Par ko šie skaitļi klusē. Tie ir pētījumu vidējie rādītāji: daļa cilvēku zaudē mazāk. Un neviens no tiem nav „kurss ar beigām” — aptaukošanās ir hroniska slimība, un rezultātu notur tikai turpināta ārstēšana vai paliekoša metode ar novērošanu.
Kāpēc agrāk citētie procenti šķiet lielāki
Ķirurģijā vēsturiski lieto citu mērvienību — % no liekā svara (%EWL). „60–80% liekā svara” un „15% no kopējās masas” izklausās kā milzīga starpība, bet daļēji tā ir aritmētika: cilvēkam ar 120 kg un ideālo svaru 75 kg zaudēt 60% liekā svara nozīmē 27 kg — tas ir 22,5% no kopējās masas. Šajā ciklā visi skaitļi ir %TWL, lai metodes varētu salīdzināt vienotā skalā.
Kā izvēlas ceļu
Nevis pēc viena skaitļa, bet pēc kopainas:
Cik smaga ir slimība. Ja BMI ir zem 35, parasti sāk ar dzīvesveidu; zāļu indikācijas vērtē atsevišķi. Starptautiskās vadlīnijas operāciju iesaka no BMI 35, bet no BMI 30 to apsver, ja ir vielmaiņas slimība — vispirms 2. tipa diabēts — vai ja neķirurģiskā ārstēšana nav devusi noturīgu rezultātu[9].
Kas jau izmēģināts. Ja vairākas nopietnas programmas beigušās ar svara atgriešanos, atkārtot to pašu ceturto reizi nav risinājums.
Blakusslimības. Diabēts, smaga atviļņa slimība, apnoja, locītavu problēmas — katra ietekmē metodes izvēli.
Jūsu attieksme. Vienam nepieņemama doma par injekcijām uz mūžu, citam — par operāciju. Abas nostājas ir leģitīmas, un plāns tām jāpielāgo.
Cena ilgtermiņā. Zāles ir maksājums katru mēnesi bez beigu datuma; operācija — vienreizējs ieguldījums plus vitamīni un kontroles. Konkrētas summas ar datumiem būs rakstā par zālēm.
Kā metodes strādā kopā
Praksē ceļi nekonkurē, bet savienojas: zāles mēdz lietot pirms operācijas, lai samazinātu svaru un operācijas risku; pēc operācijas tās dažreiz pievieno, ja svars pēc gadiem sāk atgriezties; un dzīvesveida programma ir obligāts pamats jebkurai metodei — bez tās arī ķirurģijas rezultāts ir sliktāks.
Ko negaidīt ne no viena ceļa
Neviens nedod garantētu skaitli konkrētam cilvēkam — visi skaitļi ir vidējie.
Neviens nedarbojas bez jūsu līdzdalības — arī pēc operācijas ēšanas paradumi ietekmē rezultātu.
Neviens nav „vienreiz un uz visiem laikiem” — hroniska slimība prasa novērošanu jebkurā scenārijā.
Ar ko sākt praktiski
Ceļš parasti sākas nevis pie ķirurga, bet pie ģimenes ārsta vai endokrinologa: izmeklējumi no pirmā raksta, blakusslimību novērtējums, pirmā pakāpe. Pie manis konsultācijā nonāk tie, kam apspriežama endoskopiska vai ķirurģiska metode — un daļai no viņiem es iesaku ar operāciju nesteigties. Lēmumu pieņemam kopā, un komanda — uztura speciālists, endokrinologs, vajadzības gadījumā psihologs — ir daļa no jebkura ceļa.
No kā izvairīties: „tauku dedzinātāji” un uztura bagātinātāji svara zaudēšanai bez pierādījumiem, ilgstoša badošanās bez uzraudzības, kā arī jebkura programma, kas sola konkrētu kilogramu skaitu konkrētā termiņā. Drošas metodes tā nesola.
Biežākie jautājumi
Vai vienmēr jāsāk ar diētu, pirms drīkst domāt par zālēm vai operāciju?Dzīvesveida pamats vajadzīgs vienmēr, bet smagas klīniskas aptaukošanās gadījumā vadlīnijas neprasa gadiem „izciest” neveiksmīgas diētas pirms nākamās pakāpes. Pakāpi izvēlas pēc slimības smaguma, ne pēc izturības pārbaudes.
Kura metode ir vislabākā?Tādas nav. Zāles un operācija dod lielāko vidējo svara zudumu, bet izvēli nosaka slimības smagums, blakusslimības, iepriekšējā pieredze, cena un jūsu attieksme. Tāpēc šī ir karte, ne reitings.
Ko nozīmē %TWL un %EWL?%TWL ir zudums procentos no visa ķermeņa svara, %EWL — procentos no liekā svara virs ideālā. Viens un tas pats rezultāts EWL izteiksmē izskatās lielāks, tāpēc metodes drīkst salīdzināt tikai vienā mērvienībā.
Vai zāles un operāciju var kombinēt?Var, un to dara: zāles pirms operācijas ļoti liela svara gadījumā un dažreiz pēc operācijas, ja svars atgriežas. Kombināciju izvēlas komanda, ne pacients viens pats.

Как лечат ожирение: образ жизни, препараты, эндоскопия или операция
Я хирург, и именно поэтому начну эту статью с защиты других методов: большинству людей с лишним весом операция не нужна. Эта статья — карта всех дорог, чтобы вы увидели выбор целиком, прежде чем читать о каждом методе отдельно.
Четыре ступени
В лечении ожирения используют четыре инструмента: их сочетают и подбирают индивидуально — начинать можно не только с первой ступени:
программа образа жизни — питание, движение, сон, поддержка; основа всего остального;
препараты — сегодня прежде всего инъекции группы GLP-1;
эндоскопические методы — желудочный баллон и эндоскопическая гастропластика (ESG);
хирургия — продольная резекция желудка (sleeve) или шунтирование (bypass).
Одна таблица для всего
Чтобы цифры можно было сравнивать, везде одна единица — % от общей массы тела (%TWL). У каждой цифры указан срок исследования; сравнивать можно только похожие сроки.

О чём эти цифры молчат. Это средние значения исследований: часть людей теряет меньше. И ни один путь не «курс с окончанием» — ожирение — хроническая болезнь, и результат сохраняется только при длительном лечении или постоянном методе с наблюдением.
Почему привычные проценты выглядят больше
В хирургии исторически используют другую единицу — % от лишнего веса (EWL). «60–80% лишнего веса» и «15% от общей массы» звучат как огромная разница, но отчасти это арифметика: для человека с массой тела 120 кг и идеальным весом 75 кг потерять 60% лишнего веса — это 27 кг, то есть 22,5% общей массы. В этом цикле все цифры в %TWL, чтобы методы сравнивались по одной шкале.
Как выбирают путь
Не по одной цифре, а по совокупности факторов:
Насколько тяжела болезнь. При BMI до 35 обычно начинают с образа жизни, добавляя препараты по показаниям. Международные рекомендации советуют операцию при BMI от 35, а от 30 её рассматривают при метаболической болезни — прежде всего диабете 2-го типа — или когда нехирургическое лечение не дало устойчивого результата[9].
Что уже пробовали. Если несколько серьёзных программ закончились возвратом веса, повторять то же самое в четвёртый раз — не план.
Сопутствующие болезни. Диабет, тяжёлый рефлюкс, апноэ, болезни суставов — всё это влияет на выбор метода.
Ваше отношение. Для одного неприемлема мысль о пожизненных инъекциях, для другого — об операции. Обе позиции приемлемы, и план строится с их учётом.
Долгосрочные расходы. Препараты — платёж каждый месяц без даты окончания; операция — разовое вложение плюс витамины и контроль. Конкретные суммы с датами будут в статье о препаратах.
Как методы работают вместе
На практике пути не конкурируют, а соединяются: препараты применяют перед операцией, чтобы при очень большом весе уменьшить массу тела и размер печени; после операции их иногда добавляют, если вес через годы начинает возвращаться; а программа образа жизни — обязательный фон для любого метода: без неё и результат хирургии хуже.
Чего не ждать ни от одного пути
Ни один не даёт гарантированную цифру конкретному человеку — все цифры средние.
Ни один не работает без вашего участия — и после операции пищевые привычки влияют на результат.
Ни один не «раз и навсегда» — хроническая болезнь требует наблюдения при любом сценарии.
С чего начать практически
Путь обычно начинается не у хирурга, а у семейного врача или эндокринолога: обследования из первой статьи, оценка сопутствующих болезней, первая ступень. Ко мне на консультацию попадают те, у кого обсуждается эндоскопический или хирургический метод — и части из них я советую с операцией не спешить. Решение принимаем вместе, и команда — диетолог, эндокринолог, при необходимости психолог — часть любого пути.
От чего держаться подальше: «жиросжигатели» и добавки для похудения без доказательств, длительное голодание без контроля, а также любая программа, обещающая конкретное число килограммов к конкретному сроку. Безопасные методы так не обещают.
Частые вопросы
Обязательно ли сначала «отсидеть» диету, прежде чем думать о препаратах или операции?Основа образа жизни нужна всегда, но при тяжёлом клиническом ожирении рекомендации не требуют годами «отбывать» неудачные диеты перед следующей ступенью. Ступень выбирают по тяжести болезни, а не по испытанию на стойкость.
Какой метод самый лучший?Такого нет. У препаратов и операции самые большие средние цифры, но выбор определяют тяжесть болезни, сопутствующие заболевания, прошлый опыт, цена и ваше отношение. Поэтому это карта, а не рейтинг.
Что такое %TWL и %EWL?%TWL — потеря в процентах от всего веса тела, %EWL — в процентах от лишнего веса сверх идеального. Один и тот же результат в EWL выглядит заметно больше, поэтому методы сравнивают только в одних единицах.
Можно ли сочетать препараты и операцию?Можно, и это делают: препараты перед операцией при очень большом весе и иногда после операции, если вес возвращается. Комбинацию выбирает команда, а не пациент в одиночку.

How obesity is treated: lifestyle, medication, endoscopy or surgery
I am a surgeon, and that is exactly why I open this article in defence of the other methods: most people with excess weight do not need an operation. This article is a map of all the roads — so you can see all the options before reading about each method separately.
Four steps
Obesity treatment has four tools, combined and chosen individually — you do not have to start at the bottom step:
a lifestyle programme — food, movement, sleep, support; the foundation of everything else;
medication — today, above all, the GLP-1 class of injections;
endoscopic methods — the gastric balloon and endoscopic sleeve gastroplasty (ESG);
surgery — sleeve gastrectomy or gastric bypass.
One table for everything
To make the numbers comparable, one unit is used throughout — % of total body weight (%TWL). Each figure carries its study duration; only similar periods may be compared.

What these numbers do not say: they are trial averages; individual results vary. Obesity is a chronic disease, and long-term support is needed to maintain the result.
Why the familiar percentages look bigger
Surgery has historically used a different unit — % of excess weight (EWL). "60–80% of excess weight" and "15% of total weight" sound worlds apart, but partly it is arithmetic: for someone weighing 120 kg with an ideal weight of 75 kg, losing 60% of the excess means 27 kg — which is 22.5% of total weight. In this series every figure is %TWL, so the methods sit on one common scale.
How the road is chosen
Not by a single number, but by the whole picture:
How severe the disease is. Below BMI 35, treatment starts with lifestyle, adding medication when indicated. International guidelines recommend surgery at a BMI of 35 or above, and consider it at a BMI of 30 or above when metabolic disease — above all type 2 diabetes — is present, or when non-surgical treatment has not produced a lasting result[9].
What has been tried. If several serious programmes have ended in regain, repeating the same thing a fourth time is not a plan.
Comorbidities. Diabetes, severe reflux, apnoea, joint disease — each affects the choice differently.
Your preferences. For one person lifelong injections are unacceptable; for another, an operation is. Both positions are legitimate, and the plan is built around them.
Cost over the long run. Medication is a monthly payment with no end date; surgery has an upfront cost plus vitamins and check-ups. Current cost figures will follow in the medication article.
How the methods work together
In practice the roads connect rather than compete: medication is used before surgery to lower anaesthetic risk at very high weights; it is sometimes added after surgery if weight starts creeping back years later; and a lifestyle programme is the essential foundation of every method — without it, even surgical results are worse.
What not to expect from any road
None gives a guaranteed figure for an individual — all the numbers are averages.
All need your participation — after surgery too, eating habits influence the result.
None is "once and done" — a chronic disease needs follow-up in every scenario.
Where to start in practice
The road usually starts not with a surgeon but with a family doctor or endocrinologist: the tests from the first article, an assessment of comorbidities, the first step. My consultation is for people considering an endoscopic or surgical method — and some of them I advise not to hurry into surgery. The decision is made together, and the team — a dietitian, an endocrinologist and, when needed, a psychologist — is part of every road.
What to stay away from: "fat burners" and weight-loss supplements without evidence, prolonged unsupervised fasting, and any programme that promises a specific number of kilograms by a specific date. Safe methods do not make that promise.
Frequently asked questions
Do I always have to complete a diet before medication or surgery can be considered?A lifestyle foundation is always needed, but in severe clinical obesity the guidelines do not demand years of failed diets before the next step. The step is chosen by disease severity, not as an endurance test.
Which method is the best?There is no such method. Medication and surgery give the greatest average loss, but the choice depends on disease severity, comorbidities, past attempts, cost and your own stance. That is why this is a map, not a ranking.
What are %TWL and %EWL?%TWL is loss as a percentage of total body weight; %EWL is loss as a percentage of the excess above ideal weight. The same result is numerically larger in EWL, so methods should only be compared in one unit.
Can medication and surgery be combined?Yes, and they are: medication before surgery at very high weights, and sometimes after surgery if weight returns. The combination is chosen by the team, not by the patient alone.
