Gandrīz katrs, kas pie manis vēršas aptaukošanās dēļ, jau ir novājējis — un ne vienu reizi vien. Pieci, desmit, dažreiz divdesmit kilogrami, kas pēc gada bija atgriezušies. Un gandrīz katrs no tā izdara vienu un to pašu secinājumu: „es neizturēju”. Šis raksts ir par to, kāpēc šis secinājums ir nepareizs.
Ķermenim ir savs termostats
Svaru neregulē griba. To regulē smadzenes — hipotalāms, kas saņem hormonu signālus no taukaudiem, kuņģa un zarnām un uztur svaru tam ierastajā līmenī, apmēram kā termostats uztur temperatūru. Kad cilvēks novājē, sistēma to uztver nevis kā panākumu, bet kā apdraudējumu — un ieslēdz aizsardzību divos virzienos vienlaikus: pastiprina izsalkumu un samazina enerģijas patēriņu.
Kas notiek pēc svara zaudēšanas — izmērīts
Par šo mehānismu ir precīzi mērījumi. Klasiskajā Melburnas pētījumā cilvēki desmit nedēļās zaudēja vidēji 13,5 kg, un pētnieki gadu sekoja viņu apetīti regulējošiem hormoniem[2]:
grelīns — hormons, kas veicina izsalkumu, — palika paaugstināts arī gadu pēc novājēšanas;
leptīns un vairāki sāta hormoni palika pazemināti;
subjektīvais izsalkums palika lielāks nekā pirms diētas — arī pēc gada.
Citiem vārdiem: gadu pēc novājēšanas ķermenis joprojām „prasa” zaudēto atpakaļ. Tas nav ieraduma vai rakstura jautājums — tie ir izmērāmi hormonu līmeņi[2].
Otrs slavens mērījums ir no ASV realitātes šova „The Biggest Loser” dalībniekiem, kuri konkursa laikā zaudēja vidēji 58 kg. Pēc sešiem gadiem lielākā daļa svara bija atgriezusies, bet galvenais atklājums bija cits: viņu miera vielmaiņa dedzināja aptuveni par 500 kilokalorijām dienā mazāk, nekā būtu gaidāms pēc viņu ķermeņa sastāva[3]. Vielmaiņas palēninājums nepazuda ar laiku — tas turpinājās gadiem.
„Vienkārši ēd mazāk un kusties vairāk”
Šī frāze nav gluži nepareiza — tā ir nepilnīga. Enerģijas deficīts tiešām ir vienīgais veids, kā zaudēt taukus. Bet frāze klusē par to, ka pēc novājēšanas organisms deficītam pretojas ar pastiprinātu izsalkumu un mazāku patēriņu — un šī pretestība nerimst mēnešiem un gadiem. Noturēt svaru ar gribasspēku pret savu hormonu sistēmu ir apmēram tas pats, kas ar gribasspēku ilgstoši neelpot.
Ko rāda lielākais dzīvesveida pētījums
Vērienīgākais eksperiments šajā jomā ir Look AHEAD: vairāk nekā 5000 cilvēku ar 2. tipa cukura diabētu un lieko svaru astoņus gadus saņēma intensīvu programmu — uztura konsultācijas, fiziskās aktivitātes plānu, regulāras vizītes[4]. Rezultāts pēc astoņiem gadiem:

Puse programmas dalībnieku saglabāja vismaz 5% svara zudumu — un tas ir nozīmīgi veselībai. Bet cilvēkam ar 120 kg tas nozīmē aptuveni 6 kg pēc astoņu gadu sistemātiska darba profesionāļu uzraudzībā. Tā nav programmas kritika — tā ir atbilde uz jautājumu, ko jau attīstījušās aptaukošanās gadījumā var panākt tikai ar dzīvesveida pārmaiņām.
Jo-jo efekts nav jūsu iedoma
Saliekot kopā: pēc katras novājēšanas izsalkums aug, patēriņš krīt, un hormonu spiediens turpinās gadiem. Tāpēc tipiskā trajektorija — zaudēt, noturēt dažus mēnešus, atgūt — nav personisku sakāvju virkne. Tā ir hroniskas slimības dabiskā gaita bez uzturošas ārstēšanas. Nevienam nepārmet vāju gribu, ja pēc zāļu pārtraukšanas atkal paaugstinās spiediens.
Kas no tā izriet praktiski
Svara noturēšana prasa pastāvīgu atbalstu — režīmu, kas turpinās, nevis „kursu”, kas beidzas. Tas attiecas uz visām metodēm, arī zālēm un operāciju.
Olbaltumvielas, regulāras maltītes un kustība palīdz galvenokārt noturēšanas fāzē — tās mazina muskuļu zudumu un nedaudz remdē izsalkuma sajūtu.
Ātrās un ekstrēmās diētas ir visnedrošākais ceļš: organisma atbilde ir saistīta galvenokārt ar pašu svara zudumu, un stingras nekontrolētas diētas rada papildu riskus.
Ja svars atgriežas atkal un atkal, risinājums nav vēl viena tāda pati diēta, bet ārstēšanas pakāpes maiņa — par iespējām stāsta nākamais raksts.
Kad svaram ir cits cēlonis
Retāk svara pieaugumam ir atsevišķs medicīnisks cēlonis, un to ir vērts izslēgt: vairogdziedzera mazspēja, dažas zāles (daļa antidepresantu, kortikosteroīdi, daži diabēta un epilepsijas medikamenti), kā arī ēšanas traucējumi — regulāras nekontrolējamas pārēšanās epizodes prasa nevis diētu, bet speciālista palīdzību.
Atsevišķs brīdinājums par „brīnumdiētām” un badošanos. Ilgstoša badošanās un diētas ar mazāk nekā 800 kilokalorijām dienā bez ārsta uzraudzības ir bīstamas — no žultsakmeņu veidošanās līdz sirds ritma traucējumiem. Straujš svara zudums ir viens no spēcīgākajiem žultsakmeņu riska faktoriem; par to sīkāk rakstā par žultsakmeņiem.
Galvenais no šī raksta
Svara atgriešanās pēc diētas ir bioloģijas, ne rakstura jautājums: bada hormoni un palēnināta vielmaiņa strādā pret jums mēnešiem un gadiem pēc novājēšanas. Tāpēc nopietna aptaukošanās prasa ārstēšanu, kas šo pretestību ņem vērā — un tāda ārstēšana šodien pastāv.

Vai vielmaiņu var „salauzt” ar diētām?„Salauzt” — nē, bet vielmaiņa tām pielāgojas: patēriņš miera stāvoklī kļūst mazāks, nekā gaidāms pēc ķermeņa sastāva, un šī adaptācija var turēties gadiem. Tā ir normāla fizioloģija, ne bojājums.
Kāpēc izsalkums pēc diētas nepāriet?Bada hormons grelīns paliek paaugstināts un sāta hormoni pazemināti vismaz gadu pēc novājēšanas. Izsalkums pēc diētas ir hormonāls, ne iedomāts.
Vai sports palīdz noturēt svaru?Jā, bet galvenokārt noturēšanas fāzē un veselībai kopumā. Novājēšanai kustība viena pati palīdz maz — to kompensē lielāka apetīte.

Почти каждый, кто приходит ко мне с ожирением, уже худел — и не один раз. Пять, десять, иногда двадцать килограммов, которые через год вернулись. И почти каждый делает из этого один и тот же вывод: «Я не выдержал». Эта статья о том, почему этот вывод неверен.
У тела есть свой термостат
Вес регулирует не воля, а гипоталамус. Он получает гормональные сигналы от жировой ткани, желудка и кишечника и удерживает вес на привычном уровне, примерно как термостат держит температуру. Когда человек худеет, система воспринимает это не как успех, а как угрозу — и включает защиту сразу в двух направлениях: усиливает голод и снижает расход энергии.
Что происходит после похудения — измерено
Этот механизм точно измерен. В классическом мельбурнском исследовании люди за десять недель потеряли в среднем 13,5 кг, и исследователи год следили за их гормонами голода[2]:
грелин — гормон, который усиливает голод, — оставался повышенным и через год после похудения;
лептин и несколько гормонов насыщения оставались сниженными;
субъективное чувство голода оставалось сильнее, чем до диеты, — тоже через год.
Другими словами: через год после похудения тело всё ещё «требует» вернуть потерянный вес. Это не вопрос привычки или характера — это измеримые уровни гормонов[2].
Второе знаменитое исследование посвящено участникам американского шоу «The Biggest Loser», потерявшим за время конкурса в среднем 58 кг. Через шесть лет большая часть веса вернулась, но главная находка была другой: их обмен веществ в покое сжигал примерно на 500 килокалорий в день меньше, чем ожидалось бы при их составе тела[3]. Замедление обмена не исчезло со временем — оно держалось годами.
«Просто меньше ешь и больше двигайся»
Эта фраза не то чтобы неверна — она неполна. Дефицит энергии действительно единственный способ терять жир. Но фраза умалчивает о том, что после похудения организм сопротивляется дефициту усиленным голодом и сниженным расходом — и это сопротивление не стихает месяцами и годами. Держать вес силой воли против собственной гормональной системы — примерно то же, что силой воли подолгу не дышать.
Что показало самое крупное исследование образа жизни
Самый масштабный эксперимент в этой области — Look AHEAD: более 5000 человек с диабетом 2-го типа и лишним весом в течение восьми лет получали интенсивную программу — консультации по питанию, план физической активности, регулярные визиты[4]. Результат через восемь лет:

Половина участников программы сохранила снижение веса минимум на 5% — и для здоровья это значимо. Но для человека весом 120 кг это около 6 кг после восьми лет системной работы под контролем профессионалов. Это не критика программы — это ответ на вопрос, чего можно достичь одним лишь изменением образа жизни при уже развившемся ожирении.
Эффект йо-йо — не ваша вина
Складываем вместе: после каждого похудения голод растёт, расход падает, и гормональное сопротивление продолжается годами. Поэтому типичная траектория — сбросить, удержать несколько месяцев, набрать — это не серия личных поражений. Это естественное течение хронической болезни без поддерживающего лечения. Никто не называет слабохарактерным человека, у которого снова поднялось давление после отмены лекарств.
Что из этого следует практически
Удержание веса требует постоянной поддержки — режима, который продолжается, а не «курса», который заканчивается. Это касается всех методов, включая препараты и операцию.
Белок, регулярные приёмы пищи и движение помогают главным образом в фазе удержания — они уменьшают потерю мышц и немного гасят чувство голода.
Быстрые и экстремальные диеты — самый ненадёжный путь: ответ организма связан прежде всего с самой потерей веса, а жёсткие бесконтрольные диеты добавляют свои риски.

Когда за весом стоит что-то другое
Реже набор веса бывает вызван отдельной медицинской причиной, и её стоит исключить: сниженная функция щитовидной железы, некоторые лекарства (часть антидепрессантов, кортикостероиды, отдельные препараты от диабета и эпилепсии), а также расстройства пищевого поведения. Регулярные эпизоды неконтролируемого переедания требуют не диеты, а помощи специалиста.
Отдельное предупреждение о «чудо-диетах» и голодании. Длительное голодание и диеты ниже 800 килокалорий в день без контроля врача опасны — от образования желчных камней до нарушений сердечного ритма. Возврат веса после диеты — вопрос биологии, а не характера: гормоны голода и замедленный обмен работают против вас месяцами и годами после похудения. Поэтому серьёзное ожирение требует лечения, которое учитывает это сопротивление, — и такое лечение сегодня существует.
Частые вопросы
Можно ли «сломать» обмен веществ диетами?«Сломать» — нет: после похудения расход в покое становится меньше, чем можно было бы ожидать с учётом состава тела, и эта адаптация может держаться годами. Это нормальная физиология, а не поломка.
Почему голод после диеты не уходит?Гормон голода грелин остаётся повышенным, а гормоны насыщения остаются сниженными в течение как минимум года после похудения. Голод после диеты — гормональный, а не выдуманный.
Помогает ли спорт удержать вес?Да, но главным образом в фазе удержания и для здоровья в целом. Для похудения одного движения мало: усиление аппетита ограничивает его эффект.

Almost everyone who comes to me with obesity has lost weight before — more than once. Five, ten, sometimes twenty kilograms that came back within a year. And almost everyone draws the same conclusion: "I couldn't keep it off." This article is about why that conclusion is wrong.
The body has its own thermostat
Weight is not regulated by willpower. It is regulated by the brain — the hypothalamus — which receives hormonal signals from fat tissue, the stomach and the gut and keeps weight at its accustomed level, much as a thermostat holds a temperature. When a person loses weight, the system reads it not as success but as a threat — and switches on defences in two directions at once: it turns hunger up and energy expenditure down.
What happens after weight loss — measured
This mechanism has been measured precisely. In a classic Melbourne study, people lost a mean of 13.5 kg over ten weeks, and the researchers then tracked their hunger hormones for a year[2]:
ghrelin — the hormone that drives hunger — remained elevated a full year after the weight loss;
leptin and several satiety hormones remained lowered;
subjective hunger stayed stronger than before the diet — a year on as well.
In other words: a year after losing weight, the body is still "demanding" the loss back. This is not a matter of habit or character — these are measurable hormone levels[2].
The second famous measurement comes from contestants on the American show "The Biggest Loser", who lost a mean of 58 kg during the competition. Six years later most of the weight had returned, but the key finding was different: their resting metabolism was burning roughly 500 kilocalories a day less than would be expected for their body composition[3]. The metabolic slowdown did not fade with time — it persisted for years.
"Just eat less and move more"
The phrase is not exactly wrong — it is incomplete. An energy deficit really is the only way to lose fat. But the phrase is silent about what follows: after weight loss the body fights the deficit with heightened hunger and lower expenditure — and that resistance does not subside for months and years. Holding weight off by willpower against your own hormonal system is roughly like holding your breath by willpower, indefinitely.
What the largest lifestyle trial showed
The most ambitious experiment in this field is Look AHEAD: more than 5,000 people with type 2 diabetes and excess weight received an intensive programme for eight years — dietary counselling, an activity plan, regular visits[4]. The result at eight years:

Half of the programme participants maintained at least a 5% loss — and that is meaningful for health. But for someone weighing 120 kg it means about 6 kg after eight years of systematic work under professional supervision. That is not a criticism of the programme — it is the answer to what lifestyle alone can do once obesity is established.
The yo-yo effect is not your fault
Putting it together: after every weight loss, hunger rises, expenditure falls, and the hormonal pressure continues for years. So the typical trajectory — lose, hold for a few months, regain — is not a series of personal defeats. It is the natural course of a chronic disease without maintenance treatment. Nobody calls a person weak-willed when their blood pressure climbs again after stopping the tablets.
What follows in practice
Keeping weight off needs ongoing support — a regimen that continues, not a "course" that ends. That applies to every method, including medication and surgery.
Protein, regular meals and movement help mainly in the maintenance phase — they limit muscle loss and take the edge off hunger.
Crash and extreme diets are the least reliable route: the body responds above all to the weight loss itself, and harsh uncontrolled diets add risks of their own.

When something else is behind the weight
Less often, weight gain has a separate medical cause worth ruling out: an underactive thyroid, certain medicines (some antidepressants, corticosteroids, certain diabetes and epilepsy drugs), and eating disorders — regular episodes of uncontrollable overeating call for a specialist, not a diet.
A separate warning about "miracle diets" and fasting. Prolonged fasting and diets below 800 kilocalories a day without medical supervision are dangerous — with risks ranging from gallstone formation to heart rhythm disturbances. Weight regain after a diet is a matter of biology, not character: hunger hormones and a slowed metabolism work against you for months and years after the loss. That is why severe obesity needs treatment that takes this resistance into account — and such treatment now exists.
Frequently asked questions
Can diets "break" your metabolism?"Break" — no, but weight loss causes it to adapt: resting expenditure becomes lower than expected for your body composition, and the adaptation can persist for years. That is normal physiology, not damage.
Why does hunger not go away after a diet?The hunger hormone ghrelin stays elevated and satiety hormones stay lowered for at least a year after weight loss. Post-diet hunger is hormonal, not imagined.
Does exercise help keep weight off?Yes, but mainly in the maintenance phase and for health overall. For the loss itself, exercise alone achieves little — the appetite system pushes back.
