Es esmu ķirurgs, un man nav izdevīgi slavēt injekcijas. Tieši tāpēc šis raksts būs bez ierastās nostājas „zāles pret operāciju”: jaunās svara zāles ir spēcīgākais, kas farmakoloģijā parādījies pēdējās desmitgadēs, — un tām ir trīs nopietni „bet”, kurus reklāma nepiemin.
Kā tās strādā
GLP-1 ir zarnu hormons, kas dabiski izdalās pēc ēšanas. Zāles, kas atdarina tā darbību, — semaglutīds (Ozempic, Wegovy) un tirzepatīds (Mounjaro), kas iedarbojas arī uz GIP receptoru, — dara trīs lietas: mazina izsalkumu smadzenēs, palēnina kuņģa iztukšošanos un klusina pastāvīgās domas par ēdienu. Cilvēki apraksta to vienādi: „pirmo reizi mūžā es vienkārši nedomāju par ēšanu”.
Būtiski saprast: zāles nesadedzina taukus. Tās izslēdz to hormonu pārspēku, kura dēļ diētas nenotur svaru, — un ēst mazāk kļūst iespējams bez pastāvīgas cīņas.

Konteksts: tas ir divas līdz četras reizes vairāk, nekā vidēji dod dzīvesveida programma, un aptuveni divas trešdaļas līdz trīs ceturtdaļas no tā, ko dod operācija. Ar tirzepatīda lielāko devu vairāk nekā puse dalībnieku zaudēja 20% un vairāk[6].
Un vēl viens rezultāts, kas maina situāciju: pētījumā SELECT, kurā piedalījās vairāk nekā 17 000 dalībnieku ar sirds slimību un lieko svaru, semaglutīds samazināja infarktu, insultu un kardiovaskulārās nāves relatīvo risku par 20% (notikumi 6,5% pret 8,0%)[11]. Šīs zāles ārstē ne tikai skaitļus uz svariem.
Pirmais „bet”: pēc pārtraukšanas svars atgriežas
Šis ir vissvarīgākais fakts par visu zāļu grupu, un tas ir izmērīts abām zālēm:
gadu pēc semaglutīda pārtraukšanas dalībnieki bija atguvuši aptuveni divas trešdaļas zaudētā svara[7];
tirzepatīda pētījumā SURMOUNT-4 tie, kas pārgāja uz placebo, gada laikā atguva vidēji 14 procentpunktus, kamēr tie, kas turpināja, zaudēja vēl vairāk[12].
Secinājums nav „zāles nestrādā”. Secinājums: tās strādā, kamēr tās lieto, — kā spiediena vai vairogdziedzera zāles. Plāns „sadurstīšos līdz vasarai un beigšu” beidzas ar svara atgriešanos, un par to jāzina pirms pirmās injekcijas, ne pēc pēdējās.
Otrais „bet”: blaknes
Biežākās skar gremošanas traktu: slikta dūša, caureja, aizcietējums, atraugas, vemšana — visizteiktāk devas kāpināšanas laikā, pēc tam parasti mazinās. Pētījumos zāles bija jāpārtrauc retāk, nekā mēdz baidīties: kuņģa-zarnu blakņu dēļ — 4,5% semaglutīdam[5]; jebkuru blakņu dēļ tirzepatīdam — 4,3–7,1% atkarībā no devas (placebo — 2,6%)[6].
Par ko jārunā atsevišķi:
žultsakmeņi — straujš svara zudums tos veicina neatkarīgi no metodes; par mehānismu — rakstā par akmeņiem;
muskuļu zudums — daļa zaudētā svara vienmēr ir muskuļi; olbaltumvielas un spēka slodze nav izvēles lieta;
retas, bet nopietnas — pankreatīts; zāles nelieto grūtniecībā un tad, ja ģimenē ir rets iedzimts (medulārs) vairogdziedzera vēzis;
ja plānota operācija narkozē, par injekcijām jāpasaka anesteziologam — palēninātā kuņģa iztukšošanās dēļ.
Trešais „bet”: cena bez beigu datuma
Latvijā tās izsniedz ar recepti, un valsts to lietošanu aptaukošanās ārstēšanai nekompensē (pārbaudīts 2026. gada augustā). Cena atkarīga no zālēm un devas — ap 150 eiro mēnesī par mazākām devām, 300 un vairāk par pilnu devu; precīzas cenas svārstās, jautājiet aptiekā.
Rēķinot uz gadiem, aina mainās: injekcijas divu triju gadu garumā izmaksā aptuveni tikpat, cik viena operācija, — bet operācijas efekts turpinās arī pēc tam, kamēr zāļu efekts prasa turpināt maksāt. Šī aritmētika nav arguments pret zālēm; tā ir daļa no apzinātas izvēles, un katram tā sanāk citāda.
Kad zāles ir labākā izvēle — un kad nav
Zāles parasti ir pirmā nopietnā pakāpe, ja ir indikācijas — visbiežāk BMI no 30 vai no 27 ar blakusslimībām — un dzīvesveids viens pats nedod rezultātu: šeit tās bieži ir labāka izvēle nekā operācija. (Ja BMI ir no 35 vai ir 2. tipa diabēts ar BMI no 30, operāciju var apspriest arī uzreiz — visas pakāpes nav obligātas.) Tās der arī tiem, kam operācija nav pieņemama vai ir pārāk riskanta, un kā sagatavošanās ļoti liela svara gadījumā.
Robeža otrā virzienā: ja BMI pārsniedz 40 vai ir smagas komplikācijas, zāles vidēji nesasniedz operācijas efektu — īpaši diabēta gadījumā, kur ķirurģijai ir daudz ilgāki novērojumu dati. Un, ja mūža injekcijas nav reālistiskas — finansiāli vai psiholoģiski, — par to jārunā sākumā, ne pēc diviem gadiem.
Kas nāk pēc tam
Šī joma mainās ātri. Kombinācija kagrilintīds + semaglutīds (CagriSema) 3. fāzes pētījumā deva vidēji 20,4% 68 nedēļās[20], trīskāršais agonists retatrutīds 2. fāzē — līdz 24,2% 48 nedēļās[19]; tuvojas arī tabletes injekciju vietā. Skaitļi tuvojas ķirurģijai, bet pamatjautājums paliek tas pats: efekts turas, kamēr lieto. Šo sadaļu atjaunināšu, kad zāles būs nopērkamas Latvijas aptiekās.
No kā turēties pa gabalu: „Ozempic” bez receptes internetā un neskaidras izcelsmes semaglutīda maisījumi. Tur nav zināms ne saturs, ne deva. Šīs zāles izraksta ārsts, devu kāpina pakāpeniski un uzrauga — citādi tās nav ne drošas, ne efektīvas.
Galvenais no šī raksta
GLP-1 zāles ir īsts instruments, ne mode: 15–21% svara zuduma un semaglutīdam pierādīts ieguvums sirdij. Bet tā ir ilgstoša, bieži nenoteikti ilga terapija ar mēneša maksājumu un blaknēm, ne kurss. Izvēle starp zālēm, endoskopiju un operāciju ir atkarīga no svara, veselības, naudas un jūsu attieksmes — un to izlemjam konsultācijā, ne reklāmā.

Препараты для снижения веса: Ozempic, Wegovy, Mounjaro и другие — как они работают и чего ждать
Я хирург, и мне нет никакой выгоды хвалить инъекции. Именно поэтому в этой статье не будет привычного противопоставления «препараты против операции»: новые препараты для снижения веса — самое сильное, что появилось в фармакологии за десятилетия, и у них есть три серьёзных «но», о которых молчит реклама.
Как они работают
GLP-1 — кишечный гормон, который в норме выделяется после еды. Препараты, имитирующие его действие, — семаглутид (Ozempic, Wegovy) и тирзепатид (Mounjaro), который дополнительно действует на рецептор GIP, — делают три вещи: гасят голод в мозге, замедляют опорожнение желудка и приглушают постоянные мысли о еде. Люди описывают это одинаково: «первый раз в жизни я просто не думаю о еде».
Важно понимать: препараты не сжигают жир. Они выключают тот гормональный перевес, из-за которого диеты не удерживают вес, — и можно есть меньше без постоянной борьбы.

Контекст: это в два–четыре раза больше, чем в среднем даёт программа изменения образа жизни, и примерно две трети — три четверти того, что даёт операция. На максимальной дозе тирзепатида больше половины участников потеряли 20% и больше[6].
И ещё один результат, который меняет картину: в исследовании SELECT, в котором участвовали более 17 000 человек с болезнью сердца и лишним весом, семаглутид снизил относительный риск инфарктов, инсультов и сердечно-сосудистой смерти на 20% (события у 6,5% против 8,0%)[11]. Эти препараты влияют не только на цифру на весах.
Первое «но»: после отмены вес возвращается
Это самый важный факт обо всей группе, и он измерен для обоих препаратов:
через год после отмены семаглутида участники вернули примерно две трети потерянного веса[7];
в исследовании SURMOUNT-4 те, кто перешёл на плацебо, за год вернули в среднем 14 процентных пунктов, тогда как продолжавшие лечение похудели ещё[12].
Вывод не «препараты не работают». Вывод: они работают, пока их принимаешь, — как лекарства от давления или для щитовидной железы. План «поколюсь до лета и закончу» заканчивается возвратом веса, и знать это надо до первой инъекции, а не после последней.
Второе «но»: побочные эффекты
Самые частые — желудочно-кишечные: тошнота, понос или запор, отрыжка, рвота. Они чаще возникают при повышении дозы, а затем обычно стихают. В исследованиях лечение прекращали реже, чем принято бояться: из-за желудочно-кишечных явлений — 4,5% на семаглутиде[5]; из-за любых побочных эффектов на тирзепатиде — 4,3–7,1% в зависимости от дозы (на плацебо — 2,6%)[6].
О чём стоит говорить отдельно:
желчные камни — быстрое похудение способствует их образованию при любом методе; механизм — в статье о камнях;
потеря мышц — часть потерянного веса всегда мышцы; белок и силовая нагрузка — не просто пожелание, а обязательное условие;
редкое, но серьёзное — панкреатит; препараты не применяют при беременности и при редком наследственном (медуллярном) раке щитовидной железы в семье;
если предстоит операция под наркозом, об инъекциях надо сказать анестезиологу — из-за замедленного опорожнения желудка.
Третье «но»: цена без даты окончания
В Латвии эти препараты рецептурные, и государство их применение для лечения ожирения не компенсирует (проверено в августе 2026). Цена зависит от препарата и дозы — примерно от 150 евро в месяц при низких дозах до 300 евро и более при полной дозе; точные цены меняются, уточняйте в аптеке.
В пересчёте на годы картина меняется: два-три года инъекций стоят примерно столько же, сколько одна операция, — но эффект операции продолжается и потом, а за эффект препаратов надо продолжать платить. Эта арифметика не аргумент против препаратов; это часть взвешенного выбора, и у каждого она своя.
Когда препараты — лучший выбор, а когда нет
Препараты — обычно первая серьёзная ступень при наличии показаний — чаще всего BMI от 30 или от 27 с сопутствующими болезнями, — когда образ жизни не даёт результата: здесь они часто лучше операции. (При BMI от 35 или диабете 2-го типа с BMI от 30 операцию можно обсуждать и сразу — проходить все ступени не обязательно.) Они подходят тем, для кого операция неприемлема или слишком рискованна, а также для подготовки к операции при очень большом весе.
Обратная ситуация: при BMI выше 40 или тяжёлых осложнениях препараты в среднем не достигают эффекта операции — особенно по диабету, где у хирургии многолетние данные. И если пожизненные инъекции нереалистичны — финансово или психологически, — говорить об этом надо в начале, а не через два года.
Что дальше
Эта область меняется быстро. Комбинация кагрилинтид + семаглутид (CagriSema) в 3-й фазе дала в среднем 20,4% за 68 недель[20], тройной агонист ретатрутид во 2-й фазе — до 24,2% за 48 недель[19]; на подходе и таблетки вместо инъекций. Цифры приближаются к хирургии, но базовый вопрос остаётся тем же: эффект держится, пока принимаешь. Обновлю этот раздел, когда препараты реально дойдут до латвийских аптек.
От чего держаться подальше: «Ozempic» без рецепта в интернете и кустарные смеси семаглутида с непонятных сайтов. Там неизвестны ни состав, ни доза. Эти препараты назначает врач, дозу повышают постепенно и под контролем — иначе они не будут ни безопасными, ни эффективными.

Препараты GLP-1 — настоящий инструмент, а не мода: 15–21% потери веса и доказанная у семаглутида польза для сердца. Но это длительная терапия, часто на неопределённый срок, с ежемесячным платежом и побочными эффектами, а не курс. Выбор между препаратами, эндоскопией и операцией зависит от веса, здоровья, денег и ваших предпочтений; делаем его на консультации.
Частые вопросы
Чем Ozempic отличается от Wegovy?Оба содержат семаглутид. Ozempic зарегистрирован для лечения диабета 2-го типа в меньших дозах, Wegovy — именно для лечения ожирения, в дозе до 2,4 мг. Для снижения веса предназначен Wegovy.
Можно ли пройти курс и закончить?Можно, но вес после этого в основном возвращается: через год после отмены семаглутида участники вернули около двух третей потерянного. Это поддерживающая терапия, а не курс.
Сколько это стоит в Латвии?Государственной компенсации для лечения ожирения нет (проверено в августе 2026). В зависимости от препарата и дозы — примерно от 150 до 300 евро и больше в месяц; точные цены уточняйте в аптеке.
Можно ли сочетать препараты с операцией?Можно, и это делают: перед операцией при очень большом весе и иногда после операции, если вес через годы начинает возвращаться. Комбинацию выбирает команда врачей.

Weight-loss drugs: Ozempic, Wegovy, Mounjaro and beyond — how they work and what to expect
I am a surgeon, and I have nothing to gain by praising injections. Which is exactly why this article skips the usual "drugs versus surgery" debate: the new weight-loss drugs are the most effective obesity drugs developed in decades — and they come with three serious "buts" that the advertising leaves out.
How they work
GLP-1 is a gut hormone normally released after eating. The drugs that imitate it — semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro), which additionally acts on the GIP receptor — do three things: they quiet hunger in the brain, slow stomach emptying and mute the constant thoughts about food. People describe it the same way: "for the first time in my life I simply do not think about eating."
It is important to understand that the drugs do not burn fat. They switch off the hormonal upper hand that stops diets from keeping the weight off — and eating less becomes possible without a constant fight.
The real numbers

Context: that is two to four times what a lifestyle programme averages, and roughly two thirds to three quarters of what surgery delivers. On the highest tirzepatide dose, more than half of participants lost 20% or more[6].
And one more result that changes the conversation: in the SELECT trial of over 17,000 people with heart disease and excess weight, semaglutide cut the relative risk of heart attacks, strokes and cardiovascular deaths by 20% (events in 6.5% versus 8.0%)[11]. These drugs treat more than the number on the scale.
The first "but": weight returns after stopping
This is the most important fact about the whole class, and it has been measured for both drugs:
a year after stopping semaglutide, participants had regained about two thirds of the weight they had lost[7];
in SURMOUNT-4, those switched to placebo regained a mean of 14 percentage points within a year, while those who continued lost more[12].
The conclusion is not "the drugs do not work". The conclusion is: they work while you take them — like blood-pressure or thyroid medication. The plan "I'll inject until summer and then stop" ends in regain, and that has to be known before the first injection, not after the last.
The second "but": side effects
The most common are gastrointestinal: nausea, diarrhoea or constipation, belching, vomiting — strongest during dose escalation, then usually subsiding. In the trials, stopping was rarer than people fear: 4.5% on semaglutide because of gastrointestinal effects[5], and 4.3–7.1% on tirzepatide for any adverse events, depending on dose (2.6% on placebo)[6].
Worth discussing separately:
gallstones — rapid weight loss promotes them with any method; the mechanism is in the gallstone article;
muscle loss — part of any weight lost is muscle; protein and strength training are essential, not optional;
rare but serious — pancreatitis; the drugs are not used in pregnancy or if there is a family history of a rare hereditary (medullary) thyroid cancer;
if an operation under anaesthesia is planned, tell the anaesthetist about the injections — because of the slowed stomach emptying.
The third "but": a cost with no end date
In Latvia these are prescription drugs, and the state does not reimburse them for obesity treatment (checked August 2026). The price depends on the drug and the dose — from roughly 150 euros a month at lower doses to 300 or more at the full dose; exact prices vary — ask at the pharmacy.
Over several years, the picture changes: two to three years of injections cost about as much as a single operation — but the effect of surgery continues afterwards, while the drugs' effect requires continued payment. That arithmetic is not an argument against the drugs; it is part of an informed choice, and it works out differently for everyone.
When the drugs are the better choice — and when not
Medication is usually the first serious step when the indications are met — most often a BMI of 30 or above, or 27 or above with comorbidities — and lifestyle alone has not delivered: here it is often a better choice than surgery. (With a BMI of 35 or above, or type 2 diabetes with a BMI of 30 or above, surgery can also be discussed straight away — climbing every step first is not required.) It also suits those for whom surgery is unacceptable or too risky, and serves as preparation at very high weights.
At the other end, for people with a BMI above 40 or severe complications, drugs on average do not match surgery — especially for diabetes, where surgery has far longer follow-up data. And if lifelong injections are unrealistic — financially or psychologically — that belongs at the start of the conversation, not two years in.
What comes next
This field is moving fast. The combination cagrilintide + semaglutide (CagriSema) averaged 20.4% at 68 weeks in phase 3[20]; the triple agonist retatrutide reached up to 24.2% at 48 weeks in phase 2[19]; tablets that could replace injections are on the horizon. The numbers edge towards surgery, but the core question stays the same: the effect lasts while you take the drug. I will update this section when these reach Latvian pharmacies.
Stay away from: "Ozempic" without a prescription on the internet, and compounded semaglutide mixes from unreliable websites. Their content and dose are not reliably controlled. These drugs are prescribed by a doctor, with the dose escalated gradually, and monitored — otherwise they are neither safe nor effective.
The main point of this article
These drugs are effective: 15–21% weight loss; semaglutide has proven heart benefit. But they are long-term, often open-ended therapy with a monthly bill and side effects, not a course. The choice depends on weight, health, cost and your preferences — and it is made during a consultation, not through an advertisement.

What is the difference between Ozempic and Wegovy?Both contain semaglutide. Ozempic is approved for type 2 diabetes at lower doses; Wegovy specifically for obesity treatment at up to 2.4 mg. The weight-loss product is Wegovy.
