Laparoskopiskā holecistektomija ir viena no biežāk veiktajām operācijām pasaulē, un lielākajai daļai cilvēku tā norit bez sarežģījumiem. Bet ķirurģijā nulles riska nav, un pacientam ir tiesības zināt, kas tieši var notikt un ko tādā gadījumā dara.
Vispirms — kas nav komplikācija
Pirmajās dienās normāli ir: sāpes ap iegriezumiem, sāpes labajā plecā no atlikušās gāzes, nogurums, neliela vēdera uzpūšanās, apetītes svārstības, mainīga vēdera izeja. Tas nav sarežģījums, bet atveseļošanās.
Turpmāk par to, kas patiešām ir komplikācijas.
Žults noplūde
Žults var sūkties no cistiskā vada celma vai no sīkiem vadiņiem aknu gultnē. Izpaužas ar sāpēm, sliktu dūšu, drudzi un vēdera uzpūšanos dažas dienas pēc operācijas.
Ko dara: apstiprina ar attēldiagnostiku, uzliek drenāžu un bieži veic ERCP, lai samazinātu spiedienu žultsceļos. Lielāko daļu noplūžu novērš bez atkārtotas operācijas.
Biežāk tā notiek pēc subtotālas operācijas — un tur pastāv atšķirība starp tehnikām: metaanalīzē fenestrējošajā tehnikā žults noplūde ir biežāka nekā rekonstruējošajā[13].
Žultsvada bojājums
Tas ir smagākais šīs operācijas sarežģījums, un tieši par to man vaicā visbiežāk.
Bojājums parasti rodas nevis tāpēc, ka nodreb roka, bet tāpēc, ka nepareizi atpazīst struktūras: izmainītā anatomijā žultsvadu notur par cistisko vadu. Risks ir augstāks akūta iekaisuma, saaugumu un anatomijas variāciju gadījumā.
Kā to pamana: daļu bojājumu ķirurgs redz operācijas laikā, daļa izpaužas vēlāk — ar dzelti, žults noplūdi vai sāpēm.
Ko dara: atkarībā no bojājuma veida — endoskopiska ārstēšana ar stentu vai rekonstruktīva operācija. Sarežģītos gadījumos ārstēšana notiek specializētā centrā.
Kas samazina risku: Critical View of Safety, gatavība pāriet uz atvērtu operāciju vai subtotālu holecistektomiju, holangiogrāfija neskaidrā anatomijā. SAGES konsensā tieši Critical View of Safety tika atzīts par svarīgāko elementu[12].
Pacientam svarīgi: šis risks nav vienāds visiem. Tas ir zemāks plānveida operācijā bez iekaisuma un augstāks steidzamā operācijā ar iekaisumu. Tāpēc gaidīšana līdz komplikācijai maina ne tikai operācijas apstākļus, bet arī pašu risku.
Asiņošana
Var rasties no cistiskās artērijas, aknu gultnes vai troakāra ievadīšanas vietas. Asiņošanu parasti aptur operācijas laikā. Reti nepieciešama atkārtota operācija vai asins pārliešana. Risks ir augstāks cilvēkiem, kuri lieto asins šķidrinātājus — tāpēc par visām zālēm jāziņo iepriekš.
Infekcija
Brūces infekcija ir reta un parasti viegli ārstējama. Nopietnāka ir vēdera dobuma infekcija — abscess žultspūšļa gultnē, kas parasti prasa drenāžu un antibiotikas. Biežāk tas notiek pēc operācijām ar strutainu iekaisumu.
Blakus esošu orgānu bojājums
Reti, bet iespējams zarnu, divpadsmitpirkstu zarnas, aknu vai asinsvadu bojājums. Daļa izpaužas tikai vairākas dienas pēc operācijas — ar pieaugošām sāpēm, drudzi un stāvokļa pasliktināšanos. Tieši tāpēc pēcoperācijas pazīmju saraksts ir svarīgāks par jebkuru statistiku.
Žultsvadā palikuši akmeņi
Akmens jau pirms operācijas var būt žultsvadā vai nonākt tur operācijas laikā. Izpaužas ar dzelti, sāpēm vai izmainītām aknu analīzēm. Ārstē ar ERCP. Neskaidrā situācijā palīdz holangiogrāfija operācijas laikā: centrā, kur to veica ikdienas praksē, akmeņus žultsvadā atklāja 6,5% pacientu[16].
Trombozes un anestēzija
Kāju vēnu tromboze un plaušu artērijas trombembolija ir retas, bet nopietnas. Tāpēc operācijas dienā liek piecelties un staigāt, un daļai pacientu nozīmē profilaksi. Anestēzijas riski ir atsevišķa saruna ar anesteziologu; tie lielākoties saistīti ar blakusslimībām, nevis pašu operāciju.
Vēlīnas problēmas
Trūce iegriezuma vietā — biežāk lielākajā nabas apvidus iegriezumā.
Saglabājušās sūdzības pēc operācijas. Šeit ir vērts zināt skaitļus: pētījumā ar 207 plānveida laparoskopiskām operācijām sūdzējās 58% pacientu pirmajā nedēļā, bet pēc sešiem mēnešiem — 13%, un tikai daļai no viņiem izdevās atrast konkrētu cēloni[15].
Caureja pēc operācijas — par to ir atsevišķs raksts; pētījumā ar 3385 pacientiem tā gada laikā bija 14,2%[14].
Akmeņi pūšļa atlikumā pēc subtotālas operācijas — iemesls novērošanai.
Par letalitāti
Vienu skaitli es apzināti nenosaucu. Iemesls: viens vidējais rādītājs 40 gadus vecam cilvēkam plānveida operācijā un 80 gadus vecam ar sepsi neatliekamā operācijā ir bezjēdzīgs un maldinošs.
Reāls risks ir atkarīgs no operācijas steidzamības, iekaisuma smaguma, vecuma, sirds un plaušu stāvokļa un blakusslimībām. Tieši šos rādītājus izvērtē pirms operācijas, un tieši par jūsu risku ir vērts runāt konsultācijā, nevis par vidējiem.
Pēc izrakstīšanās sazinieties ar ārstu nekavējoties, ja parādās: drudzis vai drebuļi, pieaugošas sāpes vēderā, dzeltena āda vai acu baltumi, atkārtota vemšana, pieaugoša vēdera uzpūšanās, izdalījumi vai apsārtums ap brūci, elpas trūkums, sāpes un pietūkums kājā, ģībonis.
Kā tas izskatās sarunā
Es neapsolu, ka viss noteikti būs labi. Uzdevums ir cits: lai cilvēks zinātu, kā komplikācijas izpaužas, kam zvanīt un ka gandrīz visas var ārstēt, ja tās pamana laikus.
Biežākie jautājumi
Cik liels ir žultsvada bojājuma risks?Tas nav vienāds visiem: zemāks plānveida operācijā bez iekaisuma, augstāks steidzamā operācijā ar iekaisumu. Tāpēc jautājums par konkrētu skaitli ir jāsaista ar konkrēto situāciju, ne ar vidējo rādītāju.
Ja pēc operācijas sāp vēders un ir drudzis, cik ilgi drīkst gaidīt?Negaidiet. Drudzis kopā ar pieaugošām sāpēm pēc operācijas ir iemesls sazināties ar ārstu tajā pašā dienā.
Vai sūdzības var palikt arī pēc veiksmīgas operācijas?Var. Pētījumā pēc sešiem mēnešiem sūdzības saglabājās 13% pacientu, un konkrētu cēloni atrada tikai daļai[15]. Tāpēc pirms operācijas ir svarīgi saprast, vai sūdzības tiešām nāk no žultspūšļa.
Vai var izvēlēties nelietot narkozi?Nē. Šī operācija tiek veikta vispārējā narkozē. Anestēzijas riskus atsevišķi izvērtē anesteziologs.

Риски и осложнения операции по удалению желчного пузыря
Лапароскопическая холецистэктомия — одна из самых частых операций в мире, и у большинства она проходит без осложнений. Но нулевого риска в хирургии не бывает, и пациент имеет право знать, что именно может случиться и что в этом случае делают.
Сначала — что осложнением не является
В первые дни нормально: боль вокруг разрезов, боль в правом плече от оставшегося газа, усталость, небольшое вздутие, колебания аппетита, неустойчивый стул. Это не осложнение, это восстановление.
Дальше — о настоящих осложнениях.
Подтекание желчи
Желчь может подтекать из культи пузырного протока или из мелких протоков в ложе желчного пузыря. Проявляется болью, тошнотой, температурой и вздутием живота через несколько дней после операции.
Что делают: подтверждают с помощью УЗИ или КТ, ставят дренаж и часто выполняют ЭРХПГ, чтобы снизить давление в желчных путях. Обычно подтекание прекращается без повторной операции.
Чаще это бывает после субтотальной операции — и там есть разница между техниками: в метаанализе фенестрирующая давала подтекание желчи чаще, чем реконституирующая[13].
Повреждение желчного протока
Это самое тяжёлое осложнение этой операции, и именно о нём меня спрашивают чаще всего.
Повреждение обычно происходит не потому, что «дрогнула рука», а из-за неверного опознания структуры: в изменённой анатомии желчный проток принимают за пузырный. Риск выше при остром воспалении, сращениях и вариантах анатомии.
Как замечают: часть повреждений хирург видит во время операции, часть проявляется позже — желтухой, подтеканием желчи или болью.
Что делают: в зависимости от типа повреждения — эндоскопическое лечение со стентом или реконструктивная операция. В сложных случаях лечение идёт в специализированном центре.
Что снижает риск: Critical View of Safety, готовность перейти к открытой операции или субтотальной холецистэктомии, холангиография при неясной анатомии. В консенсусе SAGES именно Critical View of Safety признан важнейшим элементом[12].
Практический смысл для пациента: этот риск не одинаков для всех. Он ниже при плановой операции без воспаления и выше при срочной на фоне воспаления. Если дождаться осложнения, меняются и условия операции, и риск.
Кровотечение
Может возникнуть из пузырной артерии, ложа желчного пузыря или места введения троакара. Обычно его останавливают во время операции. Редко требуется повторная операция или переливание крови. Риск выше у тех, кто принимает разжижающие кровь препараты, — поэтому обо всех лекарствах надо сообщить заранее.
Инфекция
Инфекция раны встречается редко и обычно легко лечится. Серьёзнее внутрибрюшная инфекция — абсцесс в ложе пузыря, который обычно требует дренирования и антибиотиков. Чаще это бывает после операций с гнойным воспалением.
Повреждение соседних органов
Редко, но возможна травма кишечника, двенадцатиперстной кишки, печени или сосудов. Некоторые проявляются через несколько дней после операции — нарастающей болью, температурой и ухудшением состояния. Именно поэтому список послеоперационных признаков важнее любой статистики.
Оставшиеся в протоке камни
Камень мог находиться в желчном протоке ещё до операции или уйти туда во время неё. Проявляется желтухой, болью или изменениями печёночных показателей. Лечится ЭРХПГ. В неясной ситуации помогает холангиография во время операции: в центре, где её делали рутинно, камни в протоке нашли у 6,5% пациентов[16].
Тромбозы и наркоз
Тромбоз вен ног и тромбоэмболия лёгочной артерии редки, но серьёзны. Поэтому в день операции надо ходить, а части пациентов назначают профилактику тромбозов. Риски наркоза — отдельный разговор с анестезиологом; они в основном связаны с сопутствующими болезнями, а не с самой операцией.
Поздние проблемы
Грыжа в месте разреза — чаще в более крупном, околопупочном.
Сохраняющиеся жалобы после операции. Здесь стоит знать цифры: в исследовании 207 плановых лапароскопических операций жалобы были у 58% в первую неделю, а через полгода — у 13%, причём конкретную причину удалось найти лишь у части из них[15].
Диарея после операции — о ней отдельная статья; в исследовании с 3385 пациентами за год она была у 14,2%[14].
Камни в остатке пузыря после субтотальной операции — повод для наблюдения.
О летальности
Одну цифру я здесь не привожу — и вот почему. Одна цифра для сорокалетнего на плановой операции и восьмидесятилетнего с сепсисом на срочной бессмысленна и вводит в заблуждение.
Реальный риск зависит от срочности операции, тяжести воспаления, возраста, состояния сердца и лёгких и сопутствующих болезней. Именно эти показатели оценивают перед операцией, и о вашем риске стоит говорить на консультации, а не о средних.
После выписки свяжитесь с врачом немедленно, если появились: температура или озноб, нарастающая боль в животе, желтизна кожи или белков глаз, повторная рвота, нарастающее вздутие живота, выделения или покраснение вокруг раны, одышка, боль и отёк ноги, обморок.
Как это выглядит в разговоре
Я не обещаю, что всё точно будет хорошо. Задача другая: чтобы человек знал, как проявятся осложнения, кому звонить и что почти все решаемы, если заметить вовремя.
Частые вопросы
Насколько велик риск повреждения желчного протока?Он не одинаков для всех: ниже при плановой операции без воспаления, выше при срочной на фоне воспаления. Поэтому вопрос о точной цифре надо привязывать к конкретной ситуации, а не к среднему показателю.
Если после операции болит живот и есть температура, сколько можно ждать?Не ждите. Температура вместе с нарастающей болью после операции — повод связаться с врачом в тот же день.
Могут ли жалобы остаться и после удачной операции?Могут. В исследовании через полгода жалобы сохранялись у 13% пациентов, и конкретную причину нашли лишь у части[15]. Поэтому до операции важно понять, действительно ли жалобы идут от пузыря.
Можно ли обойтись без наркоза?Нет. Эта операция выполняется под общим наркозом. Риски анестезии отдельно оценивает анестезиолог.

Laparoscopic cholecystectomy is one of the most frequently performed operations in the world, and for most people it goes smoothly. But surgical risk is never zero, and a patient has the right to know what can happen and what is done about it.
First — what is not a complication
Normal in the first days: pain around the incisions, right shoulder pain from residual gas, tiredness, mild bloating, appetite changes, unsettled bowels. That is not a complication; it is recovery.
Now to the real complications.
Bile leak
Bile can leak from the cystic duct stump or from small ducts in the liver bed. Signs include pain, nausea, fever and abdominal distension a few days after the operation.
What is done: confirm it with imaging, place a drain, and in most cases perform an ERCP to lower the pressure in the bile ducts. Most leaks settle without a repeat operation.
It is more common after subtotal surgery — and the technique matters there: in a meta-analysis the fenestrating approach caused bile leaks more often than the reconstituting one[13].
Bile duct injury
This is the most serious complication of the operation, and the one I am asked about most.
Injury usually happens not because "a hand slipped" but because a structure was misidentified: in distorted anatomy the bile duct is taken for the cystic duct. The risk is higher with acute inflammation, adhesions and anatomical variants.
How it is spotted: the surgeon sees some injuries during surgery; others emerge later — as jaundice, a bile leak, or pain.
What is done: depending on the type of injury — endoscopic treatment with a stent, or reconstructive surgery. Complex cases are treated at a specialist centre.
What lowers the risk: the Critical View of Safety, readiness to convert or perform subtotal cholecystectomy, and cholangiography when anatomy is unclear. The SAGES consensus rated the Critical View of Safety the most important element[12].
The practical point for a patient: this risk is not the same for everyone. It is lower in elective surgery on a quiet gallbladder and higher in emergency surgery on an inflamed one. So waiting until a complication develops changes not only the circumstances of the operation but the risk itself.
Bleeding
It can come from the cystic artery, the liver bed, or a port site. Most is stopped during the operation. A repeat operation or transfusion is rarely needed. The risk is higher in people taking blood thinners — which is why all medicines must be reported beforehand.
Infection
Wound infection is uncommon and usually easy to treat. More serious is an infection inside the abdomen — an abscess in the gallbladder bed, which normally needs drainage and antibiotics. It is more frequent after operations for purulent inflammation.
Injury to neighbouring organs
Rare but possible: injury to the bowel, the duodenum, the liver or blood vessels. Some become apparent days later, with worsening pain, fever and deterioration. That is exactly why the list of post-operative warning signs matters more than any statistic.
Retained duct stones
A stone may already be in the bile duct or enter it during surgery. Signs include jaundice, pain or abnormal liver tests. It is treated by ERCP. When anatomy is unclear, intraoperative cholangiography helps: in a centre performing it routinely, duct stones were found in 6.5% of patients[16].
Clots and anaesthesia
Deep vein thrombosis and pulmonary embolism are rare but serious, so patients walk on the day of surgery and some receive prophylaxis. Anaesthetic risk is a separate conversation with the anaesthetist; it relates mostly to other illnesses rather than to the operation itself.
Later problems
Hernia at an incision — most often at the larger, umbilical one.
Symptoms that persist after surgery. The numbers are worth knowing: in a study of 207 elective laparoscopic operations, 58% had symptoms in the first week but 13% at six months, and a specific cause was found in only a fraction of them[15].
Diarrhoea after surgery — there is a separate article on this; in a study of 3385 patients it occurred in 14.2% within a year[14].

On mortality
I do not give a single number. The reason: one average figure covering both a forty-year-old having elective surgery and an eighty-year-old with sepsis having emergency surgery is meaningless and misleading.
Real risk depends on urgency, inflammation severity, age, heart and lung function and other illnesses. Those are exactly the factors assessed before surgery, and your individual risk is worth discussing at the consultation, not an average.
After discharge, contact a doctor immediately if you develop fever or shivering, increasing abdominal pain, yellow skin or eyes, repeated vomiting, a swollen abdomen, discharge or redness around a wound, breathlessness, pain and swelling in a leg, or fainting.

I do not promise everything will be fine. The aim is for patients to know how to spot a complication, who to call, and that almost all can be dealt with if caught in time.
Frequently asked questions
How big is the risk of bile duct injury?It is not the same for everyone: lower in elective surgery on a quiet gallbladder, higher in emergency surgery on an inflamed one. So a question about a specific figure has to be tied to the situation, not to an average.
If my abdomen hurts and I have a fever after surgery, how long can I wait?Do not wait. Fever together with increasing pain after an operation is a reason to contact a doctor the same day.
Can symptoms remain even after a successful operation?They can. In one study 13% of patients still had symptoms at six months, and a specific cause was found in only some of them[15]. That is why it matters, before surgery, to establish whether the symptoms really come from the gallbladder.
Can the operation be done without general anaesthesia?No. This operation is performed under general anaesthesia. Anaesthetic risk is assessed separately by the anaesthetist.
