Cilvēki baidās nevis no operācijas, bet no nezināšanas. Tāpēc šeit ir viss pēc kārtas: kas notiek dienu iepriekš, operācijas dienā un pēc tam, un kas šajā laikā ir normāli.
Ko tieši izņem
Izņem visu žultspūsli kopā ar akmeņiem, nevis tikai akmeņus. To pacienti bieži gribētu mainīt, bet nevar: pūslis, kas vienreiz saražojis akmeņus, ražos tos atkal. Akmeņu izņemšana ar pūšļa saglabāšanu neatrisina problēmu.
Žultspūslis ir žults krātuve, nevis tās ražotne. Žulti ražo aknas, un pēc operācijas tā turpina plūst zarnā — vienkārši vienmērīgi, nevis porcijās.
Pirms operācijas
Konsultācijā izvērtējam ultrasonogrāfiju, analīzes un blakusslimības, pārrunājam anestēziju un vienojamies par datumu.
Asins šķidrinātājus un dažas citas zāles var nākties uz laiku pārtraukt — par to lemj individuāli, pašam neko atcelt nevajag.
Pēdējā maltīte un šķidruma uzņemšana — pēc anesteziologa norādēm, parasti vakarā pirms operācijas.
Ja ir aizdomas par akmeni žultsvadā, pirms operācijas veic MRCP, un dažreiz vispirms nepieciešama ERCP.
Operācijas diena
Operācija notiek vispārējā narkozē. Vēdera dobumu piepilda ar oglekļa dioksīdu, lai izveidotos telpa darbam, un caur četriem nelieliem iegriezumiem (parasti 5–10 mm) ievieto kameru un instrumentus.
Tālāk — daļa, kurai velta visvairāk laika, lai gan no malas tā izskatās kā gaidīšana. Pirms kaut ko pārgriezt, ķirurgam ir jāsasniedz Critical View of Safety: jāatbrīvo Kalo trijstūris no taukaudiem un saaugumiem un jāpārliecinās, ka žultspūslim pievienojas tieši divas struktūras — cistiskais vads un cistiskā artērija. SAGES ekspertu konsensā tieši šis paņēmiens tika atzīts par svarīgāko drošas operācijas elementu[12].
Ja anatomija nav skaidra, pareizā rīcība ir apstāties un mainīt plānu, nevis turpināt. Par to, kā tas notiek, ir atsevišķs raksts.
Kad struktūras ir droši identificētas, tās noslēdz un pārgriež, pūsli atdala no aknu gultnes un izņem caur vienu no iegriezumiem. Operācija parasti ilgst 30–60 minūtes; iekaisuša pūšļa gadījumā ilgāk.
Izņemto žultspūsli vienmēr nosūta uz histoloģisku izmeklēšanu. Tā nav formalitāte: daļa žultspūšļa audzēju tiek atklāta tieši šādi — nejauši, pēc operācijas, kas veikta labdabīgas slimības dēļ[7].
Pirmās stundas un pirmā nakts
Vairumā gadījumu mājās var doties tajā pašā dienā vai nākamajā rītā. Ilgāk paliek tad, ja bija iekaisums, blakusslimības vai operācija bija sarežģīta.
Dzert var drīz pēc pamošanās, ēst — tajā pašā dienā, sākot ar vieglu maltīti.
Piecelties un pastaigāties pa palātu vajag jau operācijas dienā. Tā nav varonība, tas samazina trombu risku.
Par plecu sāpēm. Tās pārsteidz gandrīz visus: sāp nevis vēders, bet labais plecs. To izraisa gāzes atlieku kairināta diafragma, un tas nav bojājums. Pāriet dažu dienu laikā, palīdz kustība un pretsāpju līdzeklis.

Ēšana pēc operācijas
Stingra pastāvīga diēta nav vajadzīga — tas ir viens no izplatītākajiem maldiem. Pirmajās nedēļās der mazākas un biežākas maltītes ar mazāk tauku, kamēr organisms pierod pie vienmērīgas žults plūsmas. Pēc tam vairums cilvēku ēd pilnīgi normāli.

Kas ir normāli un kas nav
Normāli pirmajās dienās: mērenas sāpes ap iegriezumiem, plecu sāpes, nogurums, neliela vēdera uzpūšanās, apetītes svārstības.
Sazinieties ar ārstu nekavējoties, ja pēc operācijas parādās: drudzis vai drebuļi, pieaugošas vēdera sāpes, dzeltena āda vai acu baltumi, atkārtota vemšana, izdalījumi no brūces, pieaugoša vēdera uzpūšanās, elpas trūkums vai sāpes kājā. Šīs pazīmes prasa novērtēšanu, nevis gaidīšanu līdz plānotajai vizītei.
Ko es pārrunāju ar pacientu pirms operācijas
Trīs lietas, vienmēr un iepriekš: ka pastāv iespēja pāriet uz atvērtu operāciju un ka tas ir drošības lēmums, nevis neveiksme; ka operācijai ir savi riski, par kuriem ir atsevišķa saruna; un ka operācija novērš akmeņu lēkmes, bet negarantē, ka pazudīs visas gremošanas sūdzības, ja to cēlonis nebija žultspūslis.

Cik ilgi ilgst operācija?Parasti 30–60 minūtes. Iekaisuma, saaugumu vai neskaidras anatomijas gadījumā ilgāk — un tas ir normāli, jo laiku prasa droša struktūru atbrīvošana.
Kāpēc pēc operācijas sāp plecs?Vēdera dobumā palikusī gāze kairina diafragmu, un sāpes izstaro uz plecu. Tas nav bojājums un pāriet dažu dienu laikā.
Kad var braukt ar auto?Kad varat strauji bremzēt bez sāpēm un vairs nelietojat stipros pretsāpju līdzekļus. Vairumam — aptuveni pēc nedēļas.
Vai pēc operācijas paliks rētas?Četras nelielas rētas 5–10 mm. Ar laiku tās kļūst gandrīz nemanāmas.

Лапароскопическая холецистэктомия: как проходит операция и восстановление
Люди боятся не операции, а неизвестности. Поэтому здесь всё по порядку: что происходит накануне, в день операции и после, и что в это время нормально.
Что именно удаляют
Удаляют весь желчный пузырь вместе с камнями, а не только камни. Это главное, что люди хотели бы изменить и чего изменить нельзя: пузырь, однажды образовавший камни, образует их снова. Удаление камней с сохранением пузыря проблему не решает.
Желчный пузырь хранит желчь, а не производит её. Желчь вырабатывает печень, и после операции она продолжает поступать в кишку — просто равномерно, а не порциями.
Перед операцией
На консультации оцениваем УЗИ, анализы и сопутствующие болезни, обсуждаем наркоз и договариваемся о дате.
Иногда разжижающие кровь и другие препараты нужно временно отменить — это решается индивидуально, самому отменять ничего не нужно.
Последний приём пищи и жидкости — по указаниям анестезиолога, обычно вечером накануне.
Если есть подозрение на камень в желчном протоке, перед операцией делают MRCP, а иногда сначала требуется ERCP.
День операции
Операция идёт под общим наркозом. В брюшную полость нагнетают углекислый газ, создавая рабочее пространство, и через четыре небольших разреза (обычно 5–10 мм) вводят камеру и инструменты.
Дальше — самая долгая часть, хотя со стороны кажется, что ничего не происходит. Прежде чем что-либо пересечь, хирург обязан получить Critical View of Safety: очистить треугольник Кало, отделить нижнюю треть пузыря от печени и увидеть только пузырный проток и пузырную артерию. В экспертном консенсусе SAGES именно этот приём признан важнейшим элементом безопасной операции[12].
Если анатомия неясна, правильное действие — остановиться и сменить план, а не продолжать. О том, как это происходит, есть отдельная статья.
Когда структуры чётко выделены, их пережимают и пересекают, пузырь отделяют от ложа печени и извлекают через один из разрезов. Операция обычно занимает 30–60 минут, при воспалённом пузыре — дольше.
Удалённый пузырь всегда отправляют на гистологическое исследование. Это не формальность: часть опухолей желчного пузыря выявляется именно так — случайно, после операции, сделанной по поводу доброкачественной болезни[7].
Первые часы и первая ночь
В большинстве случаев домой можно в тот же день или на следующее утро. Дольше остаются, если было воспаление, есть сопутствующие болезни или операция оказалась сложной.
Пить можно вскоре после пробуждения, есть — в тот же день, начиная с лёгкой еды.
Встать и пройтись по палате нужно уже в день операции. Это не героизм, это снижает риск тромбов.
О боли в плече. Она застаёт врасплох почти всех: болит не живот, а правое плечо. Так остатки газа раздражают диафрагму; это не повреждение. Проходит за несколько дней, помогают движение и обезболивающее.

Сроки ориентировочные. После операции при воспалении или после перехода в открытую операцию всё идёт медленнее, и это не осложнение, а другая исходная ситуация.
Еда после операции
Строгая пожизненная диета не нужна — это одно из самых распространённых заблуждений. В первые недели лучше есть меньше, но чаще и ограничить жиры, пока пищеварение привыкает к равномерному поступлению желчи. Дальше большинство людей едят как обычно.

Что нормально, а что нет
Нормально в первые дни: умеренная боль вокруг разрезов, боль в плече, усталость, небольшое вздутие, колебания аппетита.
Свяжитесь с врачом немедленно, если после операции появились температура или озноб, нарастающая боль в животе, желтизна кожи или белков глаз, повторная рвота, выделения из раны, нарастающее вздутие живота, одышка или боль в ноге. Эти признаки требуют оценки, а не ожидания планового визита.
Что я обсуждаю с пациентом до операции
Три вещи я обсуждаю всегда: что возможен переход в открытую операцию и что это решение ради безопасности, а не неудача; что у операции есть свои риски, о которых отдельный разговор; и что операция убирает камни и приступы, но не гарантирует, что исчезнут все жалобы на пищеварение, если их причина была не в пузыре.
Частые вопросы
Сколько длится операция?Обычно 30–60 минут. При воспалении, сращениях или неясной анатомии дольше — и это нормально, потому что время уходит на безопасное выделение структур.
Почему после операции болит плечо?Оставшийся в животе газ раздражает диафрагму, и боль отдаёт в плечо. Это не повреждение, проходит за несколько дней.
Когда можно за руль?Когда сможете резко затормозить без боли и уже не принимаете сильные обезболивающие. У большинства это около недели.
Останутся ли шрамы?Четыре небольших рубца по 5–10 мм. Со временем они становятся почти незаметными.

Laparoscopic cholecystectomy: how the operation and recovery go
People fear not the operation but the unknown. So here is everything in order: what happens the day before, on the day, and afterwards — and what is normal along the way.
What exactly is removed
The whole gallbladder is removed together with the stones, not just the stones. This is what people most often want to avoid, but cannot: a gallbladder that has made stones once will make them again. Taking out the stones and keeping the organ does not solve the problem.
The gallbladder stores bile; it does not make it. The liver makes bile, and after the operation it keeps flowing into the bowel — simply steadily rather than in bursts.
Before the operation
At the consultation we review the ultrasound, blood tests and other conditions, discuss anaesthesia and agree a date.
Blood thinners and some other medicines may need to be paused — this is decided individually, and you should not stop anything on your own.
Stop eating and drinking when the anaesthetist says, usually the evening before.
If a stone in the bile duct is suspected, an MRCP is done beforehand, and sometimes an ERCP is needed first.
The day of surgery
The operation is done under general anaesthesia. The abdomen is filled with carbon dioxide to create working space, and a camera and instruments are placed through four small incisions, usually 5–10 mm.
Then comes the part that takes the most time, although from outside it looks like waiting. Before cutting anything, the surgeon must achieve the Critical View of Safety: clear Calot's triangle of fat and adhesions and confirm that exactly two structures enter the gallbladder — the cystic duct and the cystic artery. In the SAGES expert consensus this manoeuvre was rated the single most important element of a safe operation[12].
If the anatomy is not clear, the right action is to stop and change the plan rather than push on. There is a separate article on how that works.
Once the structures are safely identified, they are sealed and divided; the gallbladder is then separated from the liver bed and removed through one of the incisions. The operation usually takes 30–60 minutes, or longer with inflammation.
The removed gallbladder always goes for histological examination. That is not a formality: some gallbladder tumours are found this way — incidentally, after an operation done for benign disease[7].
The first hours and the first night
Most people go home the same day or the next morning. Inflammation, other illnesses or a difficult operation may mean a longer stay.
You can drink soon after waking and eat the same day, starting with something light.
Get up and walk around the ward on the day of surgery. That is not heroism; it lowers the risk of clots.
About shoulder pain. It catches almost everyone by surprise: it is not the abdomen that hurts but the right shoulder. Residual gas irritates the diaphragm; it is harmless. It settles within a few days; movement and a painkiller help.

These timings are approximate. After an operation with inflammation, or after conversion to open surgery, everything moves more slowly — that is not a complication but a different starting point.
Eating afterwards
A strict lifelong diet is not needed — that is one of the most common misconceptions. In the first weeks smaller, more frequent meals with less fat suit most people while digestion adjusts to a steady flow of bile. After that most people eat entirely normally.

What is normal and what is not
Normal in the first days: moderate pain around the incisions, shoulder pain, tiredness, mild bloating, a variable appetite.
Contact a doctor immediately if after the operation you develop fever or chills, increasing abdominal pain, yellow skin or eyes, repeated vomiting, discharge from a wound, a swollen abdomen, breathlessness, or pain in the leg. These need assessment rather than waiting for the planned appointment.
What I discuss with patients beforehand
Three things, always and in advance: that conversion to open surgery is possible and is a safety decision rather than a failure; that the operation has its own risks, which are a separate conversation; and that surgery removes stones and stops attacks but cannot guarantee relief from all digestive complaints, if the gallbladder was not what caused them.
Frequently asked questions
How long does the operation take?Usually 30–60 minutes. Longer with inflammation, adhesions or unclear anatomy — and that is normal, because time is spent safely identifying the structures.
Why does my shoulder hurt afterwards?Gas left in the abdomen irritates the diaphragm and the pain is felt in the shoulder. It is not damage and settles within a few days.
