Hemoroīdi ir visizplatītākā proktoloģiskā problēma un vienlaikus visbiežāk pārprastā. Daļu pacientu ārstē no tā, kas nav jāārstē. Otru daļu — no hemoroīdiem, kaut cēlonis ir cits. Šajā rakstā izskaidroju, kas notiek anālajā kanālā, kā nosaka pakāpi un kā saprast, ka ir laiks doties pie ārsta.
Hemoroīdi nav vēnu varikoze
Hemoroidālie spilventiņi ir normāla anatomiska struktūra. Tie ir katram cilvēkam no dzimšanas — trīs asinsvadu spilventiņi anālā kanāla sieniņā, kas nodrošina tā precīzu noslēgšanos un dod aptuveni piektdaļu no miera spiediena. Bez tiem nebūtu pilnvērtīgas fēču un gāzu saturēšanas.
Slimība sākas tad, kad spilventiņus turošie saistaudi ar gadiem novājinās un spilventiņi sāk slīdēt uz leju. Tajos veidojas asins sastrēgums, tie palielinās, asiņo un izvirzās ārā. Šo mehānismu 1975. gadā aprakstīja Tomsons, un tas ir mūsdienu izpratnes pamatā[2].
No tā izriet praktisks secinājums: hemoroīdi ir simptomi, nevis atrade. Ja spilventiņi ir palielināti, bet netraucē — tie nav jāārstē. Pārdiagnostika šajā jomā ir bieža.
Iekšējie, ārējie un kombinētie
Anālajā kanālā ir robeža — ķemmveida līnija. Tā izšķir visu.
Virs līnijas — iekšējie hemoroīdi. Tos klāj gļotāda, kurā nav sāpju receptoru. Tāpēc tie asiņo un izvirzās, bet nesāp. Un tāpēc tos var apstrādāt ambulatori, bez anestēzijas.
Zem līnijas — ārējie hemoroīdi. Tos klāj āda ar pilnu jutību. Tie izraisa asas sāpes, īpaši trombozes gadījumā.
Kombinētie — abi komponenti reizē. Manā praksē tas ir biežākais variants, un tieši tas nosaka metodes izvēli.
Sāpes nekomplicētiem iekšējiem hemoroīdiem nav raksturīgas. Ja sāp — meklējam trombozi, anālo plaisu vai abscesu, nevis vainojam hemoroīdus.

Klasiskā operācija
Pakāpi nevar noteikt pēc pacienta stāsta vai fotogrāfijas. Vajadzīga anoskopija — īsa izmeklēšana ar caurulīti, kas ilgst minūti un ko veic uzreiz konsultācijā.
Asiņošana no taisnās zarnas nav automātiski hemoroīdi. ASCRS 2024. gada vadlīnijas nosaka: pacientiem ar riska faktoriem — vecums, kolorektāla vēža gadījumi ģimenē, svara zudums, dzelzs deficīta anēmija, izmaiņas vēdera izejā — pirms hemoroīdu ārstēšanas jāveic pilna resnās zarnas izmeklēšana[1]. Tumšas asinis, sajauktas ar fēcēm, ir atsevišķs brīdinājums: hemoroīdiem raksturīgas spilgti sarkanas asinis uz papīra vai uz fēcēm.
Kas vēl slēpjas zem „hemoroīdiem”
Ar to pašu sūdzību pie manis nonāk pacienti, kuriem patiesībā ir anālā plaisa, perianāla fistula, taisnās zarnas prolapss, kondilomas vai ādas bārkstis, ko cilvēki bieži sauc par hemoroīdiem. Ādas krokas jeb bārkstis (skin tags) nav hemoroīdi — tās ir palikušas pēc pārciestas trombozes un pašas neizzūd.
Tāpēc pirmā vizīte vienmēr sākas ar izmeklēšanu, ne ar recepti.
Kad ārstēt, kad ne
Ārstējam simptomus, ne attēlu. Ja mezgli ir, bet neasiņo, neizvirzās un netraucē — nekas nav jādara. Ja ir asiņošana, izvirzīšanās, nieze vai izdalījumi, kas ietekmē ikdienu, — sākam ar mazāko un kāpjam augstāk tikai tad, ja tas nelīdz.
Lielākā daļa pacientu līdz operācijai nemaz nenonāk. I un II pakāpē parasti pietiek ar režīmu un ambulatoru procedūru, kas ilgst dažas minūtes.
Biežākie jautājumi
Vai hemoroīdi var pāriet paši?Akūta epizode var norimt, un ar režīmu un šķiedrvielām simptomi bieži izzūd. Bet novājinātie saistaudi neatjaunojas — tāpēc bez ārstēšanas problēma parasti atgriežas, un ar gadiem pakāpe pieaug.
Vai vienmēr vajag operāciju?Nē. Lielākajai daļai pacientu ar I un II pakāpi pietiek ar režīmu un ambulatoru procedūru. Operācija nepieciešama galvenokārt IV pakāpē un liela ārējā komponenta gadījumā.
Ar ko sākt?Ar konsultāciju un anoskopiju — bez tās pakāpi noteikt nevar. Ja ir riska faktori vai netipiska asiņošana, vispirms izslēdzam resnās zarnas patoloģiju.

Геморрой: почему он возникает и когда нужно лечить
Геморрой — самая частая проктологическая проблема и одновременно та, которую чаще всего понимают неправильно. Одних лечат от того, что лечить не нужно. Других лечат от геморроя, хотя причина другая. В этой статье объясняю, что происходит в анальном канале, как определяют стадию и по каким признакам пора к врачу.
Геморрой — это не варикоз вен
Геморроидальные подушки — нормальная анатомическая структура. Они есть у каждого человека с рождения: три сосудистых подушки в стенке анального канала, которые обеспечивают тонкое удержание и дают около пятой части давления покоя. Без них полноценного удержания не было бы.
Болезнь начинается, когда удерживающая соединительная ткань с годами ослабевает и подушки начинают сползать вниз. В них застаивается кровь, они увеличиваются, кровоточат и выпадают. Этот механизм описал Томсон в 1975 году, и он лежит в основе современного понимания[2].
Отсюда практический вывод: геморрой — это симптомы, а не находка. Если подушки увеличены, но не беспокоят, лечить их не нужно. Гипердиагностика здесь встречается часто.
Внутренний, наружный и комбинированный
В анальном канале есть граница — зубчатая линия. Она определяет всё.
Выше линии — внутренние узлы. Их покрывает слизистая без болевых рецепторов. Поэтому они кровоточат и выпадают, но не болят. И поэтому их можно обрабатывать амбулаторно, без анестезии.
Ниже линии — наружные узлы. Их покрывает кожа с полной чувствительностью. Они болят резко, особенно при тромбозе.
Комбинированные — оба компонента сразу. В моей практике это самый частый вариант, и именно он определяет выбор метода.
Боль для неосложнённого внутреннего геморроя не характерна. Если болит, ищем тромбоз, анальную трещину или абсцесс, а не списываем на геморрой.
Четыре стадии
Внутренние узлы делят по классификации Goligher. Стадию определяет один критерий: выпадает ли узел и вправляется ли.

Классическая операция
Стадию нельзя определить по рассказу пациента или по фотографии. Нужна аноскопия — короткий осмотр трубочкой, который занимает минуту и делается прямо на консультации.
Кровь из прямой кишки — это не автоматически геморрой. Рекомендации ASCRS 2024 предписывают: пациентам с факторами риска — возраст, случаи колоректального рака в семье, потеря веса, железодефицитная анемия, изменение стула — до лечения геморроя нужно полное обследование толстой кишки[1]. Тёмная кровь, перемешанная с калом, — отдельный сигнал: для геморроя характерна ярко-красная кровь на бумаге или поверх кала.
Что ещё прячется под словом «геморрой»
С той же жалобой ко мне приходят пациенты, у которых на самом деле анальная трещина, свищ, выпадение прямой кишки, кондиломы или кожные бахромки, которые люди часто называют геморроем. Кожные бахромки (skin tags) — не геморрой: они остаются после перенесённого тромбоза и сами не исчезают.
Поэтому первый визит всегда начинается с осмотра, а не с рецепта.
Когда лечить, а когда нет
Лечим симптомы, а не картинку. Если узлы есть, но не кровят, не выпадают и не мешают — делать ничего не нужно. Если есть кровотечение, выпадение, зуд или выделения, которые влияют на повседневную жизнь, начинаем с малого и поднимаемся выше только тогда, когда это не помогло.
Большинство пациентов до операции вообще не доходит. При I и II стадии обычно достаточно режима и амбулаторной процедуры, занимающей несколько минут.
Частые вопросы
Может ли геморрой пройти сам?Острый эпизод может стихнуть, и на фоне режима и клетчатки симптомы часто уходят. Но ослабленная соединительная ткань не восстанавливается, поэтому без лечения проблема обычно возвращается, а стадия с годами растёт.
Всегда ли нужна операция?Нет. Большинству пациентов с I и II стадией достаточно режима и амбулаторной процедуры. Операция нужна в основном при IV стадии и большом наружном компоненте.
С чего начать?С консультации и аноскопии — без неё стадию определить нельзя. При факторах риска или нетипичном кровотечении сначала исключаем патологию толстой кишки.

Haemorrhoids: why they develop and when they need treatment
Haemorrhoids are the most common proctological complaint and the most misunderstood one. Some patients are treated for something that needs no treatment. Others are treated for haemorrhoids when the cause is different. Here I explain what happens in the anal canal, how the grade is determined, and what tells you it is time to see a doctor.
Haemorrhoids are not varicose veins
Haemorrhoidal cushions are a normal anatomical structure. Everyone is born with them: three vascular cushions in the wall of the anal canal that provide fine sealing and account for roughly a fifth of resting anal pressure. Without them, continence would be incomplete.
Disease begins when the connective tissue holding these cushions weakens with age and they start sliding downwards. They become congested, enlarge, bleed and prolapse. Thomson described this mechanism in 1975, and it underpins the modern understanding[2].
The practical conclusion: haemorrhoids are symptoms, not a finding. If the cushions are enlarged but cause no trouble, they need no treatment. Overdiagnosis here is common.
Internal, external and combined
There is a border inside the anal canal — the dentate line. It determines everything.
Above the line — internal haemorrhoids. Covered by mucosa without pain receptors. They bleed and prolapse but do not hurt. That is why they can be treated in the office without anaesthesia.
Below the line — external haemorrhoids. Covered by fully innervated skin. They hurt sharply, especially when thrombosed.
Combined — both components at once. In my practice this is the most frequent pattern, and it is what dictates the choice of method.
Pain is not typical of uncomplicated internal haemorrhoids. If it hurts, we look for thrombosis, an anal fissure or an abscess rather than blaming haemorrhoids.
Four grades
Internal haemorrhoids are graded using the Goligher classification. One criterion decides the grade: whether the pile prolapses, and whether it goes back.

Conventional surgery
The grade cannot be established from a description or a photograph. Anoscopy is required — a one-minute examination done during the consultation itself.
Rectal bleeding does not automatically mean haemorrhoids. The ASCRS 2024 guidelines state that patients with risk factors — age, family history of colorectal cancer, weight loss, iron-deficiency anaemia, change in bowel habit — need full colonic evaluation before haemorrhoids are treated[1]. Dark blood mixed with stool is a separate warning: haemorrhoidal bleeding is bright red, on the paper or on top of the stool.
What else hides behind the word “haemorrhoids”
Patients arrive with the same complaint but turn out to have an anal fissure, a fistula, rectal prolapse, condylomas or skin tags, which people often call haemorrhoids. Skin tags are not haemorrhoids: they remain after a past thrombosis and do not resolve on their own.
This is why the first visit always begins with an examination, not a prescription.
When to treat and when not to
We treat symptoms, not pictures. If the piles are there but do not bleed, prolapse or bother you, nothing needs doing. If there is bleeding, prolapse, itching or discharge affecting daily life, we start small and escalate only when that fails.
Most patients never reach surgery. Grades I and II are usually resolved with lifestyle measures and an office procedure lasting a few minutes.
Frequently asked questions
Can haemorrhoids resolve on their own?An acute episode can settle, and symptoms often disappear with diet and fibre. But weakened connective tissue does not regenerate, so without treatment the problem usually returns and the grade increases over the years.
Is surgery always necessary?No. For most patients with grade I and II disease, lifestyle measures and an office procedure are enough. Surgery is mainly needed for grade IV and for a large external component.
Where do I start?With a consultation and anoscopy — the grade cannot be determined without it. If risk factors or atypical bleeding are present, we first rule out colonic pathology.
