный склерозирующий холангит, азиатское происхождение и полип на широком основании, включая очаговое утолщение стенки пузыря более 4 мм.
Отдельно рекомендации допускают операцию и тогда, когда полип сочетается с жалобами, которые мог бы объяснить желчный пузырь, а другой причины найти не удаётся. Здесь важное условие: пациент заранее должен знать, что жалобы после операции могут и сохраниться.
На консультацию стоит взять все прежние УЗИ, а не только последнее. Без сравнения нельзя сказать, растёт полип или нет. Тактику определяют размер, рост и факторы риска вместе.
Острый бескаменный холецистит — другая болезнь с похожим названием
Это состояние часто путают с ситуацией «пузырь болит, а камней нет». Это совершенно другое заболевание. Острый бескаменный холецистит чаще всего развивается у тяжелобольного человека — после большой операции, травмы, ожогов, сепсиса, в отделении интенсивной терапии. Вне реанимации он встречается реже, но возможен.
И тактика там другая: часто сначала делают пункционное дренирование пузыря, а не операцию, потому что пациент может её не перенести. Насколько тяжелы эти больные, видно по цифрам: в исследовании бескаменного и калькулёзного холецистита после дренирования 30-дневная летальность в бескаменной группе составила 45%, а большинству выживших последующее удаление пузыря уже не потребовалось[8].
Практический вывод для пациента: если такой диагноз поставили амбулаторно, стоит уточнить, что именно имелось в виду. А при быстром ухудшении медлить нельзя.
Функциональные расстройства пузыря — самая спорная тема
Есть люди с типичной желчной коликой, у которых УЗИ полностью нормальное. Тогда применяют сцинтиграфию с измерением фракции выброса желчного пузыря: сниженная фракция — гипокинезия, очень высокая — гиперкинезия.
Качество доказательств в этой области ниже, чем при камнях, и я об этом пациенту говорю. Исследования в основном ретроспективные и небольшие. Например, в работе о гиперкинетическом пузыре у 48 пациентов симптомы ушли в 96% случаев, но это ретроспективное исследование без контрольной группы[9].
Поэтому операцию здесь я обсуждаю только при совпадении трёх условий: боль действительно похожа на желчную колику, другие причины исключены, и пациент понимает, что гарантий тут меньше, чем при камнях.
Синдром Жильбера — в моей практике частый повод для необоснованного направления
Синдром Жильбера — не болезнь желчного пузыря. Это врождённый вариант гена UGT1A1, из-за которого печень медленнее перерабатывает билирубин. Билирубин повышается при стрессе, голодании, болезни или недосыпе. Печень этот синдром не разрушает и жизнь не укорачивает.
Удаление желчного пузыря синдром Жильбера не лечит. Склонность к повышению билирубина после операции сохранится, и пациент должен знать это заранее — иначе человек остаётся без пузыря и с теми же цифрами в анализах.
При этом связь с камнями реальная, и отрицать её нельзя: вариант UGT1A1*28 повышает риск желчных камней[5], а у людей с хроническим гемолизом камни встречаются заметно чаще и появляются в более раннем возрасте[6].
Как я это различаю. Синдром Жильбера без камней — операция не нужна. Синдром Жильбера с симптомными камнями или их осложнениями — оперируем из-за камней, а не из-за синдрома, и я заранее говорю, что билирубин останется повышенным.
Хронический гемолиз
При наследственном сфероцитозе, серповидноклеточной анемии и других хронических гемолитических состояниях пигментные камни образуются рано, нередко уже в молодом возрасте. Здесь удаление пузыря часто планируют вместе с операцией на селезёнке, и решение принимаем совместно с гематологом.
Когда есть подозрение на опухоль
Фарфоровый желчный пузырь (кальцинированная стенка) и очаговое утолщение стенки требуют отдельной оценки. Современные данные о фарфоровом пузыре не так пугают, как старые учебники: решение принимают по типу кальцификации, подозрительности находки и операционному риску, а не автоматически.
В систематическом обзоре случайно выявленный рак желчного пузыря обнаружен в 0,36% холецистэктомий (в разных исследованиях 0,19–1,6%), и это примерно половина всех случаев рака желчного пузыря. До операции его чаще связывали с возрастом, утолщённой стенкой, полипами, фарфоровым пузырём и изменёнными печёночными пробами[7].
Когда я говорю «оперировать не будем»
Это не менее важно, чем список показаний:
Сладж без жалоб — это не диагноз для операции.
Полип 3–4 мм без факторов риска — по рекомендациям не требуется даже наблюдение.
Синдром Жильбера без камней — пузырь ни при чём.
Неспецифические жалобы на пищеварение при полностью нормальном пузыре — вздутие, изжога и горечь во рту чаще всего вызваны другим. Если убрать здоровый пузырь, жалобы останутся, а пузыря уже не будет.
Неотложно обратиться за помощью нужно, если боль в правом подреберье длится больше 5–6 часов, есть повышение температуры или озноб, желтизна кожи или белков глаз, тёмная моча или повторная рвота — независимо от того, находили камни на УЗИ или нет.
Что взять на консультацию
Чтобы принять обоснованное решение, мне нужны: все УЗИ с точно указанными размерами полипов, анализы крови на билирубин и печёночные пробы и, если есть, прежние описания исследований для сравнения. С этим обычно уже на первом визите можно определить тактику; в части случаев нужны дополнительные обследования.
Частые вопросы
Нашли полип 4 мм. Что делать?Если факторов риска нет, по европейским рекомендациям такому полипу не требуется даже наблюдение. Если фактор риска есть — контрольное УЗИ через 6 месяцев, год и два года.
Нужно ли удалять пузырь при синдроме Жильбера?Нет. Удаление пузыря на синдром Жильбера не влияет, склонность к повышению билирубина сохраняется. Сам синдром показанием не является; оперируют по другим причинам — например, при симптомных камнях или подозрительном полипе.
УЗИ чистое, но боль типичная. Может ли быть виноват пузырь?Может. Тогда применяют сцинтиграфию с измерением фракции выброса. Но доказательная база здесь слабее, и я предупреждаю об этом до принятия решения.
Надо ли оперировать сладж в пузыре?Если жалоб нет — нет. Сладж сам по себе не показание к операции.

In most cases the gallbladder is removed because of stones. But there are situations where the ultrasound shows no stones and the gallbladder still has to go — and situations where people are sent for surgery without good reason. This article covers both.
Gallbladder polyps — the one topic with clear numbers
The word "polyp" in an ultrasound report frightens almost everyone. In reality most are cholesterol polyps, not tumours. Management depends on size and risk factors. In 2022 four European associations (ESGAR, EAES, EFISDS and ESGE) issued updated joint guidelines[2]:

The risk factors in these guidelines are age over 60, primary sclerosing cholangitis, Asian ethnicity and a sessile lesion, including focal gallbladder wall thickening over 4 mm.
The guidelines also allow surgery when a polyp comes with symptoms the gallbladder could explain and no other cause is found. One point matters here: the patient must know in advance that the symptoms may persist after the operation.
Bring all your previous ultrasound reports to the consultation, not only the latest one. Without comparison it is impossible to say whether a polyp is growing. Size, growth and risk factors determine the plan.
Acute acalculous cholecystitis — a different disease with a similar name
This is often confused with "my gallbladder hurts but there are no stones". It is an entirely different condition. Acute acalculous cholecystitis usually develops in a severely ill person — after major surgery, trauma, burns or sepsis, typically in intensive care. Outside intensive care it is less common, but possible.
Management is different too: percutaneous drainage of the gallbladder often comes first rather than surgery, because the patient may not survive an operation. How ill these patients are is clear from the figures: in a study comparing acalculous and calculous cholecystitis after drainage, 30-day mortality in the acalculous group was 45%, and most survivors did not need the gallbladder removed later[8].
The practical conclusion for a patient: if that diagnosis appears in an outpatient report, ask exactly what was meant by it. And if things are deteriorating quickly, assessment must not wait.
Functional gallbladder disorders — the least certain territory
Some people have typical biliary pain with a completely normal ultrasound. Hepatobiliary scintigraphy is then used to measure gallbladder ejection fraction: a reduced fraction means hypokinesia, a very high one hyperkinesia.
The quality of evidence here is lower than for stones, and I say so. The studies are mostly retrospective and small. In one series on hyperkinetic gallbladder, symptoms resolved in 96% of 48 patients — but that was a retrospective study without a control group[9].
So I only discuss surgery here when three conditions are met at once: the pain really is biliary in character, other causes have been excluded, and the patient understands that results are less certain than with stones.
Gilbert syndrome — in my practice, one of the most common unjustified referrals
Gilbert syndrome is not a gallbladder disease. It is an inherited variant of the UGT1A1 gene that makes the liver process bilirubin more slowly. Bilirubin rises with stress, fasting, illness or lack of sleep. The syndrome does not damage the liver and does not shorten life.
Removing the gallbladder does not treat Gilbert syndrome. Bilirubin still tends to rise after the operation, and the patient needs to know this beforehand — otherwise they end up without a gallbladder and with the same blood results.
At the same time the link with stones is real and cannot be ignored: the UGT1A1*28 variant raises the risk of gallstones[5], and in those with chronic haemolysis, stones are considerably more common and appear at a younger age[6].
How I distinguish them: Gilbert syndrome without stones — no operation. Gilbert syndrome with symptomatic stones or their complications — we operate for the stones, not for the syndrome, and I say in advance that bilirubin will stay raised.
Chronic haemolysis
In hereditary spherocytosis, sickle cell disease and other chronic haemolytic conditions, pigment stones form early, often in young adulthood. Here removal of the gallbladder is frequently planned together with splenectomy, and the decision is always made with a haematologist rather than in isolation.
When cancer is suspected
A porcelain gallbladder (a calcified wall) and focal wall thickening call for separate assessment. Modern data on the porcelain gallbladder are less alarming than the older textbooks: the decision rests on the calcification pattern, suspicious features and operative risk; it is not automatic.
In a systematic review, incidental gallbladder cancer was found in 0.36% of cholecystectomies (0.19–1.6% across studies), and these incidental cases account for about half of all gallbladder cancers. Before surgery it was more often associated with age, a thickened wall, polyps, a porcelain gallbladder and abnormal liver tests[7].
When I advise against surgery
This matters just as much as the list of indications:
Sludge without symptoms — that is not a reason to operate.
A 3–4 mm polyp without risk factors — the guidelines do not even require follow-up.
Gilbert syndrome without stones — the gallbladder is not the problem.
Non-specific digestive complaints with a completely normal gallbladder — bloating, heartburn and a bitter taste usually come from elsewhere. Remove a healthy gallbladder, and the complaints remain.
Seek urgent help if pain in the right upper abdomen lasts more than 5–6 hours, if there is fever or chills, yellow skin or eyes, dark urine or repeated vomiting — whether or not stones were seen on ultrasound.
What to bring to the consultation
So that the decision is not made out of thin air, I need all your ultrasound reports with exact polyp measurements, bilirubin and liver enzyme tests, and any earlier imaging reports for comparison. With that, the first visit is usually enough to set the plan; in some cases further tests are needed.
Frequently asked questions
A 4 mm polyp was found. What now?Without risk factors, the European guidelines do not even require follow-up for a polyp this size. If a risk factor is present, follow-up ultrasound at 6 months, 1 year and 2 years is recommended.
Does Gilbert syndrome mean the gallbladder should be removed?No. Removing the gallbladder does not affect Gilbert syndrome, and bilirubin still tends to rise. The syndrome itself is not an indication; surgery is done for an independent reason — symptomatic stones or a suspicious polyp, for example.
My ultrasound is clear but the pain is typical. Can it still be the gallbladder?It can. Hepatobiliary scanning then measures the ejection fraction. But the evidence here is weaker, and this uncertainty should be considered before any decision is made.
Does gallbladder sludge require surgery?Not if there are no symptoms. Sludge on its own is not an indication for surgery.
