Konsultācijā par žultsakmeņiem gandrīz vienmēr izskan divi jautājumi: kāpēc tie radās tieši man un vai tiešām nav tabletes, kas tos izšķīdinātu. Uz abiem ir atbildes, un trekna pārtika nav pie vainas.
Akmeņi nav visi vienādi
Lielākā daļa akmeņu Eiropā ir holesterīna akmeņi. To rašanās nav „aizsērējums” no ēdiena, bet trīs procesu sakritība: žultī ir pārāk daudz holesterīna attiecībā pret žultsskābēm, žultspūslis saraujas pārāk gausi, un žultī parādās kristalizācijas centri. Šie procesi darbojas kopā, un jo vairāk to sakrīt, jo lielāka iespēja, ka akmens izveidosies.
Pigmenta akmeņi veidojas pavisam citādi — no bilirubīna sāļiem, galvenokārt kalcija bilirubināta. Tie parādās, ja sarkanās asins šūnas sadalās ātrāk nekā parasti vai ja bilirubīna pārstrāde aknās ir palēnināta. Tāpēc tie ir raksturīgi cilvēkiem ar hronisku hemolīzi un biežāk sastopami arī tiem, kam ir Žilbēra sindroms.
Praksē tas nozīmē: akmeņu veids nosaka, vai izšķīdināšana vispār ir teorētiski iespējama. Ar tabletēm var mēģināt ietekmēt tikai holesterīna akmeņus — pigmenta akmeņi nešķīst nekādos apstākļos.
Kas patiešām palielina risku
Par riska faktoriem raksta daudz, taču lielākā daļa sarakstu balstās uz novērojumiem, kuros cēloni no sakritības atšķirt nevar. Šeit noder ģenētiskie pētījumi: Mendeļa randomizācija izmanto iedzimtus gēnu variantus un ļauj tuvoties cēloņsakarībai. Lielā analīzē, kurā apvienoti UK Biobank un FinnGen dati, apstiprinājās, ka risku neatkarīgi ietekmē[3]:

Tajā pašā darbā alkohola lietošanas saistība ar akmeņiem neapstiprinājās. Vidukļa apkārtmērs izrādījās nozīmīgāks par pašu svaru — svarīgs ir tauku izvietojums, ne tikai to daudzums.
Papildus tam risku palielina sieviešu dzimums un estrogēni, grūtniecība, vecums, iedzimtība un vairākas zarnu slimības, kurās traucēta žultsskābju atpakaļuzsūkšanās (piemēram, Krona slimība vai tievās zarnas gala rezekcija).
Gēni, par kuriem pacienti nezina
Ja mātei vai māsai bija akmeņi, tā nav sakritība. Visbiežāk sastopamais zināmais ģenētiskais variants ir ABCG8 p.D19H — holesterīna transportētāja variants, kas palielina akmeņu risku aptuveni 1,8 reizes un darbojas jau bērnu vecumā[5]. Tajā pašā pētījumā apstiprinājās arī Žilbēra sindroma varianta UGT1A1*28 saistība ar akmeņiem.
Praksē tas nozīmē vienu: ja ģimenē akmeņi bijuši vairākiem, jūsu risks ir augstāks, un ar uzturu vien to pilnībā atsvērt nevar — lai gan dzīvesveids kopējo risku joprojām ietekmē.
Strauja svara zaudēšana — pašu radīts risks
Cilvēki to nereti rada paši, cenšoties kļūt veselāki. Strauji zaudējot svaru, aknas izdala vairāk holesterīna, bet žultspūslis saraujas retāk — un tā ir gandrīz ideāla vide, lai veidotos akmeņi.
Pētījumā ar 959 pacientiem pēc bariatriskās operācijas 8% pacientu divu gadu laikā radās simptomātiska žultsakmeņu slimība. Risks bija augstāks tiem, kam akmeņi bez sūdzībām bija atrasti jau pirms operācijas. Ursodeoksiholskābes profilakse akmeņu veidošanos samazināja aptuveni uz pusi[4].
Tāpēc, plānojot lielu svara zaudēšanu, es iesaku divas lietas: darīt to pakāpeniski un neizlaist maltītes. Ilga badošanās nozīmē, ka žultspūslis stundām ilgi nesaraujas.
Vai akmeņus var izšķīdināt ar tabletēm
Teorētiski jā, praksē — reti. Ursodeoksiholskābe var izšķīdināt akmeņus, taču tikai tad, ja vienlaikus pastāv visi nosacījumi: akmeņi ir holesterīna, mazāki par 5 mm, bez kalcija un rentgenā nav redzami, žultspūslis strādā, un žultspūšļa vads ir caurejams. Pašas EASL vadlīnijas šo metodi kā standarta ārstēšanu neiesaka[1].
Ar to problēmas nebeidzas:
ārstēšana ilgst mēnešiem, nereti gadu vai ilgāk;
tā jāturpina bez pārtraukuma;
pēc pārtraukšanas akmeņi bieži veidojas atkārtoti, jo pati vielmaiņa nav mainījusies;
žultspūslis, kurā akmeņi jau bijuši, paliek izmainīts.
Savā praksē cilvēku, kas atbilst visiem kritērijiem vienlaikus, es sastopu ļoti reti. Ja jums to piedāvā kā vienkāršu alternatīvu operācijai, ir vērts pajautāt, kāpēc tā nolemts.
Kas nedarbojas
Šie jautājumi konsultācijās atkārtojas visbiežāk.
„Aknu un žultspūšļa tīrīšana” ar eļļu un citronu. Tas, kas pēc tam parādās izkārnījumos un tiek uzskatīts par akmeņiem, ir mīksti ziepjveida veidojumi, kas rodas no pašas eļļas zarnās. Īsti akmeņi paliek vietā, un to var pārbaudīt ar ultrasonogrāfiju nākamajā dienā.
Žultsdzenošie līdzekļi akmeņu „izvadīšanai”. Tas var reāli kaitēt: piespiedu žultspūšļa saraušanās var iedzīt akmeni žultsvadā, un cilvēks nonāk slimnīcā ar dzelti vai pankreatītu. Nevienam no šiem līdzekļiem nav pierādīts, ka tas izvada akmeņus.
Akmeņu drupināšana ar triecienviļņiem. Nierakmeņiem tā darbojas, žultsakmeņiem — praktiski vairs netiek lietota: žultspūslis paliek nemainīts, un akmeņi atgriežas.
Kas tiešām samazina risku
Pakāpeniska svara samazināšana, nevis strauja.
Regulāras maltītes — žultspūslim vajag sarauties vairākas reizes dienā.
Fiziskā aktivitāte un vidukļa apkārtmēra samazināšana.
Smēķēšanas atmešana.
Kafija — ģenētiskajos datos tā saistīta ar zemāku risku[3].
Un tas, ko šie pasākumi nedara: jau izveidojušies akmeņi nepazūd. Mazāk veidojas jauni, bet esošie paliek.
Nekavējoties jāvēršas pēc palīdzības, ja sāpes labajā paribē ilgst vairāk par 5–6 stundām, pievienojas drudzis, āda vai acu baltumi kļūst dzelteni, parādās tumšs urīns vai atkārtota vemšana. Tās vairs nav vienkārši „akmeņu sāpes” — tas var būt žultspūšļa iekaisums, akmens žultsvadā vai pankreatīts.
Ko no tā secināt
Akmeņi rodas no vielmaiņas un ģenētikas, ne no vienām neveiksmīgām vakariņām. Izšķīdināt tos gandrīz nekad nav reāli. Tāpēc īstais jautājums nav „kā izvadīt akmeņus”, bet gan „vai šie akmeņi man traucē un ko ar tiem darīt tālāk”. Uz to atbildam konsultācijā, apskatot ultrasonogrāfiju un analīzes.
Biežākie jautājumi
Vai žultsakmeņus var izšķīdināt ar tabletēm?Tikai retos gadījumos. Nepieciešami nelieli holesterīna akmeņi bez kalcija, strādājošs žultspūslis un caurejams žultspūšļa vads. Ārstēšana ilgst mēnešiem, un pēc tās pārtraukšanas akmeņi bieži veidojas atkārtoti.
Vai „aknu tīrīšana” ar eļļu un citronu izvada akmeņus?Nē. Tas, kas parādās izkārnījumos, ir mīksti ziepjveida veidojumi no pašas eļļas, nevis akmeņi. To var pārbaudīt: nākamajā dienā ultrasonogrāfijā akmeņi ir tur, kur bija.
Ja mātei bija akmeņi, vai tie būs arī man?Risks ir augstāks. Zināmi vairāki gēnu varianti, tostarp ABCG8 p.D19H, kas palielina risku aptuveni 1,8 reizes. Tas nenozīmē, ka akmeņi būs obligāti, bet uzturs to pilnībā neatsver.
Vai kafija tiešām pasargā?Ģenētiskajos datos kafija saistīta ar zemāku žultsakmeņu risku. Tas nav iemesls sākt dzert kafiju ārstēšanas nolūkā, taču atteikties no tās akmeņu dēļ nav pamata.

Почему образуются желчные камни и можно ли их растворить
На консультации по поводу камней почти всегда звучат два вопроса: почему они появились именно у меня и правда ли нет таблетки, которая их растворит. На оба есть ответы, и ни один не звучит как «виновата жирная пища».
Камни бывают разные
Большинство камней в Европе — холестериновые. Они образуются не от «засорения» едой, а при совпадении трёх процессов: в желчи слишком много холестерина по отношению к желчным кислотам, пузырь сокращается вяло, и в желчи появляются центры кристаллизации. Эти процессы действуют вместе: их сочетание повышает риск образования камня.
Пигментные камни устроены иначе — они из солей билирубина, в основном из билирубината кальция. Они появляются, когда эритроциты разрушаются быстрее обычного или когда переработка билирубина в печени замедлена. Поэтому они характерны для людей с хроническим гемолизом и чаще встречаются при синдроме Жильбера.
Практический смысл: от типа камня зависит, возможно ли растворение в принципе. Таблетками можно пытаться повлиять только на холестериновые камни — пигментные не растворяются ни при каких условиях.
Что действительно повышает риск
О факторах риска пишут много, но большая часть списков построена на наблюдениях, где причину от совпадения не отличить. Здесь помогает менделевская рандомизация: по врождённым различиям в генах она отделяет причину от совпадения. В крупном анализе, объединившем данные UK Biobank и FinnGen, независимыми факторами были[3]:

снижает риск
В той же работе связь с алкоголем не подтвердилась. Окружность талии оказалась значимее самого веса — важно, где расположен жир, а не только сколько его.
Дополнительно риск повышают женский пол и эстрогены, беременность, возраст, наследственность и ряд заболеваний и состояний, при которых нарушено обратное всасывание желчных кислот (например, болезнь Крона или резекция конечного отдела тонкой кишки).
Гены, о которых пациенты не знают
Если камни были у матери или сестры — это не совпадение. Самый распространённый известный вариант — ABCG8 p.D19H, вариант транспортёра холестерина, который повышает риск камней примерно в 1,8 раза, и это проявляется уже в детстве[5]. В той же работе подтвердилась и связь с вариантом UGT1A1*28, отвечающим за синдром Жильбера.
На практике это значит одно: если камни были у нескольких родственников, ваш риск выше, и одним правильным питанием его не устранить — хотя образ жизни на общий риск всё равно влияет.
Быстрое похудение: рукотворный фактор риска
Люди нередко создают его себе сами, пытаясь стать здоровее. При быстрой потере веса печень выделяет больше холестерина, а пузырь сокращается реже — почти идеальные условия для образования камней.
В исследовании с участием 959 пациентов после бариатрической операции у 8% в течение двух лет развилась симптомная желчнокаменная болезнь. Риск был выше у тех, у кого камни без жалоб находили ещё до операции. Приём урсодезоксихолевой кислоты снижал образование камней примерно вдвое[4].
Поэтому при планах на серьёзное похудение я советую две вещи: снижать вес постепенно и не пропускать приёмы пищи. Долгое голодание означает, что пузырь часами не сокращается.
Можно ли растворить камни таблетками
Теоретически да, на практике — редко. Урсодезоксихолевая кислота способна растворить камни, но только если одновременно выполняются все условия: камни холестериновые, меньше 5 мм, без кальция и рентгенонегативные, пузырь работает, а пузырный проток проходим. Сами рекомендации EASL не советуют этот метод как стандартное лечение[1].
На этом сложности не заканчиваются:
лечение длится месяцами, нередко год и дольше;
принимать нужно непрерывно;
после отмены камни часто образуются заново, потому что сам обмен веществ не изменился;
пузырь, в котором были камни, остаётся изменённым.
В своей практике человека, который подходит сразу по всем критериям, я встречаю очень редко. Если вам предлагают это как простую альтернативу операции, стоит спросить, по каким именно критериям принято такое решение.
Что не работает
Эти вопросы на консультациях повторяются чаще всего.
«Чистка печени и желчного» маслом с лимоном. То, что потом появляется в стуле и принимается за камни, — мягкие мылоподобные комки, которые образуются из самого масла в кишечнике. Настоящие камни остаются на месте, и это можно проверить на УЗИ на следующий день.
Желчегонные для «изгнания» камней. Здесь есть реальный вред: принудительное сокращение пузыря может загнать камень в проток, и вместо избавления человек попадает в больницу с желтухой или панкреатитом. Ни одно из этих средств не выводит камни.
Дробление камней ударной волной. Для почечных камней метод работает, для желчных практически не используется: пузырь со всеми его недостатками остаётся, и камни возвращаются.
Что действительно снижает риск
Постепенное снижение веса, а не быстрое.
Регулярные приёмы пищи — пузырю нужно сокращаться несколько раз в день.
Физическая активность и уменьшение окружности талии.
Отказ от курения.
Кофе — по генетическим данным связан с более низким риском[3].
И то, чего эти меры не делают: уже образовавшиеся камни от них не исчезают. Они снижают риск новых камней, а не число имеющихся.
Немедленно обратиться за помощью нужно, если боль в правом подреберье длится больше 5–6 часов, присоединяется температура, появляется желтизна кожи или белков глаз, тёмная моча или повторная рвота. Это уже не просто «боль от камней» — это может быть воспаление пузыря, камень в протоке или панкреатит.
Какой из этого вывод
Камни возникают из-за особенностей обмена и генетики, а не от одного неудачного ужина. Растворить их почти никогда не удаётся. Поэтому настоящий вопрос не «как вывести камни», а «мешают ли мне эти камни и что с ними делать дальше». На него я отвечаю на консультации, глядя на УЗИ и анализы.
Частые вопросы
Можно ли растворить камни таблетками?Только в редких случаях. Нужны мелкие холестериновые камни без кальция, работающий пузырь и проходимый пузырный проток. Лечение длится месяцами, а после отмены камни часто образуются заново.
Правда ли, что «чистка печени» маслом с лимоном выводит камни?Нет. То, что появляется в стуле, — мягкие мылоподобные комки из самого масла, а не камни. Это легко проверить: на следующий день на УЗИ камни там же, где были.

Why gallstones form and whether they can be dissolved
Two questions come up at almost every gallstone consultation: why did this happen to me, and is there really no tablet that dissolves them? Both have answers, and neither is "you ate too much fat".
Not all stones are the same
Most stones in Europe are cholesterol stones. They do not form because food clogs something up; they form when three processes coincide: bile carries too much cholesterol relative to bile acids, the gallbladder empties sluggishly, and crystallisation centres appear in the bile. These processes work together: the more that coincide, the more readily stones form.
Pigment stones are different — they are made of bilirubin salts, mostly calcium bilirubinate. They appear when red blood cells break down faster than normal, or when the liver processes bilirubin more slowly. That is why they are typical of people with chronic haemolysis and are more common in Gilbert syndrome.
Why this matters: the stone type determines whether dissolution is even theoretically possible. Tablets can only act on cholesterol stones — pigment stones do not dissolve under any circumstances.
What genuinely raises the risk
Risk factor lists are everywhere, but most come from studies that cannot separate cause from coincidence. Genetic studies help here: Mendelian randomisation uses inherited gene variants and gets closer to causality. In a large analysis combining UK Biobank and FinnGen data, the following independently affected risk[3]:

In the same study, alcohol showed no association. Waist circumference mattered more than weight itself — where the fat sits counts, not only how much of it there is.
Female sex and oestrogens, pregnancy, age, family history and several bowel conditions that disturb bile acid recycling (Crohn disease or resection of the terminal ileum, for example) add further risk.
The genes patients do not know about
If your mother or sister had stones, that is not a coincidence. The most common known variant is ABCG8 p.D19H, a cholesterol transporter variant that raises the risk roughly 1.8-fold and does so already in childhood[5]. The same study also confirmed the link with UGT1A1*28, the variant behind Gilbert syndrome.
In practice this means one thing: if several relatives had stones, your risk is higher, and diet alone cannot eliminate it — though lifestyle still shapes the overall risk.
Rapid weight loss — a risk factor people create themselves
People often create this risk themselves while trying to get healthier. During rapid weight loss the liver secretes more cholesterol while the gallbladder contracts less often — close to ideal conditions for stone formation.
In a study of 959 patients after bariatric surgery, 8% developed symptomatic gallstone disease within two years. Risk was higher in those who already had asymptomatic stones before the operation. Taking ursodeoxycholic acid roughly halved stone formation[4].
For major weight loss, I advise two things: lose weight gradually and do not skip meals. A long fast means the gallbladder does not contract for hours.
Can stones be dissolved with tablets
In theory, yes; in practice, rarely. Ursodeoxycholic acid can dissolve stones, but only when every condition is met at once: the stones are cholesterol stones, under 5 mm, non-calcified and radiolucent, the gallbladder still works, and the cystic duct is open. The EASL guidelines themselves do not recommend this as standard treatment[1].
The difficulties do not end there:
treatment takes months, often a year or longer;
it has to be continuous;
stones frequently re-form after it stops, because the underlying metabolism has not changed;
a gallbladder that has made stones remains prone to making them.
In my own practice I very rarely see someone who meets all the criteria at once. If this is offered to you as a simple alternative to surgery, it is worth asking which criteria the decision was based on.
What does not work
These are the questions that come up most often.
"Liver and gallbladder flushes" with oil and lemon. What later appears in stool and is mistaken for stones is soft, soap-like material formed from the oil itself in the gut. The real stones stay where they were, and an ultrasound the next day will show them.
Bile stimulants to "expel" stones. There is real harm here: forcing the gallbladder to contract can drive a stone into the bile duct, and instead of relief the person ends up in hospital with jaundice or pancreatitis. None of these remedies is a proven way to clear stones.
Shock wave lithotripsy. It works for kidney stones; for gallstones it is essentially no longer used — the gallbladder and all its problems remain, and the stones come back.
What actually lowers the risk
Gradual weight loss rather than rapid.
Regular meals — the gallbladder needs to contract several times a day.
Physical activity and a smaller waist circumference.
Stopping smoking.
Coffee — linked to a lower risk in the genetic data[3].
And what these measures do not do: stones that have already formed do not disappear. They reduce the formation of new stones, not the number of existing ones.
Seek help immediately if pain in the right upper abdomen lasts more than 5–6 hours, if fever develops, or if you notice yellow skin or eyes, dark urine or repeated vomiting. That is no longer simple "stone pain" — it may be inflammation of the gallbladder, a stone in the bile duct, or pancreatitis.
What this adds up to
Stones come from metabolism and genetics, not from one unlucky dinner. Dissolving them is almost never realistic. So the real question is not "how do I flush the stones out" but "are these stones causing me trouble, and what should be done about them". That is what we answer at the consultation, looking at your ultrasound and blood tests.
Frequently asked questions
Can gallstones be dissolved with tablets?Only in rare cases. It requires small cholesterol stones without calcium, a working gallbladder and an open cystic duct. Treatment takes months, and stones often re-form once it stops.
