Lielākā daļa cilvēku ar žultsakmeņiem nekad nejūt neko. Bet, ja lēkme sākas, cilvēks parasti to atceras uz visu mūžu — un pēc tam nezina, vai tas bija kaut kas nopietns vai vienkārši „nepiemērots ēdiens”. Rakstā skaidrots, kā to atšķirt.
Kā izskatās īsta žults kolika
Nosaukums maldina: vārds „kolika” liek domāt par viļņveida sāpēm, kas nāk un iet. Patiesībā žults kolika ir noturīgas sāpes, kas dažu desmitu minūšu laikā pieaug un tad nemainās.
Kur: labajā paribē vai zem krūšu kaula. Bieži izstaro uz muguru, starp lāpstiņām vai uz labo plecu.
Cik ilgi: parasti no 30 minūtēm līdz vairākām stundām. Sāpes, kas ilgst piecas sekundes vai nemainīgi visu dienu, parasti nav žults kolika.
Kad: bieži pēc treknas maltītes, nereti naktī vai agri no rīta.
Ar ko kopā: slikta dūša, dažreiz vemšana, svīšana, nemiers. Cilvēks nevar atrast pozu, kurā būtu vieglāk.
Galvenā atšķirība no visa pārējā: kolika beidzas. Akmens, kas aizsprostoja izeju no žultspūšļa, atslīd atpakaļ, un sāpes pāriet — bieži tikpat pēkšņi, kā sākās. Ja sāpes nepāriet vairākas stundas, tā vairs nav vienkārša kolika.
Ar ko to visbiežāk sajauc
Šis ir iemesls, kāpēc cilvēki gadiem ārstē „gastrītu”, kamēr īstais cēlonis ir žultspūslis — un otrādi, kāpēc dažiem izņem žultspūsli, bet sāpes paliek.

Sāpes mainās pēc vēdera izejas, ir vēdera pūšanās, nav nakts lēkmju
Aizkuņģa dziedzera iekaisums
Sāpes stiprākas, jostveida, izstaro uz muguru, ilgst ilgāk, stāvoklis pasliktinās
Sirds
Spiediens krūtīs slodzes laikā, elpas trūkums — šeit kavēties nedrīkst
Atsevišķi par to, ko žultsakmeņi neizraisa: pastāvīga vēdera pūšanās, rūgtums mutē no rīta, atsevišķu produktu nepanesība un „smaguma sajūta” bez lēkmēm. Šīs sūdzības ir biežas, bet cēlonis parasti ir cits, un žultspūšļa izņemšana tās nenovērš.
Izmeklējumi: kas tiešām vajadzīgs
Ultrasonogrāfija — pamata izmeklējums. Akmeņus žultspūslī tā rāda ļoti ticami. Uz izmeklējumu jāierodas tukšā dūšā, citādi sarāvies žultspūslis akmeņus var noslēpt.
Asins analīzes — pilna asins aina, bilirubīns, aknu rādītāji (ALAT, ASAT, sārmainā fosfatāze, GGT), amilāze vai lipāze. Tās parāda nevis pašus akmeņus, bet to, vai ir iekaisums, aizsprostojums vai aizkuņģa dziedzera iesaiste.
MRCP (žultsceļu magnētiskā rezonanse) — kad ir aizdomas, ka akmens ir nokļuvis žultsvadā. To nenozīmē visiem, bet tikai tad, ja uz to norāda analīzes vai ultrasonogrāfijas atradne.
Endoskopiskā ultrasonogrāfija — retāk, sarežģītos gadījumos, kad MRCP nedod atbildi.
Kas nav vajadzīgs: datortomogrāfija akmeņu meklēšanai. Daļa žultsakmeņu tajā vispār nav redzama, un normāla CT atradne nenozīmē, ka akmeņu nav.
Kad tā vairs nav vienkārša lēkme
Šī ir raksta svarīgākā daļa. Žults kolika pati par sevi nav dzīvībai bīstama. Bīstamas ir situācijas, kurās aizsprostojums nepāriet vai pievienojas infekcija.
Neatliekamā palīdzība tagad, ne rīt, ja:
sāpes ilgst vairāk par 5–6 stundām un nemazinās;
pievienojas drudzis vai drebuļi;
āda vai acu baltumi kļūst dzelteni;
urīns kļūst tumšs, izkārnījumi gaiši;
atkārtota vemšana, nespēja dzert;
sāpes kļūst jostveida un izstaro uz muguru;
apjukums, reibonis, straujš pulss.
Viena kombinācija ir īpaši svarīga. Sāpes kopā ar drudzi un dzelti ir klasiskā žultsceļu infekcijas triāde — stāvoklis, kurā pēc Tokijas vadlīnijām nepieciešama steidzama ārstēšana un bieži žultsceļu drenāža[11].
Ko darīt lēkmes laikā mājās
Ja lēkme jau sākusies un nav nevienas no iepriekš minētajām pazīmēm:
Neēdiet, kamēr sāpes nepāriet. Ēdiens liek žultspūslim sarauties vēl vairāk.
Drīkst lietot ierastos pretsāpju līdzekļus. Nesteroīdie pretiekaisuma līdzekļi žults kolikas gadījumā iedarbojas labi.
Siltums uz labās paribes var mazināt spazmu. Ja sāpes pieaug — sildīšanu pārtrauciet.
Nelietojiet žultsdzenošos līdzekļus. Lēkmes laikā tie var pasliktināt šo situāciju.
Pierakstiet laiku: cikos sākās, cik ilgi turējās, ar ko bija saistīta. Konsultācijā tas ir vērtīgāk par jebkuru aprakstu „sāp jau sen”.
Pirmā lēkme nozīmē, ka būs arī nākamā
Tas ir galvenais, ko gribu pateikt šajā rakstā. Kad akmeņi sāk izraisīt lēkmes, tās atkārtojas. Nākamā lēkme var būt pēc nedēļas vai pēc gada, un neviens nevar pateikt, vai tā atkal beigsies pati vai pāraugs komplikācijā.
Tāpēc pēc pirmās tipiskās lēkmes ir vērts nevis gaidīt nākamo, bet mierīgi, plānveidā pārrunāt taktiku. Plānota operācija un tā pati operācija, kas steidzami jāveic ar iekaisušu žultspūsli, — tās ir divas pilnīgi atšķirīgas situācijas.
Biežākie jautājumi
Cik ilgi drīkst gaidīt, ja sāpes pārgāja pašas?Ja lēkme bija tipiska un pilnībā pārgāja, tā nav neatliekama situācija — bet plānveida konsultāciju atlikt uz gadiem nav vērts. Nākamās lēkmes laiku paredzēt nevar.
Vai ultrasonogrāfija var akmeņus nepamanīt?Žultspūslī esošos akmeņus tā rāda ļoti ticami, ja izmeklējums veikts tukšā dūšā. Sliktāk ultrasonogrāfija redz akmeņus žultsvados — tur izmanto MRCP.
Sāpes ir, bet akmeņu neatrada. Ko tas nozīmē?Iespējami vairāki cēloņi: kuņģis, atvilnis, zarnas, retāk žultspūšļa funkcionāli traucējumi. Tas nozīmē, ka jāmeklē tālāk, nevis uzreiz jāizņem žultspūslis.
Vai lēkmes laikā drīkst lietot pretsāpju līdzekli?Drīkst. Tas nenoslēpj bīstamu stāvokli: drudzis, dzelte un ilgstošas sāpes nepazūd arī pēc pretsāpju līdzekļa.

Симптомы желчных камней: как распознать приступ и когда ехать в больницу
Большинство людей с камнями не чувствуют ничего. Но если приступ начался, человек обычно запоминает его на всю жизнь — а потом не знает, было это что-то серьёзное или просто «не та еда». Эта статья поможет разобраться.
Как выглядит настоящая желчная колика
Название вводит в заблуждение: слово «колика» заставляет думать о волнообразной боли, которая накатывает и отпускает. На деле желчная колика — это постоянная боль, которая нарастает за несколько десятков минут и потом держится на одном уровне.
Где: в правом подреберье или под грудиной. Часто отдаёт в спину, между лопатками или в правое плечо.
Сколько: обычно от 30 минут до нескольких часов. Боль, которая длится пять секунд или равномерно весь день, — как правило, не желчная колика.
Когда: часто после жирной еды, нередко ночью или под утро.
С чем вместе: тошнота, иногда рвота, потливость, беспокойство. Человек не может найти позу, в которой легче.
Главное отличие от всего остального: колика заканчивается. Камень, перекрывший выход из пузыря, соскальзывает обратно, и боль проходит — часто так же внезапно, как началась. Если боль не уходит несколько часов, это уже не простая колика.
С чем её чаще всего путают
Из-за этой путаницы люди годами лечат «гастрит», хотя причина в пузыре, а некоторым удаляют пузырь, но боль остаётся.
Что это может быть
Чем отличается от желчной колики
Язва желудка или гастрит
Боль больше по центру, связана с пустым желудком, стихает после еды или антацидов
Рефлюкс (GERD)
Жжение за грудиной, хуже лёжа и при наклонах, связано с кислотой
Синдром раздражённого кишечника
Боль меняется после стула, есть вздутие, нет ночных приступов
Панкреатит
Боль сильнее, опоясывающая, отдаёт в спину, длится дольше, состояние ухудшается
Сердце
Давление в груди при нагрузке, одышка — здесь медлить нельзя
Отдельно о том, чего камни не вызывают: постоянного вздутия, горечи во рту по утрам, непереносимости отдельных продуктов и «тяжести» без приступов. Эти жалобы встречаются часто, но причина обычно в другом, и удаление пузыря их не снимает.
Обследования: что действительно нужно
УЗИ — основное исследование. Камни в пузыре оно показывает очень надёжно. Приходить надо натощак, иначе сокращённый пузырь может скрыть камни.
Анализы крови — общий анализ, билирубин, печёночные показатели (АЛТ, АСТ, щелочная фосфатаза, ГГТ), амилаза или липаза. Они показывают не камни, а то, есть ли воспаление, закупорка или поражение поджелудочной.
MRCP (магнитно-резонансная холангиопанкреатография) — если подозревают камень в общем желчном протоке. Её назначают не всем, а по показаниям или данным УЗИ.
Эндоскопическое УЗИ — реже, в сложных случаях, когда MRCP не даёт ответа.
Чего не нужно: компьютерной томографии для поиска камней. Часть желчных камней на ней вообще не видна, и нормальный результат КТ не означает, что камней нет.
Когда это уже не просто приступ
Это самая важная часть статьи. Сама по себе желчная колика не опасна для жизни. Опасны ситуации, когда закупорка не проходит или присоединяется инфекция.
Одно сочетание особенно важно. Боль вместе с температурой и желтухой — классическая триада инфекции желчных путей. При этом состоянии по Токийским рекомендациям нужно срочное лечение, а часто и дренирование желчных путей[11].
Что делать во время приступа дома
Если приступ уже начался и ни одного из признаков выше нет:
Не ешьте, пока боль не пройдёт. Еда заставляет пузырь сокращаться ещё сильнее.
Обезболивающее, которым вы обычно пользуетесь, допустимо. Нестероидные противовоспалительные при желчной колике работают хорошо.
Тёплая грелка на правое подреберье может уменьшить спазм. Если боль нарастает, уберите грелку.
Не принимайте желчегонные. Во время приступа они могут сделать хуже.
Запишите время: во сколько началось, сколько держалось, с чем было связано. На консультации это ценнее любого описания «болит давно».
Первый приступ означает, что будет и следующий
Это главное, что я хочу донести этой статьёй. Если приступы начались, сами они не прекратятся. Следующий может быть через неделю или через год, и никто не скажет заранее, закончится ли он снова сам или перейдёт в осложнение.
Поэтому после первого типичного приступа стоит не ждать следующего, а спокойно, в плановом порядке обсудить тактику. Плановая операция и срочная при воспалённом пузыре — две совершенно разные ситуации.
Частые вопросы
Сколько можно ждать, если боль прошла сама?Если приступ был типичным и полностью прошёл, это не неотложная ситуация — но откладывать плановую консультацию на годы не стоит. Время следующего приступа предсказать нельзя.
Может ли УЗИ не увидеть камни?Камни в самом пузыре оно показывает очень надёжно, если исследование сделано натощак. Хуже УЗИ видит камни в протоках — там используют MRCP.
Боль есть, а камней не нашли. Что это значит?Причин может быть несколько: желудок, рефлюкс, кишечник, реже функциональные расстройства пузыря. Это значит, что надо искать дальше, а не сразу убирать пузырь.
Можно ли принимать обезболивающее во время приступа?Можно. Оно не замаскирует опасное состояние: температура, желтуха и многочасовая боль сохраняются и после обезболивающего.

Gallstone symptoms: recognising an attack and when to go to hospital
Most people with gallstones never feel anything. But if an attack does start, people usually remember it for life — and then have no idea whether it was something serious or just the wrong meal. This article is about telling the difference.
What real biliary colic looks like
The name misleads: "colic" suggests wave-like pain that comes and goes. In reality biliary colic is steady pain that builds gradually and then holds at one level.
Where: in the right upper abdomen or below the breastbone. Often radiating to the back, between the shoulder blades, or to the right shoulder.
How long: usually 30 minutes to several hours. Pain lasting five seconds, or staying steady all day, is generally not biliary colic.
When: often after a fatty meal, frequently at night or in the early morning.
Accompanied by: nausea, sometimes vomiting, sweating, restlessness. People cannot find a position that eases it.
The key difference from everything else: colic ends. The stone blocking the gallbladder outlet slips back, and the pain passes — often as suddenly as it started. If pain does not go away for several hours, it is no longer simple colic.
What it is most often mistaken for
This is why people treat "gastritis" for years while the gallbladder is the cause — and, conversely, why some have the gallbladder removed and the pain stays.

Pain more central, linked to an empty stomach, eases after food or antacids
Reflux (GERD)
Burning behind the breastbone, worse lying down or bending
Irritable bowel syndrome
Pain changes after a bowel movement, bloating present, no night attacks

Pain stronger, band-like, radiating to the back, lasts longer, condition worsens
The heart
Chest pressure on exertion, breathlessness — this must not wait
Separately, what gallstones do not cause: constant bloating, a bitter taste in the morning, intolerance of particular foods, and "heaviness" without attacks. These complaints are common, but they usually come from elsewhere, and gallbladder removal does not relieve them.
Investigations: what is actually needed
Ultrasound — the basic test. It shows stones in the gallbladder very reliably. Fast beforehand, or a contracted gallbladder can hide them.
Blood tests — full blood count, bilirubin, liver tests (ALT, AST, alkaline phosphatase, GGT), amylase or lipase. They show not the stones themselves but whether there is inflammation, obstruction, or pancreatic involvement.
MRCP (magnetic resonance cholangiopancreatography) — when a stone is suspected to have passed into the bile duct. Ordered only when blood tests or ultrasound findings suggest it.
Endoscopic ultrasound — less often, in difficult cases when MRCP is inconclusive.
What is not needed: a CT scan to look for stones. Some gallstones are invisible on CT, and a normal scan does not mean there are none.
When it is no longer just an attack
This is the most important part of the article. Biliary colic itself is not life-threatening. What is dangerous is when the obstruction does not resolve or infection sets in.
Emergency care now, not tomorrow, if:
pain lasts more than 5–6 hours and does not ease;
fever or shivering develops;
the skin or the whites of the eyes turn yellow;
urine darkens, stools become pale;
there is repeated vomiting or you cannot keep fluids down;
the pain becomes band-like and radiates to the back;
there is confusion, dizziness, or a racing pulse.
One combination matters especially. Pain together with fever and jaundice is the classic triad of acute cholangitis — a condition for which the Tokyo Guidelines call for urgent treatment and often biliary drainage[11].
What to do at home during an attack
If an attack has begun and none of the signs above are present:
Do not eat until the pain passes. Food triggers gallbladder contraction.
You can take your usual painkiller. Non-steroidal anti-inflammatories work well in biliary colic.
A warm pad over the right upper abdomen can ease the spasm. If pain increases, take it off.
Do not take bile stimulants. During an attack they can make things worse.
Note the timing: when it started, how long it lasted, what preceded it. At the consultation that is worth more than any description of "it has hurt for a long time".
A first attack means there will be another
This is the main thing I want people to take from this article. Once stones start causing attacks, they do not stop on their own. The next one may come in a week or in a year, and no one can say in advance whether it will settle by itself or turn into a complication.
So after a first clear attack it is better to discuss the plan calmly than wait for the next one. Elective surgery and one done urgently on an inflamed gallbladder are very different.
Frequently asked questions
How long can I wait if the pain went away by itself?If the attack was clear-cut and fully resolved, it is not an emergency — but putting off an elective consultation for years is unwise. The timing of the next attack cannot be predicted.
Can an ultrasound miss stones?It shows stones inside the gallbladder very reliably when done fasting. It is less reliable for duct stones — MRCP is used there.
There is pain but no stones were found. What does that mean?There can be several causes: the stomach, reflux, the bowel, less often a functional gallbladder disorder. Investigate further; do not remove the gallbladder straight away.
Can I take a painkiller during an attack?Yes. It will not mask a dangerous situation: fever, jaundice and pain lasting hours remain visible even after a painkiller.
