Šis raksts ir domāts, lai cilvēks, kurš atliek lēmumu, saprastu, kas tieši var notikt un cik ātri. Visas šeit aprakstītās komplikācijas ir ārstējamas — bet gandrīz visas nozīmē, ka mierīgas plānveida operācijas vietā notiek kaut kas cits.
Kāpēc „gaidīsim” ir arī lēmums
Kad akmeņi ir klusi, gaidīšana ir pamatota izvēle. Kad tie jau sākuši izraisīt lēkmes, situācija mainās: katra nākamā lēkme var beigties pati, bet var arī nebeigties. Neviens iepriekš nezina, kura būs pēdējā.
Galvenā doma: plānveida „mierīga” žultspūšļa operācija un tā pati iekaisuša pūšļa operācija nakts vidū — tehniski tās ir divas dažādas operācijas ar dažādu risku. Komplikācija atņem iespēju izvēlēties.
Akūts holecistīts — visbiežākā komplikācija
Akmens iestrēgst izejā no žultspūšļa un neatslīd atpakaļ. Žults paliek pūslī, siena pietūkst, pievienojas infekcija.
Kā izpaužas: sāpes labajā paribē, kas nepāriet ilgāk par 5–6 stundām, drudzis, slikta dūša, sāpīgums pieskaroties.
Ko dara: hospitalizācija, antibiotikas un vairumā gadījumu — operācija tuvāko dienu laikā. Tokijas vadlīnijas vieglas un vidēji smagas pakāpes gadījumā piemērotiem pacientiem iesaka agrīnu laparoskopisku operāciju, nevis gaidīšanu[10].
Kad tā ir smaga: ja pievienojas orgānu funkciju traucējumi, pieeja mainās — vispirms stabilizācija vai žultspūšļa drenāža, operācija vēlāk.
Neārstēts akūts holecistīts var radīt strutu uzkrāšanos pūslī, sienas nekrozi un perforāciju. Tas jau ir vēdera dobuma iekaisums ar pavisam citu smagumu.
Akmens žultsvadā un dzelte
Akmens no pūšļa var izkļūt kopējā žultsvadā un tur iesprūst. Žults vairs neaizplūst zarnā.
Kā izpaužas: dzeltena āda un acu baltumi, tumšs urīns, gaiši izkārnījumi, nieze, sāpes.
Ko dara: akmeni no vada parasti izņem endoskopiski (ERCP), un žultspūsli izņem atsevišķi — visbiežāk tās pašas hospitalizācijas laikā vai drīz pēc tās.
Šeit ir svarīga viena lieta, ko pacienti bieži nesaprot: ERCP izņem akmeni no vada, bet neatrisina pašu problēmu. Akmeņu avots — žultspūslis — paliek vietā, un nākamais akmens var aiziet to pašu ceļu.
Holangīts — infekcija žultsceļos
Ja aizsprostotā vadā pievienojas infekcija, stāvoklis ātri kļūst bīstams. Klasiskā triāde ir sāpes, drudzis un dzelte; smagos gadījumos pievienojas asinsspiediena kritums un apjukums.
Tokijas vadlīnijas šeit prasa neatliekamu ārstēšanu: antibiotikas, atbalsta terapiju un žultsceļu drenāžu, kuras steidzamība ir atkarīga no smaguma pakāpes[11]. Tas ir viens no nedaudzajiem stāvokļiem šajā rakstā, kur stundas patiešām ir svarīgas.
Žultsakmeņu izraisīts pankreatīts
Akmens, ejot pa vadu, var uz laiku aizsprostot arī aizkuņģa dziedzera izvadu. Rezultāts ir aizkuņģa dziedzera iekaisums — no viegla līdz smagam.
Kā izpaužas: stipras jostveida sāpes ar izstarojumu mugurā, vemšana, stāvokļa pasliktināšanās.
Ko dara: ārstēšana slimnīcā. Pēc atveseļošanās žultspūsli izņem — citādi pankreatīts atkārtojas.
Tas ir gadījums, kurā operācijas atlikšana „uz vēlāk” ir īpaši slikta doma: atkārtots žultsakmeņu pankreatīts ir paredzams, nevis nejaušs.
Retākas, bet nopietnas situācijas
Mirici sindroms — akmens pūšļa kaklā saspiež blakus esošo kopējo aknu vadu. Rada dzelti bez akmens pašā vadā un padara operāciju tehniski sarežģītu.
Žultspūšļa un zarnas fistula — ilgstoša iekaisuma dēļ starp pūsli un zarnu izveidojas savienojums.
Akmeņu izraisīts zarnu nosprostojums — liels akmens caur fistulu nokļūst zarnā un tur iesprūst. Reta situācija, kas prasa neatliekamu operāciju.
Žultspūšļa vēzis — reta slimība. Saistība ar ilgstošu akmeņu klātbūtni ir aprakstīta, taču to nedrīkst pasniegt kā gaidāmu scenāriju: nejauši atklāts žultspūšļa vēzis sastopams aptuveni 0,36% holecistektomiju[7]. Tas nav arguments operēt visus, bet ir iemesls neignorēt sabiezinātu sienu vai polipu.
Ko no tā visa vajadzētu paturēt prātā
Ne to, ka viss beigsies slikti — vairumam nekas no iepriekš minētā nenotiks. Bet to, ka izvēle nav starp „operēt” un „nekas nenotiks”. Izvēle ir starp plānveida operāciju tad, kad jūs esat vesels un varat izvēlēties datumu, un iespējamu operāciju tad, kad izvēles vairs nav.
Neatliekami: sāpes labajā paribē ilgāk par 5–6 stundām, drudzis vai drebuļi, dzeltena āda vai acu baltumi, tumšs urīns, atkārtota vemšana, jostveida sāpes ar izstarojumu mugurā, apjukums vai straujš pulss. Ar jebkuru no šīm pazīmēm jābrauc uz slimnīcu, nevis jāgaida rīts.
Biežākie jautājumi
Cik ātri lēkme var pāraugt komplikācijā?Tas var notikt dažās stundās. Tāpēc robeža ir noteikta: ja sāpes nepāriet ilgāk par 5–6 stundām, vairs nedrīkst gaidīt.
Ja akmeni izņēma ar ERCP, vai žultspūslis vēl jāizņem?Vairumā gadījumu jā. ERCP atbrīvo vadu, bet akmeņu avots paliek, un nākamais akmens var aiziet to pašu ceļu.
Vai antibiotikas var aizstāt operāciju holecistīta gadījumā?Tās var apturēt iekaisumu, bet akmeņus neizvāc. Pēc konservatīvas ārstēšanas holecistīts bieži atkārtojas, tāpēc operāciju parasti tomēr plāno.
Vai no akmeņiem rodas vēzis?Žultspūšļa vēzis ir reta slimība. Saistība ar ilgstošu akmeņu klātbūtni ir aprakstīta, taču tas nav gaidāmais scenārijs un nav arguments operēt bez citām indikācijām.

Эта статья для того, чтобы человек, откладывающий решение, понимал, что именно может произойти и как быстро. Все описанные здесь осложнения лечатся — но почти все означают, что вместо спокойной плановой операции случается что-то другое.
Почему «подождём» — тоже решение
Пока камни молчат, ожидание — обоснованный выбор. Когда они уже дают приступы, ситуация меняется: каждый следующий приступ может закончиться сам, а может и не закончиться. Никто заранее не знает, каким будет следующий.
Главная мысль: плановая операция на «спокойном» пузыре и та же операция на воспалённом среди ночи — технически разные операции с разными рисками. При осложнении выбирать уже не приходится.
Острый холецистит — самое частое осложнение
Камень застревает на выходе из пузыря и не соскальзывает обратно. Желчь остаётся внутри, стенка отекает, присоединяется инфекция.
Как проявляется: боль в правом подреберье, не проходящая больше 5–6 часов, температура, тошнота, болезненность при прикосновении.
Что делают: госпитализация, антибиотики и в большинстве случаев операция в ближайшие дни. Токийские рекомендации при лёгкой и средней степени, если нет противопоказаний, советуют раннюю лапароскопическую операцию, а не выжидание[10].
При тяжёлой степени: если возникает органная дисфункция, подход меняется — сначала стабилизация или дренирование пузыря, операция позже.
Нелеченый острый холецистит может привести к скоплению гноя, некрозу стенки и перфорации. Это уже воспаление в брюшной полости совсем другой тяжести.
Камень в желчном протоке и желтуха
Камень из пузыря может выйти в общий желчный проток и застрять там. Желчь перестаёт оттекать в кишку.
Как проявляется: желтизна кожи и белков глаз, тёмная моча, светлый кал, кожный зуд, боль.
Что делают: камень из протока обычно удаляют эндоскопически (ЭРХПГ), а пузырь убирают отдельно — как правило, во время той же госпитализации или вскоре после неё.
Здесь важно одно, чего пациенты часто не понимают: ЭРХПГ убирает камень из протока, но не решает саму проблему. Источник камней — пузырь — остаётся на месте, и следующий камень может пойти тем же путём.
Холангит — инфекция желчных путей
Если в закупоренном протоке присоединяется инфекция, состояние быстро становится опасным. Классическая триада — боль, температура и желтуха; в тяжёлых случаях добавляются падение давления и спутанность сознания.
Токийские рекомендации здесь требуют немедленного лечения: антибиотики, поддерживающая терапия и дренирование желчных путей, срочность которого зависит от степени тяжести[11]. Это одно из немногих состояний в этой статье, где часы действительно имеют значение.
Билиарный панкреатит
Камень в протоке может временно перекрыть отток из поджелудочной железы. Результат — панкреатит, от лёгкого до тяжёлого.
Как проявляется: сильная опоясывающая боль, отдающая в спину, рвота, ухудшение состояния.
Что делают: лечение в стационаре. После выздоровления пузырь удаляют — без этого панкреатит повторяется.
Это тот случай, когда откладывать операцию «на потом» — особенно плохая идея: повторный билиарный панкреатит — предсказуемое событие, а не невезение.
Более редкие, но серьёзные ситуации
Синдром Мириззи — камень в шейке пузыря сдавливает соседний общий печёночный проток. Даёт желтуху без камня в самом протоке и делает операцию технически сложной.
Свищ между пузырём и кишкой — из-за длительного воспаления образуется сообщение между ними.
Кишечная непроходимость, вызванная камнем, — крупный камень через свищ попадает в кишку и застревает там. Редкая ситуация, требующая неотложной операции.
Рак желчного пузыря — редкая болезнь. Связь с длительным носительством камней описана, но подавать её как ожидаемый сценарий нельзя: случайно выявленный рак пузыря встречается примерно в 0,36% холецистэктомий[7]. Это не довод оперировать всех, но повод не игнорировать утолщённую стенку или полип.
Что из всего этого стоит вынести
Не то, что всё кончится плохо — у большинства ничего из перечисленного не случится. А то, что выбор не между «оперировать» и «ничего не будет». Выбор между плановой операцией тогда, когда вы здоровы и можете выбрать дату, и возможной операцией тогда, когда ситуацию выбираете не вы.
Неотложно: боль в правом подреберье дольше 5–6 часов, температура или озноб, желтизна кожи или белков глаз, тёмная моча, повторная рвота, опоясывающая боль, отдающая в спину, спутанность сознания или частый пульс. При любом из этих признаков нужно ехать в больницу, а не ждать утра.
Частые вопросы
Как быстро приступ может перейти в осложнение?Это может быть вопрос часов. Поэтому граница задана: боль, не проходящая больше 5–6 часов, — уже не повод ждать.
Если камень убрали при ЭРХПГ, пузырь всё равно надо удалять?В большинстве случаев да. ЭРХПГ освобождает проток, но источник камней остаётся, и следующий камень может пойти тем же путём.
Могут ли антибиотики заменить операцию при холецистите?Они могут погасить воспаление, но камни не убирают. После консервативного лечения холецистит часто повторяется, поэтому операцию обычно всё равно планируют.
Правда ли, что от камней бывает рак?Рак желчного пузыря — редкая болезнь. Связь с длительным носительством камней описана, но это не ожидаемый сценарий и не довод оперировать без других показаний.

This article is meant to show anyone postponing a decision exactly what can happen and how fast. Every complication described here is treatable — but almost all turn a planned operation into an emergency.
Why "let's wait" is also a decision
While stones are silent, waiting is a reasonable choice. Once they are causing attacks, the situation changes: the next attack may settle by itself, or it may not. No one knows in advance which one will be the last.
The core point: operating electively on a quiet gallbladder and urgently on an inflamed one are different jobs with different risks. A complication takes that choice away.
Acute cholecystitis — the most common complication
A stone lodges in the gallbladder outlet and does not slip back. Bile stays inside, the wall swells, infection sets in.
How it shows: right upper abdominal pain that does not pass within 5–6 hours, fever, nausea, tenderness to touch.
What is done: admission, antibiotics, and in most cases an operation within days. For mild and moderate grades in suitable patients, the Tokyo Guidelines advise early laparoscopic surgery rather than waiting[10].
When it is severe: if organ dysfunction develops, the approach changes — stabilisation or gallbladder drainage first, surgery later.
Untreated acute cholecystitis can progress to pus build-up in the gallbladder, necrosis of the wall, and perforation. That causes far more severe inflammation in the abdomen.
A stone in the bile duct and jaundice
A stone can escape the gallbladder into the common bile duct and become stuck. Bile can no longer drain into the bowel.
How it shows: yellow skin and eyes, dark urine, pale stools, itching, pain.
What is done: the duct stone is usually removed endoscopically (ERCP), and the gallbladder is removed separately — normally during the same admission or soon after.
One thing here is often misunderstood: ERCP clears the stone from the duct but does not solve the problem. The source of the stones — the gallbladder — stays in place, and the next stone can travel the same route.
Cholangitis — infection of the bile ducts
If an obstructed duct becomes infected, the situation turns dangerous quickly. The classic triad is pain, fever and jaundice; in severe cases low blood pressure and confusion follow.
The Tokyo Guidelines require immediate treatment here: antibiotics, supportive care, and biliary drainage whose urgency depends on severity[11]. This is one of the few conditions in this article where hours genuinely matter.
Gallstone pancreatitis
A stone passing down the duct can also temporarily block the pancreatic duct. The result is pancreatitis, ranging from mild to severe.
How it shows: severe band-like pain radiating to the back, vomiting, deteriorating condition.
What is done: treatment in hospital. After recovery the gallbladder is removed — otherwise, pancreatitis may recur.
This is the case where postponing surgery is a particularly poor idea: recurrent gallstone pancreatitis is a predictable event, not bad luck.
Rarer but serious situations
Mirizzi syndrome — a stone in the gallbladder neck compresses the adjacent common hepatic duct. It causes jaundice without a stone in the duct itself and makes surgery technically difficult.
A fistula between gallbladder and bowel — prolonged inflammation creates a connection between them.
Gallstone ileus — a large stone passes through such a fistula into the bowel and lodges there. A rare situation requiring emergency surgery.
Gallbladder cancer — a rare disease. An association with long-standing stones is described, but it must not be presented as an expected outcome: incidental gallbladder cancer is found in about 0.36% of cholecystectomies[7]. That is not an argument to operate on everyone, but it is a reason not to ignore a thickened wall or a polyp.
What to take from all this
Not that things will end badly — for most people none of the above will happen. But that the choice is not between "operate" and "nothing will happen". The choice is between an elective operation while you are well and can pick the date, and a possible operation at a moment you do not choose.
Urgent: right upper abdominal pain lasting more than 5–6 hours, fever or chills, yellow skin or eyes, dark urine, repeated vomiting, band-like pain radiating to the back, confusion or a racing pulse. Any of these means you should go to hospital rather than wait until morning.

How quickly can an attack turn into a complication?It can be a matter of hours. That is why the rule is: pain that does not pass within 5–6 hours is a reason not to wait.
If the stone was removed by ERCP, does the gallbladder still need to come out?In most cases yes. ERCP clears the duct, but the source of the stones remains, and the next one can take the same route.
Can antibiotics replace surgery in cholecystitis?They can settle the inflammation, but they do not remove the stones. Cholecystitis often recurs after conservative treatment, so surgery is usually still planned.
Is it true that stones cause cancer?Gallbladder cancer is a rare disease. An association with long-standing stones is described, but it is not the expected outcome and not a reason to operate without other indications.
