Uz šo jautājumu cilvēki parasti atbild paši — un parasti nepareizi abos virzienos. Vieni gadiem ārstē „slinkumu”, lai gan viņiem ir hroniska slimība. Citi satraucas par kilogramiem, kas veselību neapdraud. Šis raksts ir par to, kur ir robeža.
Aptaukošanās nav rakstura īpašība
Sāksim ar galveno. Aptaukošanās ir hroniska slimība ar savu bioloģiju, nevis gribasspēka trūkums. Ķermeņa svaru regulē smadzenes un hormoni, un šī sistēma aktīvi pretojas svara zaudēšanai — par to, kā tieši, ir atsevišķs raksts. Kamēr cilvēks svara dēļ vaino sevi, viņš neārstējas. Tieši tāpat neviens negaida, ka paaugstinātu asinsspiedienu var pārvarēt ar raksturu.
Ko rāda BMI — un ko tas nerāda
BMI aprēķina: svaru kilogramos dala ar augumu metros kvadrātā. Klasiskās robežas:

BMI ir ērts skrīningam, bet tas mēra svaru, ne taukus. Muskuļots cilvēks var „iekrist” aptaukošanās kategorijā bez liekiem taukiem, un otrādi — cilvēkam ar normālu BMI var būt bīstami daudz tauku ap iekšējiem orgāniem.
Tāpēc svarīgs ir arī vidukļa apkārtmērs — tas aptuveni rāda vēdera tauku daudzumu, kas vielmaiņu bojā visvairāk. Eiropas izcelsmes cilvēkiem risks ir palielināts no 94 cm vīriešiem un 80 cm sievietēm un izteikti palielināts no 102 cm un 88 cm.
Jaunie kritēriji: preklīniska un klīniska aptaukošanās
2025. gadā 58 ekspertu komisija žurnālā The Lancet Diabetes & Endocrinology piedāvāja pieeju, kas beidzot atbild uz jautājumu „kad tā ir slimība”[1]. Būtība ir vienkārša:
Preklīniska aptaukošanās — tauku ir par daudz, bet orgāni strādā normāli. Tas vēl nav slimības stāvoklis, bet paaugstināts risks saslimt ar cukura diabētu, sirds un asinsvadu slimībām un vairākiem audzējiem.
Klīniska aptaukošanās — liekie tauki jau traucē orgānu darbību vai ikdienas dzīvi. Tā ir hroniska slimība, kas jāārstē, negaidot komplikācijas.
Komisija arī iesaka BMI izmantot tikai kā sākotnējo skrīningu: diagnozei papildus vajadzīgs vai nu tiešs tauku mērījums, vai vismaz viens antropometrisks rādītājs — vidukļa apkārtmērs, vidukļa–gurnu vai vidukļa–auguma attiecība. Vienīgais izņēmums: ja BMI pārsniedz 40, lieko tauku klātbūtne tiek pieņemta bez papildu mērījumiem[1].
Ko liekie tauki bojā
Klīniskas aptaukošanās izpausmes, kuras redzu praksē visbiežāk:
2. tipa cukura diabēts un prediabēts — visciešākā saistība no visām.
Paaugstināts asinsspiediens un sirds slimības.
Miega apnoja — krākšana ar elpošanas pauzēm, nogurums no rīta, miegainība dienā. Ļoti bieži paliek neatpazīta.
Aknu aptaukošanās — klusa, bet gadu gaitā var novest pie aknu bojājuma.
Locītavu slimības — ceļgali un gūžas nes katru lieko kilogramu.
Atviļņa slimība (GERS) — par to manā vietnē ir atsevišķa lapa.
Auglības traucējumi abiem dzimumiem un lielāki riski grūtniecībā.
„Man analīzes labas” — vai tas nomierina
Daļai cilvēku ar aptaukošanos analīzes gadiem ir kārtībā. Šo stāvokli mēdz saukt par „metaboliski veselu aptaukošanos”, bet nosaukums maldina: pēc Lancet klasifikācijas tā biežāk ir preklīniska aptaukošanās — riska stāvoklis, nevis drošs stāvoklis (ar nosacījumu, ka svars vēl nav ierobežojis orgānu darbību un ikdienas aktivitāti)[1]. Daļa šo cilvēku laika gaitā pāriet klīniskajā grupā. Labas analīzes šodien nozīmē, ka ir laiks rīkoties bez steigas, nevis ka rīkoties nevajag.
Ko es iesaku pārbaudīt
Ja BMI pārsniedz 30 vai viduklis pārsniedz minētās robežas, saprātīgais minimums ir:
glikoze tukšā dūšā un HbA1c — cukura diabēta un prediabēta meklēšanai;
lipīdi (holesterīns un frakcijas);
aknu rādītāji un aknu ultrasonogrāfija, ja tie ir izmainīti (normālas analīzes taukaino aknu slimību neizslēdz);
asinsspiediena mērījumi vairākas dienas pēc kārtas;
TSH — retos gadījumos svars aug vairogdziedzera slimību dēļ;
jautājumi par miegu: krākšana, elpošanas pauzes, dienas miegainība.
Papildus jānovērtē orgānu darbība un ierobežojumi ikdienā — tas izšķir, cik aktīvi jārīkojas.
Kad jāsāk ārstēt
Pēc Lancet komisijas loģikas atbilde ir skaidra: klīniska aptaukošanās jāārstē tāpat kā jebkura cita hroniska slimība — laikus un ar pierādītām metodēm, negaidot, kamēr orgānu bojājums kļūst neatgriezenisks[1]. Preklīniskas aptaukošanās gadījumā pamatā ir riska mazināšana un novērošana; turpmākā rīcība ir individuāls lēmums.
Tas, kādas tieši ir ārstēšanas iespējas — no dzīvesveida programmām un zālēm līdz endoskopijai un operācijai —, ir apkopots cikla trešajā rakstā.
Atsevišķi gadījumi, kad nevajag gaidīt plānveida izmeklēšanu: straujš svara pieaugums dažu mēnešu laikā bez skaidra iemesla, svara pieaugums kopā ar izteiktu nespēku, pietūkumu vai elpas trūkumu, kā arī ēšanas uzvedības problēmas — regulāras neapturamas pārēšanās epizodes. Katrā no šīm situācijām ir vērts vērsties pie ārsta tuvākajā laikā.
Galvenais no šī raksta
Robežu starp „lieko svaru” un slimību nosaka nevis pēc izskata un pat ne pēc BMI viena paša, bet pēc tā, vai tauki jau traucē orgāniem strādāt. To var izmērīt un pārbaudīt. Un, ja tā jau ir slimība, tai ir efektīva ārstēšana — visa ceļa karte ir atrodama nākamajos cikla rakstos.

Vai aptaukošanās tiešām ir slimība, ne tikai dzīvesveida sekas?Jā. To apstiprina gan PVO, gan 2025. gada Lancet komisija: svaru regulē bioloģiska sistēma, un klīniska aptaukošanās bojā orgānus tāpat kā citas hroniskas slimības. Dzīvesveids ir daļa no ārstēšanas, ne vainas pierādījums.
Mans BMI ir 32, bet analīzes labas. Vai man kaut kas jādara?Tā, visticamāk, ir preklīniska aptaukošanās — riska stāvoklis. Rīcību izvēlas individuāli, bet pārbaudes un riska mazināšana ir saprātīgas: daļa cilvēku ar laiku pāriet klīniskajā grupā.
Vai BMI der visiem?Nē. Tas pārvērtē taukus muskuļotiem cilvēkiem un nenovērtē vēdera taukus. Tāpēc diagnozei līdzās BMI izmanto vidukļa apkārtmēru vai tiešu tauku mērījumu.

Ожирение — не черта характера
Начнём с главного. Ожирение — хроническая болезнь со своей биологией, а не недостаток силы воли. Вес тела регулируют мозг и гормоны, и эта система активно сопротивляется похудению — о том, как именно, есть отдельная статья. Пока человек винит в весе себя, он не лечится. Точно так же никто не ждёт, что повышенное давление можно победить характером.
Что показывает BMI — и чего он не показывает
Индекс массы тела (BMI) считают так: вес в килограммах делят на квадрат роста в метрах. Классические границы:

BMI удобен для скрининга, но он измеряет вес, а не жир. Мускулистый человек может «попасть» в категорию ожирения без грамма лишнего жира, и наоборот — у человека с нормальным BMI может быть опасно большое количество жира вокруг внутренних органов.
Поэтому важна и окружность талии — она показывает именно жир на животе, который сильнее всего вредит обмену веществ. Для людей европейского происхождения риск повышен при талии от 94 см у мужчин и 80 см у женщин, а значительно повышен — при 102 см и 88 см соответственно.
Новые критерии: преклиническое и клиническое ожирение
В 2025 году комиссия из 58 экспертов в журнале The Lancet Diabetes & Endocrinology предложила подход, который наконец отвечает на вопрос «когда это болезнь»[1]. Суть простая:
Преклиническое ожирение — жира слишком много, но органы работают нормально. Это ещё не болезнь, но повышенный риск диабета, сердечно-сосудистых болезней, ряда опухолей.
Клиническое ожирение — лишний жир уже мешает работе органов или повседневной жизни. Это хроническая болезнь, которую надо лечить, не дожидаясь осложнений.
Комиссия также советует использовать BMI только как первичное сито: для диагноза дополнительно нужно либо прямое измерение жира, либо хотя бы один антропометрический показатель — окружность талии, отношение талии к бёдрам или талии к росту. Единственное исключение: при BMI 40 и выше избыток жира принимается без дополнительных измерений[1].
По чему бьёт лишний жир
Проявления клинического ожирения, которые я вижу в практике чаще всего:
Диабет 2-го типа и предиабет — самая тесная связь из всех.
Повышенное давление и болезни сердца.
Апноэ сна — храп с паузами дыхания, разбитость по утрам, сонливость днём. Очень часто остаётся нераспознанным.
Жировая болезнь печени — тихая, но за годы способна привести к повреждению печени.
Болезни суставов — колени и тазобедренные суставы несут каждый лишний килограмм.
Нарушения фертильности у обоих полов и повышенные риски при беременности.
«У меня анализы хорошие» — успокаивает ли это
У части людей с ожирением анализы годами в порядке. Это состояние называют «метаболически здоровым ожирением», но название обманывает: по классификации Lancet это чаще преклиническое ожирение — состояние риска, а не безопасности (при условии, что вес ещё не ограничил работу органов и повседневную активность)[1]. Часть этих людей со временем переходит в клиническую группу. Хорошие анализы сегодня означают, что есть время действовать без спешки, а не то, что действовать не нужно.
Что я советую проверить
Если BMI выше 30 или талия больше названных границ, разумный минимум такой:
глюкоза натощак и HbA1c — поиск диабета и предиабета;
липиды (холестерин и фракции);
печёночные показатели; при их изменении — УЗИ печени (нормальные анализы жировую болезнь печени не исключают);
измерение давления в течение нескольких дней подряд;
TSH — чтобы исключить нарушение функции щитовидной железы;
вопросы про сон: храп, паузы дыхания, дневная сонливость.
Этого списка достаточно для первичной оценки риска и выбора дальнейшего обследования.
Когда начинать лечить
По логике комиссии Lancet, ответ ясен: клиническое ожирение лечат как любую другую хроническую болезнь — вовремя и доказанными методами, не дожидаясь, пока повреждение органов станет необратимым[1]. При преклиническом ожирении основа — снижение риска и наблюдение; насколько далеко идти, решается индивидуально.

Отдельные случаи, когда не стоит ждать плановой проверки: быстрый набор веса за несколько месяцев без ясной причины, набор веса в сочетании с выраженной слабостью, отёками или одышкой, а также проблемы пищевого поведения — регулярные эпизоды неконтролируемого переедания. В каждой из этих ситуаций стоит обратиться к врачу в ближайшее время.

Частые вопросы
Ожирение правда болезнь, а не следствие образа жизни?Да. Это подтверждают и ВОЗ, и комиссия Lancet 2025 года: вес регулирует биологическая система, и клиническое ожирение повреждает органы так же, как другие хронические болезни. Образ жизни — часть лечения, а не доказательство вины.
BMI 32, но анализы хорошие. Мне что-то делать?Хорошие анализы не исключают клиническое ожирение: нужно оценить работу органов и повседневные ограничения. Если нарушений нет, это преклиническое ожирение.
BMI подходит всем?Нет. Он переоценивает количество жира у мускулистых людей и недооценивает его количество на животе. Поэтому для диагноза наряду с BMI используют окружность талии или прямое измерение жира.
Какая окружность талии опасна?Для людей европейского происхождения риск растёт от 94 см у мужчин и 80 см у женщин; при 102 см и 88 см он значительно повышен. Измерять стоя, на выдохе, посередине между рёберной дугой и гребнем подвздошной кости.

People usually answer this question for themselves — and usually get it wrong in both directions. Some spend years treating "laziness" when they have a chronic disease. Others worry about kilograms that pose no threat to their health. This article is about where to draw the line.
Obesity is not a character trait
Let us start with the main point. Obesity is a chronic disease with its own biology, not a lack of willpower. Body weight is regulated by the brain and hormones, and that system actively resists weight loss — a separate article explains how. As long as people blame themselves for their weight, they do not get treated. In just the same way, nobody expects high blood pressure to be overcome by strength of character.
What BMI shows — and what it does not
Body mass index (BMI) is weight in kilograms divided by height in metres squared. The classic bands:

BMI is convenient for screening, but it measures weight, not fat. A muscular person can land in the obesity category despite little excess fat, and the reverse is also true — someone with a normal BMI can carry a dangerous amount of fat around the internal organs.
That is why waist circumference matters too — it reflects abdominal fat, the kind that damages metabolism most. For people of European descent, risk is increased at ≥94 cm in men and ≥80 cm in women, and markedly increased at ≥102 cm and ≥88 cm.
The new criteria: preclinical and clinical obesity
In 2025 a commission of 58 experts, writing in The Lancet Diabetes & Endocrinology, proposed an approach that finally answers the question "when is it a disease?"[1]. The essence is simple:
Preclinical obesity — there is too much fat, but the organs work normally. Not yet an illness, but an increased risk of diabetes, cardiovascular disease and several cancers.
Clinical obesity — excess fat is already interfering with organ function or daily life. That is a chronic illness to be treated without waiting for complications.
The commission also advises using BMI only as an initial screen: the diagnosis additionally requires either a direct measurement of body fat or at least one anthropometric measure — waist circumference, waist-to-hip or waist-to-height ratio. The one exception: above a BMI of 40, excess fat is assumed without further measurement[1].
Where excess fat strikes
The manifestations of clinical obesity I see most often in practice:
Type 2 diabetes and prediabetes — the tightest link of all.
High blood pressure and heart disease.
Sleep apnoea — snoring with pauses in breathing, waking unrefreshed, daytime sleepiness. Very often unrecognised.
Fatty liver disease — silent, but over years it can lead to liver damage.
Joint disease — knees and hips carry every extra kilogram.

Fertility problems in both sexes and higher risks in pregnancy.
"My blood tests are fine" — is that reassuring?
In some people with obesity the blood tests stay normal for years. This is often called "metabolically healthy obesity", but the name misleads: in the Lancet classification it is more often preclinical obesity — a state of risk, not of safety (provided the weight has not yet limited organ function or daily activity)[1]. Some of these people move into the clinical group over time. Good results today mean there is time to act without hurry, not that no action is needed.
What I advise checking
If BMI is above 30, or the waist exceeds the thresholds above, the sensible minimum is:
fasting glucose and HbA1c — screening for diabetes and prediabetes;
lipids (cholesterol and its fractions);
liver tests, and a liver ultrasound if they are abnormal (normal tests do not rule out fatty liver disease);
blood pressure measured over several consecutive days;
TSH — occasionally weight rises because of thyroid disease;
questions about sleep: snoring, pauses in breathing, daytime sleepiness.
That assessment must also cover organ function and limitations in daily life.
When treatment should start
Under the Lancet Commission's framework, the answer is clear: clinical obesity is treated like any other chronic disease — promptly and with proven methods, without waiting for organ damage to become irreversible[1]. In preclinical obesity the basis is risk reduction and monitoring; how far to go is an individual decision.

Situations that should not wait for a routine check-up: rapid weight gain over a few months without a clear reason, weight gain together with marked weakness, swelling or breathlessness, and eating-behaviour problems — regular episodes of uncontrollable overeating. The line between "excess weight" and disease is determined not by appearance, and not even by BMI alone, but by whether fat is already interfering with how the organs work. Is obesity really a disease and not just a lifestyle consequence?Yes. Both the WHO and the 2025 Lancet Commission confirm it: weight is regulated by a biological system, and clinical obesity damages organs the way other chronic diseases do. Lifestyle is part of the treatment, not proof of fault.
My BMI is 32 but my tests are fine. Should I do anything?It is preclinical obesity only if organ function and daily activities are preserved. Treatment is not obligatory, but checks and risk reduction are sensible: some people move into the clinical group over time.
Does BMI work for everyone?No. It overestimates fat in muscular people and underestimates abdominal fat. That is why, alongside BMI, waist circumference or a direct fat measurement is used for the diagnosis.
