Šo sarunu es nekad neatstāju uz beigām. Cilvēkam, kurš apsver bariatrisko operāciju, ir tiesības zināt ne tikai vidējos rezultātus, bet arī to, kas var noiet greizi, cik bieži tas notiek un ko tādā gadījumā dara.
Pareizais atskaites punkts
Operācijas risku nav jēgas vērtēt pret nulli — reālā alternatīva nav „bez riska”, bet neārstētas smagas aptaukošanās risks: diabēts, sirds slimības, apnoja. SOS pētījumā 90 dienu mirstība pēc operācijas bija 0,2%, bet ilgtermiņā operētajiem nāves risks bija par 23% zemāks nekā neoperētajiem[10]. Salīdzinājums iet abos virzienos, un tāpēc lēmums vienmēr jāizvērtē individuāli.
Agrīnās komplikācijas — pirmās dienas un nedēļas
Šuvju līnijas vai savienojuma noplūde — nopietnākā agrīnā komplikācija: kuņģa saturs nonāk vēdera dobumā. Izpaužas ar pieaugošām sāpēm, drudzi, paātrinātu pulsu. Ārstē ar drenāžu, endoskopiskām metodēm vai atkārtotu operāciju — un iznākumu izšķir tas, cik ātri tiek noteikta diagnoze.
Asiņošana — parasti pirmajās stundās vai dienās; lielākoties apstājas ar zālēm un novērošanu, retāk vajadzīga atkārtota laparoskopija.
Trombozes un plaušu embolija — tāpēc jau operācijas dienā liek celties un staigāt, un lielākā daļa pacientu saņem asins šķidrinātājus profilaksei.
Brūču infekcija — laparoskopijā reta un parasti viegli ārstējama.
Kopējais nopietnu agrīno komplikāciju līmenis mūsdienu centros ir daži procenti, un lielākā daļa tiek atrisināta bez paliekošām sekām. Bet skaitļi ir vidējie — jūsu personīgo risku veido blakusslimības, svars un sagatavotība.
Vēlīnās komplikācijas — mēneši un gadi
Sašaurinājums (striktūra) — biežāk pēc bypass savienojuma vietā; izpaužas ar rīšanas grūtībām un vemšanu; parasti ārstē ar endoskopisku paplašināšanu.
Čūla savienojuma vietā pēc bypass — īpaši smēķētājiem un NSAID lietotājiem; tāpēc jāizvairās gan no smēķēšanas, gan no NSAID.
Iekšējā trūce pēc bypass — zarnu cilpa iesprūst starp audiem; izpaužas ar lēkmjveida vēdersāpēm, dažreiz mēnešiem pēc operācijas; prasa operāciju.
Žultsakmeņi — straujš svara zudums tos veicina; par to atsevišķs raksts.
Atvilnis pēc sleeve — līdz trešdaļai pacientu tas pasliktinās[15]; smagos gadījumos konvertē uz bypass.
Vitamīnu un minerālvielu deficīti — dzelzs, B12, D vitamīns, kalcijs; tāpēc uzraudzība un piedevas ir mūža daļa[18].
Dempinga sindroms pēc bypass — pēc salda vai trekna ēdiena strauji rodas vājums, svīšana, sirdsklauves; tas ir nepatīkams, bet kontrolējams ar uztura korekciju.
Ādas pārpalikums — pēc liela svara zuduma bieži; plastiku plāno ne agrāk kā pēc svara stabilizācijas.
Divas lietas, par kurām runā par maz
Alkohols. Pēc bariatriskās operācijas, īpaši bypass, alkohols uzsūcas ātrāk un pastiprināti, un alkohola pārmērīgas lietošanas risks gados pēc operācijas pieaug. Ja attiecības ar alkoholu jau ir sarežģītas, par to jārunā pirms operācijas.
Garastāvoklis. Vairumam dzīves kvalitāte uzlabojas, bet daļai pirmais gads pēc operācijas ir emocionāli grūts — mainās ēšana kā mierinājums, attiecības, pašuztvere. Psihologa atbalsts šajā posmā nav vājuma pazīme, bet daļa no ārstēšanas.
Sagatavošanās, kas reāli maina risku
Smēķēšanas pārtraukšana — vismaz 4–6 nedēļas pirms operācijas; smēķēšana palielina noplūdes, čūlas un plaušu komplikāciju risku.
Aknu samazināšanas diēta pirms operācijas — mazāka aknu kreisā daiva nozīmē drošāku piekļuvi kuņģim.
Diabēta un spiediena sakārtošana ar endokrinologu un ģimenes ārstu.
Apnojas pārbaude — neārstēta apnoja ir narkozes risks; dažiem pirms operācijas vajadzīga CPAP terapija.
Gastroskopija un analīzes — bāzes stāvoklis, ar kuru pēc tam salīdzināsim.
GLP-1 zāles vai balons ļoti liela svara gadījumā — kā sagatavošanās solis atsevišķiem pacientiem pēc komandas lēmuma.
Pēc izrakstīšanās zvaniet nekavējoties, ja parādās: drudzis virs 38 °C, pieaugošas vēdersāpes, paātrināts pulss miera stāvoklī, elpas trūkums vai sāpes krūtīs, atkārtota vemšana, nespēja dzert, melni izkārnījumi, apsārtums vai izdalījumi no brūces, sāpes un pietūkums kājā. Ar šīm pazīmēm negaida plānoto vizīti — tās pārbauda tajā pašā dienā.

Bariatriskā operācija mūsdienās ir droša ķirurģija ar zināmu, vadāmu komplikāciju spektru — un ar risku, kas jāsver pret neārstētas slimības risku, ne pret nulli. Kura komplikācija ir visbīstamākā?Šuvju vai savienojuma noplūde pirmajās dienās. Tāpēc pēcoperācijas pazīmes — drudzis, pieaugošas sāpes, ātrs pulss — nekad nav „jāpacieš līdz rītam”, bet jāziņo uzreiz.
Vai vitamīni tiešām jālieto visu mūžu?Jā, īpaši pēc bypass. Tas nav sīkums: dzelzs, B12 un D vitamīna deficīti attīstās klusi un gadu gaitā. Kontroles analīzes ir daļa no plāna.
Kāpēc pirms operācijas jāatmet smēķēšana?Smēķēšana palielina noplūdes, čūlas un plaušu komplikāciju risku — tas ir viens no nedaudzajiem riska faktoriem, kuru pilnībā kontrolējat jūs pats.

Правильная точка отсчёта
Риск операции нет смысла сравнивать с нулём: реальная альтернатива — не отсутствие риска, а риск нелеченого тяжёлого ожирения (диабет, болезни сердца, апноэ). В исследовании SOS 90-дневная смертность после операции была 0,2%, а за годы наблюдения у оперированных риск смерти был на 23% ниже, чем у неоперированных[10]. Сравнение идёт в обе стороны, поэтому решение всегда индивидуально.
Ранние осложнения — первые дни и недели
Несостоятельность швов или соединения — самое серьёзное раннее осложнение: содержимое желудка попадает в брюшную полость. Проявляется нарастающей болью, температурой, учащённым пульсом. Лечат дренированием, эндоскопическими методами или повторной операцией — и исход зависит от времени до постановки диагноза.
Кровотечение — обычно в первые часы или дни; в большинстве случаев останавливается при наблюдении и лечении, реже нужна повторная лапароскопия.
Тромбозы и лёгочная эмболия — поэтому уже в день операции пациентов поднимают и заставляют ходить, и большинство из них получают профилактику разжижающими кровь препаратами.
Инфекция ран — при лапароскопии редка и обычно легко лечится.
Общий уровень серьёзных ранних осложнений в современных центрах — единицы процентов, и большинство проходит без стойких последствий. Но цифры средние — ваш личный риск складывается из сопутствующих болезней, веса и подготовки.
Поздние осложнения — месяцы и годы
Сужение (стриктура) — чаще после bypass в месте соединения; проявляется затруднённым глотанием и рвотой; обычно лечится эндоскопическим расширением.
Язва в месте соединения после bypass — особенно у курильщиков и тех, кто принимает NSAID; поэтому курение и NSAID под запретом.
Внутренняя грыжа после bypass — петля кишки ущемляется между тканями; проявляется приступообразной болью в животе, иногда через месяцы после вмешательства; требует операции.
Желчные камни — быстрое похудение им способствует; об этом отдельная статья.
Рефлюкс после sleeve — у части пациентов (до трети) он усиливается[15]; в тяжёлых случаях конвертируют в bypass.
Дефициты витаминов и минералов — железо, B12, витамин D, кальций; поэтому контроль и добавки — часть жизни[18].
Демпинг-синдром после bypass — после сладкой или жирной еды резко возникают слабость, потливость, сердцебиение; неприятен, но контролируется коррекцией питания.
Избыток кожи — частое явление после большой потери веса; вопрос пластики рассматривают не раньше стабилизации веса.
Две вещи, о которых говорят слишком мало
Алкоголь. После бариатрической операции, особенно bypass, алкоголь всасывается быстрее и интенсивнее, и риск злоупотребления в последующие годы возрастает. Если отношения с алкоголем уже сложные, об этом надо говорить до операции.
Настроение. У большинства качество жизни улучшается, но у части послеоперационный год эмоционально труден — меняются роль еды как утешения, отношения и самовосприятие. Поддержка психолога на этом этапе — не признак слабости, а часть лечения.
Подготовка, которая реально меняет риск
Отказ от курения — минимум за 4–6 недель до операции; курение повышает риск несостоятельности, язвы и лёгочных осложнений.
Диета для уменьшения печени перед операцией — меньшая левая доля печени означает более безопасный доступ к желудку.
Контроль диабета и давления с эндокринологом и семейным врачом.
Проверка на апноэ — нелеченое апноэ повышает риск наркоза; некоторым перед операцией нужна CPAP-терапия.
Гастроскопия и анализы — базовое состояние, с которым потом будем сравнивать.
GLP-1-препараты или баллон при очень большом весе — как подготовительный мост у отдельных пациентов, по решению команды.
После выписки звоните немедленно, если появились: температура выше 38 °C, нарастающая боль в животе, учащённый пульс в покое, одышка или боль в груди, повторная рвота, невозможность пить, чёрный стул, покраснение или выделения из раны, боль и отёк ноги. С этими признаками не ждут планового визита — их проверяют в тот же день.

I never leave this conversation for last. Someone considering bariatric surgery has the right to know not only the average results but also what can go wrong, how often it happens, and what is done when it does.
The right reference point
There is no sense in measuring surgical risk against zero — the real alternative is not “no risk” but the risk of untreated severe obesity: diabetes, heart disease, apnoea. In the SOS study, 90-day mortality after surgery was 0.2%, while in the long run patients who had surgery had a 23% lower risk of death than those who did not[10]. The comparison runs both ways, which is why the decision is always an individual balance.
Early complications — the first days and weeks
Leak from the staple line or the join — the most serious early complication: stomach contents reach the abdominal cavity. Signs include increasing pain, fever and a racing pulse. Treated with drainage, endoscopic methods or reoperation — and the outcome depends on prompt diagnosis.
Bleeding — usually in the first hours or days; generally stops with medication and observation, occasionally requiring repeat laparoscopy.
Clots and pulmonary embolism — which is why patients are up and walking on the day of surgery, and most receive blood-thinner prophylaxis.
Wound infection — rare with laparoscopy and usually easy to treat.
The overall rate of serious early complications in modern centres is in the low single-digit percentages, and most resolve without lasting consequences. But the figures are averages — your personal risk depends on comorbidities, weight and preparation.
Late complications — months and years
Narrowing (stricture) — more often after bypass at the join; causes difficulty swallowing and vomiting; usually treated with endoscopic dilation.
An ulcer at the join after bypass — especially with smoking and NSAID use, which is why both must be avoided.
Internal hernia after bypass — a loop of bowel becomes trapped between tissues; presents as crampy abdominal pain, sometimes months after surgery; requires an operation.
Gallstones — rapid weight loss promotes them; there is a separate article.
Reflux after sleeve — it worsens in up to a third of patients[15]; severe cases are converted to bypass.
Vitamin and mineral deficiencies — iron, B12, vitamin D, calcium — which is why monitoring and supplements are part of life[18].
Dumping syndrome after bypass — weakness, sweating and palpitations soon after sweet or fatty food; unpleasant, but managed with dietary adjustment.
Excess skin — common after large weight loss; contouring surgery is considered only once weight has stabilised.
Two things that are discussed too little
Alcohol. After bariatric surgery, especially bypass, alcohol is absorbed faster and hits harder, and the risk of alcohol misuse rises in the years after surgery. If the relationship with alcohol is already complicated, it belongs in the conversation before the operation.
Mood. For most people, quality of life improves, but for some the first post-operative year is emotionally hard — food as comfort, relationships and self-image all change. A psychologist's support at this stage is not a sign of weakness but part of the treatment.
Preparation that genuinely changes the risk
Stopping smoking — at least 4–6 weeks before surgery; smoking raises the risk of leaks, ulcers and lung complications.
A liver-shrinking diet before surgery — a smaller left liver lobe means safer access to the stomach.
Getting diabetes and blood pressure in order with the endocrinologist and family doctor.
Screening for apnoea — untreated apnoea is an anaesthetic risk; some need CPAP therapy before surgery.
Gastroscopy and blood tests — the baseline against which we will later measure.
GLP-1 medication or a balloon at very high weights — as a preparatory bridge for selected patients, by team decision.
After discharge, call immediately if you develop: fever above 38 °C, increasing abdominal pain, a fast pulse at rest, breathlessness or chest pain, repeated vomiting, inability to keep fluids down, black stools, redness or discharge from a wound, pain and swelling in a leg. With these signs, do not wait for the planned visit — get assessed the same day.

Modern bariatric surgery is a safe procedure with a known, manageable spectrum of complications — and with a risk that is weighed against the risk of the untreated disease, not against zero. Preparation lowers the chance of complications, and noticing them in time limits their consequences. So my job is not only to operate but to teach you the signs worth a phone call.

How dangerous is the operation itself?In SOS, 90-day mortality was 0.2%, and the rate of serious early complications in modern laparoscopic centres is in the low single digits. Personal risk is assessed before surgery based on comorbidities and preparation.
Which complication is the most dangerous?A staple-line or join leak in the first days. That is why post-operative signs — fever, increasing pain, a fast pulse — are never “endured until morning” but reported at once.
Do I really need vitamins for life?Yes, especially after bypass. It is not a formality: iron, B12 and vitamin D deficiencies develop silently over years. Follow-up blood tests are part of the plan.
Why must I stop smoking before surgery?Smoking raises the risk of leaks, ulcers and lung complications — one of the few risk factors entirely under your own control.
