Starp injekcijām katru nedēļu un operāciju ar anatomijas maiņu ir divas metodes, par kurām jautā bieži un zina maz: kuņģa balons un endoskopiskā gastroplastika. Abas veic caur muti, bez neviena griezuma. Tālāk — kā tās atšķiras un kuram kura der.
Kuņģa balons: kā tas darbojas
Endoskopijas laikā kuņģī ievieto silikona balonu un piepilda ar šķidrumu — atkarībā no modeļa parasti 400–700 ml. Balons aizņem vietu, palēnina kuņģa iztukšošanos, un sāta sajūta iestājas ātrāk. Procedūra ilgst apmēram 20–30 minūtes; mājās var doties tajā pašā dienā.
Skaitļi no ASGE metaanalīzes ar 1683 pacientiem: vidēji 13,2% no kopējās masas pēc 6 mēnešiem un 11,3% pēc 12 mēnešiem[13]. Tas ir mazāk nekā jaunajām zālēm un ievērojami mazāk nekā operācijai — bet bez injekcijām un bez anatomijas maiņas.
Balona galvenā īpatnība: tam ir noteikts termiņš. Balonu izņem pēc 6–12 mēnešiem, un tālāk viss ir atkarīgs no tā, kādi paradumi šajā laikā izveidojušies. Bez turpinājuma plāna svars pēc izņemšanas bieži atgriežas — tas jāzina pirms ievietošanas, ne pēc izņemšanas.
Pirmās dienas pēc ievietošanas parasti ir grūtākās: slikta dūša un spazmas, kamēr kuņģis pierod. Retos gadījumos balonu nepanesības dēļ izņem priekšlaicīgi.
ESG: šuves bez griezumiem
Endoskopiskā gastroplastika (endoscopic sleeve gastroplasty) ir jaunāka metode: caur muti ar šujošo endoskopu kuņģi no iekšpuses sašuj garenā, šaurākā formā. Nekas netiek izgriezts, un vēders paliek bez rētām; procedūra notiek narkozē un aizņem 1–2 stundas.
Skaitļi no randomizētā MERIT pētījuma: vidēji 13,6% no kopējās masas 52 nedēļās pret 0,8% kontroles grupā, un pēc diviem gadiem 68% dalībnieku noturēja būtisku zudumu. Nopietnas blakusparādības — 2% dalībnieku, bez nāves gadījumiem un bez ķirurģiskas iejaukšanās nepieciešamības[8].
Atšķirībā no balona ESG ir ilgstošas darbības metode: šuves paliek, un rezultāts pētījumos turas vismaz divus gadus, lai gan šuvju noturība nav garantēta. Datu par gadu desmitiem — kādi ir ķirurģijai — šai metodei vēl nav, un tas ir tās galvenais ierobežojums.

Padomāsim par vietu, ko šīs metodes patiesībā ieņem šodien, kad zāles dod 15–21%:
ja injekcijas nepanes vai nevēlas — endoskopija ir vidēji iedarbīga alternatīva;
ja vajadzīgs vienreizējs solis, ne mūža maksājums — ESG izmaksas vairāku gadu laikā var būt zemākas par zāļu izmaksām;
kā tilts pirms lielas operācijas — atsevišķiem pacientiem ar ļoti lielu svaru balonu lieto svara samazināšanai pirms operācijas — pēc komandas lēmuma[13];
ja BMI virs 40 vai smags diabēts — endoskopiskās metodes vidēji nesasniedz vajadzīgo (lēmums vienmēr ir individuāls), un tur nopietni jārunā par operāciju.
Un viens ierobežojums, kas attiecas uz abām: tāpat kā zāles un operācija, tās darbojas tikai kopā ar uztura un paradumu programmu. MERIT pētījumā abas grupas saņēma dzīvesveida atbalstu — atšķirību radīja metode plus programma, ne metode viena pati[8].
Blakusparādības un riski
Balonam: slikta dūša un vemšana pirmajās dienās ļoti bieži; retāk nepanesība ar priekšlaicīgu izņemšanu; reti — čūla vai balona iztukšošanās, tāpēc dažu modeļu balonos šķidrumam pievieno krāsvielu, kas iekrāso urīnu, ja balons plīst.
ESG: sāpes un slikta dūša pirmajās dienās; nopietnas komplikācijas MERIT pētījumā 2%[8] — asiņošana vai šķidruma uzkrāšanās, kas parasti risināma bez operācijas.
Pēc jebkuras no šīm procedūrām nekavējoties jāsazinās ar ārstu, ja ir stipras, nepārejošas vēdersāpes, atkārtota vemšana vai nespēja uzņemt šķidrumu, drudzis, melni izkārnījumi vai zaļganzils urīns (balona krāsvielas pazīme). Tās var būt komplikāciju pazīmes, kuras jānovērtē tajā pašā dienā.
Galvenais no šī raksta
Balons ir īslaicīgs palīgs ar skaidru beigu datumu, ESG — paliekošāks vidusceļš bez griezumiem. Abi dod 11–14% svara zudumu un abi prasa atbalsta programmu. Tie neaizstāj ne zāles, ne operāciju — tie ieņem vietu starp tām — un tieši tur tie arī pieder.

Желудочный баллон и эндоскопическая гастропластика (ESG): средний путь между препаратами и операцией
Между еженедельными инъекциями и операцией с изменением анатомии есть два метода, о которых спрашивают часто, а знают мало: желудочный баллон и эндоскопическая гастропластика. Оба выполняются через рот, без единого разреза. Дальше — чем они отличаются и кому какой подходит.
Желудочный баллон: как это работает
Во время эндоскопии в желудок помещают силиконовый баллон и заполняют его жидкостью — в зависимости от модели обычно 400–700 мл. Баллон занимает место, замедляет опорожнение желудка, и сытость наступает раньше. Процедура занимает примерно 20–30 минут, домой можно в тот же день.
Цифры из метаанализа ASGE с 1683 пациентами: в среднем 13,2% от общей массы через 6 месяцев и 11,3% через 12 месяцев[13]. Это меньше, чем у новых препаратов, и заметно меньше, чем у операции, — но без инъекций и без изменения анатомии.
Главная особенность баллона: он временный. Баллон извлекают через 6–12 месяцев, и дальше всё зависит от того, какие привычки сложились за это время. Без плана продолжения вес после извлечения часто возвращается — знать это надо до установки, а не после извлечения.
Первые дни после установки обычно самые тяжёлые: тошнота и спазмы, пока желудок привыкает. В редких случаях баллон из-за непереносимости извлекают досрочно.
ESG: швы без разрезов
Эндоскопическая гастропластика (endoscopic sleeve gastroplasty) — более новый метод: через рот с помощью эндоскопа на желудок изнутри накладывают швы, придавая ему более узкую форму. Ничего не вырезается, живот остаётся без шрамов; процедура проводится под наркозом и занимает 1–2 часа.
Цифры из рандомизированного исследования MERIT: в среднем 13,6% от общей массы за 52 недели против 0,8% в контрольной группе, а через два года 68% участников удерживали существенную потерю веса. Серьёзные осложнения возникли в 2% случаев, без смертей и без необходимости в хирургическом вмешательстве[8].
В отличие от баллона, ESG — метод длительного действия: швы остаются, и результат в исследованиях держится минимум два года, хотя долговечность швов не гарантирована. Данных за десятилетия, как у хирургии, у метода пока нет, и это его главное ограничение.

Где эти методы на общей карте
Посмотрим на место, которое эти методы занимают сегодня, когда препараты дают 15–21%:
если инъекции не переносятся или не подходят — эндоскопия остаётся альтернативой средней силы;
если нужен разовый шаг, а не пожизненный платёж — в долгосрочной перспективе ESG может быть экономически выгоднее препаратов;
как мост перед большой операцией — у отдельных пациентов с очень большим весом баллоном снижают вес по решению команды[13];
если BMI выше 40 или тяжёлый диабет — эндоскопические методы в среднем не дают нужного результата (решение всегда индивидуально), и там всерьёз обсуждается операция.
И одно ограничение, общее для обоих: как препараты и операция, они работают только вместе с программой питания и привычек. В MERIT обе группы получали поддержку в изменении образа жизни — разницу дал метод плюс программа, а не метод сам по себе[8].
Побочные эффекты и риски
У баллона: тошнота и рвота в первые дни очень часто; реже непереносимость с досрочным извлечением; редко — язва или сдувание баллона, поэтому в некоторых моделях в жидкость добавляют краситель, окрашивающий мочу, если баллон лопнул.
У ESG: боль и тошнота в первые дни; серьёзные осложнения в MERIT — 2%[8]: кровотечение или скопление жидкости, обычно излечимые без операции.
После любой из этих процедур нужно немедленно связаться с врачом, если есть сильная непроходящая боль в животе, многократная рвота или невозможность пить, температура, чёрный стул или сине-зелёная моча (краситель из лопнувшего баллона). Это могут быть признаки осложнений, которые надо оценить в тот же день.

Баллон — временный помощник с ясной датой окончания, ESG — более долговременный средний путь без разрезов. Оба дают 11–14% потери веса, и оба требуют сопровождающей программы. Они не заменяют ни препараты, ни операцию — они заполняют пространство между ними, и именно там их место.
Частые вопросы
Как долго баллон остаётся в желудке?В зависимости от модели 6–12 месяцев, затем его извлекают эндоскопически. Дольше оставлять нельзя — растёт риск разрыва и осложнений.
Возвращается ли вес после извлечения баллона?Без плана продолжения — часто. Поэтому время баллона нужно использовать для смены привычек и ещё до установки договориться, что будет после извлечения: программа, препараты или другой шаг.
Чем ESG отличается от рукавной операции?При рукавной операции часть желудка необратимо удаляют; при ESG желудок только ушивают изнутри. ESG щадящая и без шрамов, но потеря веса меньше, и долгосрочных данных меньше.
Компенсирует ли государство эти методы?Для лечения ожирения — нет, это платные процедуры. Точная стоимость зависит от клиники; обсуждаем на консультации.

The gastric balloon and endoscopic sleeve gastroplasty (ESG): the middle road between drugs and surgery
Two methods that people often ask about and rarely understand sit between weekly injections and anatomy-changing surgery: the gastric balloon and endoscopic sleeve gastroplasty. Both are done through the mouth, without a single incision. Here is how they differ and who each one suits.
The gastric balloon: how it works
During an endoscopy a silicone balloon is placed in the stomach and filled with fluid — depending on the model, usually 400–700 ml. The balloon takes up space, slows stomach emptying, and makes you feel full sooner. The procedure takes roughly 20–30 minutes; you go home the same day.
The numbers from an ASGE meta-analysis of 1,683 patients: a mean loss of 13.2% of total body weight at 6 months and 11.3% at 12 months[13]. That is less than the new drugs and considerably less than surgery — but with no injections and no change to anatomy.
The balloon's defining feature: it is temporary. It is removed after 6–12 months, and maintaining weight loss afterwards requires a continuation plan. Without one, weight often returns after removal — something to know before placement, not after.
The first days after placement are usually the hardest: nausea and cramps while the stomach adjusts. Occasionally the balloon has to be removed early because of intolerance.
ESG: stitches without incisions
Endoscopic sleeve gastroplasty is the younger method: through the mouth, with a suturing endoscope, the stomach is stitched from the inside into a longer, narrower shape. Nothing is cut out, and the procedure leaves no abdominal scars; it is done under general anaesthesia and takes 1–2 hours.
The numbers from the randomised MERIT trial: a mean loss of 13.6% of total body weight at 52 weeks versus 0.8% in the control group, and at two years, 68% of participants maintained substantial weight loss. Serious adverse events occurred in 2%, with no deaths and no need for surgical rescue[8].
Unlike the balloon, ESG is a longer-lasting method: the sutures stay, and in the trials the result holds for at least two years, though long-term durability is not guaranteed. What the method lacks is decades of follow-up — the kind surgery has — and that is its main limitation.

Where these methods fit on the map
Consider the place these methods hold today, when the drugs deliver 15–21%:
if injections are not tolerated or not wanted — endoscopy is a mid-strength alternative;
if a one-off procedure is preferred to an ongoing expense — over years, ESG can cost less than drugs;
as a bridge before major surgery — in selected patients at very high weights the balloon is used to reduce weight before surgery — a team decision[13];
at a BMI over 40 or with severe diabetes — both methods on average fall short, and that is where surgery deserves a serious conversation.
And one limitation shared by both: like drugs and surgery, they only work together with a food-and-habits programme. In MERIT both groups received lifestyle support — the difference came from the method plus the programme, not the method alone[8].
Side effects and risks
Balloon: nausea and vomiting are common in the first days; occasionally intolerance leading to early removal; rarely an ulcer or balloon deflation — in dye-filled models, leakage colours the urine.
ESG: pain and nausea in the first days; serious complications in MERIT — 2%[8]: bleeding or fluid around the stomach, usually managed without surgery.
After either procedure, contact a doctor immediately for severe, unrelenting abdominal pain, repeated vomiting or inability to keep fluids down, fever, black stools, or blue-green urine (a sign that a dye-filled balloon is leaking). These can be signs of complications that need assessment the same day.

The balloon is a temporary helper with a clear end date; ESG is a more lasting middle road without incisions. Both deliver 11–14% weight loss, and both need a programme around them. They replace neither drugs nor surgery — they fill the space in between, and that is exactly where they belong.

How long does the balloon stay in the stomach?Depending on the model, 6–12 months; it is then removed endoscopically. It must not stay longer — the risk of rupture and complications grows.
Does weight return after the balloon is removed?Without a continuation plan — often. So the balloon period should be used to change habits, and what happens after removal — a programme, medication or another step — should be agreed before placement.
How does ESG differ from sleeve surgery?In a sleeve, part of the stomach is removed surgically and irreversibly; in ESG the stomach is only stitched from the inside. ESG is less invasive and leaves no abdominal scars, but the weight loss is smaller and the long-term data are limited.
